Impingement of the deep surface of the subscapularis tendon and the reflection pulley on the anterosuperior glenoid rim: a preliminary report.

Sixteen patients underwent detailed arthroscopic evaluation; all had moderate to severe, primarily unexplained shoulder pain provoked by anterior elevation and internal rotation, and all were nonresponsive to subacromial injection of local anesthetic. None of the patients had any symptoms or signs of instability. Partial subscapularis lesions were documented in 10 of 13 patients who had undergone preoperative arthro-magnetic resonance imaging. At arthroscopy, an isolated lesion of the common humeral insertion of the superior glenohumeral and coracohumeral ligaments (a so-called pulley lesion)

Biomechanical effect of medial advancement of the supraspinatus tendon. A study in cadavera.

During the repair of some rotator-cuff tears, the torn tendon cannot be freed up adequately to permit reattachment at its original anatomical site of insertion. An option is to advance the site of insertion medially and reattach the tendon to a trough in the sulcus or to the humeral head. The biomechanical effects of such medial advancement on the moment arm of the supraspinatus muscle during glenohumeral elevation were studied in ten fresh-frozen shoulders from cadavera. Medial advancement of the site of insertion of the supraspinatus tendon was simulated by the placement of suture anchors in…

MR imaging of the rotator cuff: peritendinous and bone abnormalities in an asymptomatic population.

OBJECTIVE: MR imaging of the shoulder was performed in an asymptomatic population to determine the prevalence of MR-evident peritendinous an … nous shoulder abnormalities among asymptomatic individuals. The prevalence of subacromial spurs and humeral head cysts correlated closely with the severity of MR-evident rotator cuff abnormalities, as did changes in the bursa and peribursal fat. Acromioclavicular joint osteoarthrosis is seen in many shoulders independently of rotator cuff disease; therefore, i …

[Impingement syndrome of the shoulder. Clinical data and radiologic findings].

Subcoracoid impingement syndrome pain is elicited by some positions of the upper limbs, i.e., adduction and inward rotation, whenever coracohumeral space reduces. Although acquired or congenital malformations of the humeral head and/or coracoid apophysis are the most common causes of painful syndromes, repeated flections and inward rotations of the upper limbs, typical of some sports, such as swimming and tennis, and of some sports, such as swimming and tennis, and of some kinds of work, are predisposing factors. The subcoracoid impingement syndrome exhibits on pathogenomonic signs at clinics …

Autotraction stress roentgenography for demonstration anterior and inferior instability of the shoulder joint.

A form of stress roentgenography is presented for the diagnosis of anterior and inferior shoulder instabilities. Stress is induced by anterioinferior traction concentrated on the shoulder during extension of the flexed hip, when patients hold their legs on the side being examined, with both hands. The anterior and inferior shifts of the humeral head in relation to the glenoid fossa then are measured from anteroposterior and modified axillary roentgenograms. This method was employed in 16 shoulders with recurrent anterior dislocation, 11 recurrent anterior subluxations, nine multidirectional in…

BACKGROUND: Proximal humeral fractures represent 4%-5% of all fractures, with increasing frequencies in the geriatric population due to cond … more effective for treating anterior dislocations. The Deltopectoral approach had a higher risk of humeral head necrosis, likely due to extensive soft tissue disruption, whereas the deltoid split approach better preserved blood supply, reducing risk of avascular necrosis. Comparison of Clinical and Radiographic Outcomes Between Central and Lateral Lesions After Osteochondral Autograft Transplantation for Osteochondritis Dissecans of the Humeral Capitellum.

BACKGROUND: Clinical studies have reported satisfactory results after osteochondral autograft transplantation (OAT) for central lesions of unstable osteochondritis dissecans (OCD) of the elbow. However, the outcomes after OAT for lateral lesions remain unclear. HYPOTHESIS: The clinical outcomes of OAT would be better for central lesions than for lateral lesions. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: The authors retrospectively evaluated 103 juvenile athletes (mean age, 13.2 years; range, 11-16 years) who underwent OAT for symptomatic OCD of the humeral capitellum from 2

Long-term follow-up after latissimus dorsi transfer for irreparable posterosuperior rotator cuff tears.

BACKGROUND: Irreparable posterosuperior rotator cuff tears are treated in several ways. Transfer of the latissimus dorsi is an alternative with acceptable mid-term results, but long-term results have rarely been published. METHODS: The cases of 108 consecutive patients with 115 shoulders treated with latissimus dorsi transfer between 2000 and 2005 were reviewed clinically and radiographically. Ninety-three shoulders in eighty-six patients were included in the follow-up analysis. The mean duration of follow-up was 9.3 years (range, 6.6 to 11.7 years), and the mean age at the operation was fift

[Multiple fractures following seizures in a pregnant woman].

A 27-year-old woman, immigrated from Libanon, pregnant and at term was admitted with pain in the lower abdomen. The history was badly recognized because of the patient's inability to speak Danish. X-ray revealed a fracture of the left acetabulum with a minor central dislocation of the femoral head. During a new seizure she had another fracture of the right acetabulum also with a minor central dislocation of the femoral head and an undisplaced fracture of the proximal humerus on the left side. She was then delivered by a caesarean section. The seizures were later found to be due to grand mal ty

Open arthrolysis for elbow stiffness increases carrying angle but has no impact on functional recovery.

BACKGROUND: With the exception of normal anatomic changes in the medial collateral ligament and radial head, other factors related to carrying angle changes have not been systematically studied. We reviewed patients who underwent open arthrolysis of the elbow, and evaluated if open arthrolysis could change carrying angle. We then identified factors associated with carrying angle changes. METHODS: Fifty patients with a minimum of 24 months of follow-up after open arthrolysis were evaluated retrospectively. Preoperative and postoperative carrying angles were compared. RESULTS: The carrying ang
