Histopathology and immunohistochemistry of depigmented lesions in lupus erythematosus.

BACKGROUND: Depigmented lesions (DL) have been described in areas previously damaged by inflammation in lupus erythematosus (LE). In the absence of typical lesions, distinction with other achromic diseases might be challenging. We studied the histological features and the behavior of melanocytes in these lesions. METHODS: Tissue sections of 12 patients with lupus and DL were stained with hematoxylin-eosin, periodic acid-Schiff and Fontana-Masson. Melanocytes were counted by immunohistochemistry methods using Melan A and HMB-45. Ten biopsies of normal skin were used as controls. RESULTS: The

Repigmentation by outer-root-sheath-derived melanocytes: proof of concept in vitiligo and leucoderma.

BACKGROUND: Treatment of depigmented skin is an unmet medical need. OBJECTIVE: Melanocytes or stem cells thereof residing in the outer root sheath (ORS) of hair follicles might be used to repigment skin. METHODS: After de-epidermisation, autologous ORS cell solutions were applied to 5 patients with vitiligo and 1 with leucoderma. RESULTS: Stable repigmentation in a variable percentage was documented in all the patients. CONCLUSION: Applying ORS-derived melanocytes is a promising technology to improve autologous melanocyte transplantation.

Ultrastructural findings in progressive macular hypomelanosis indicate decreased melanin production.

BACKGROUND: The pathogenesis of progressive macular hypomelanosis (PMH) is unknown. Recently, Westerhof et al. (Arch Dermatol 2004; 140: 210 … e I, II and III transferred melanosomes, as seen in healthy skin of skin type I to IV. CONCLUSION: Hypopigmentation in PMH seems to be the result of an altered melanogenesis based on a decrease in melanin formation and a change in the distribution of melanosomes. In lesional skin of PMH patients with skin type V and VI less melanized, aggregated melanosomes in st …

A pilot randomized control trial of scar repigmentation with UV light and dry tattooing.

OBJECTIVE: This study assessed the utility of focused scar injury with an ink-free tattooing technique combined with UV light exposure for the treatment of hypopigmented cervicofacial scars. STUDY DESIGN: Pilot prospective, randomized, control trial. SUBJECTS: Individuals with long-standing hypopigmented cervicofacial scars. METHODS: Twenty individuals randomly underwent UVA light exposure with or without dry tattooing. Scars were evaluated pretreatment and at seven-months post-treatment. Outcome measures included observer and patient subjective assessment of the scar, and objective evaluat

Genetic aspects of neurocutaneous disorders.

Among the conditions that are included under the heading of "neurocutaneous disorders" are neurofibromatosis 1, tuberous sclerosis complex, von Hippel-Lindau, incontinentia pigmenti, Sturge-Weber syndrome, hypomelanosis of Ito, and linear nevus sebaceous syndromes. The clinical features, pathogenesis, and neurobiological basis of some of these disorders are discussed in other articles in this issue. We will focus on genetic aspects of a selected subgroup of these conditions, concentrating on the genetic defect, mutation spectrum, clinical genetic testing, and issues pertinent to counseling.

Phylloid pattern of pigmentary disturbance in a case of complex mosaicism.

Among the various types of pigmentary disturbances associated with mosaicism, the phylloid pattern (Greek phyllon = leaf, eidos = form) is characterized by multiple leaf-like patches reminiscent of an art nouveau painting. The number of cases displaying this unusual pattern is so far limited. We describe a phylloid pattern of hypomelanosis in a 3-year-old girl with multiple congenital anomalies including microcephaly, midfacial hypoplasia, cleft lip, coloboma, posteriorly rotated ears, pectus carinatum, and pronounced mental and physical retardation. In addition, this child had oval or oblong

Onchocerciasis presenting with lower extremity, hypopigmented macules.

Onchocerciasis, or river blindness, is a parasitic infection caused by the filarial nematode, Onchocerca volvulus. It infects 18 million people worldwide, but is rarely seen in the United States. It is one of the leading causes of blindness in the developing world. Although onchocerciasis is also known as river blindness, it is not just a disease of the eyes, but rather a chronic multisystem disease. Clinically, onchocerciasis takes three forms: 1) eye disease; 2) subcutaneous nodules; and 3) a pruritic hypopigmented or hyperpigmented papular dermatitis. We present an 18-year-old African femal

Molecular basis of congenital hypopigmentary disorders in humans: a review.

Many specific gene products are sequentially made and utilized by the melanocyte as it emigrates from its embryonic origin, migrates into specific target sites, synthesizes melanin(s) within a specialized organelle, transfers pigment granules to neighboring cells, and responds to various exogenous cues. A mutation in many of the respective encoding genes can disrupt this process of melanogenesis and can result in hypopigmentary disorders. Following are examples highlighting this scenario. A subset of neural crest derived cells emigrate from the dorsal surface of the neural tube, become committ

Quantifying functional vision in a mouse model of oculocutaneous albinism type 1.

Oculocutaneous albinism type 1 (OCA1) is characterized by lack of melanin pigment due to mutations in the tyrosinase gene. Lack of ocular pigment results in retinal developmental anomalies and profound visual deficits. Murine OCA1 models share genotypic and phenotypic similarities with human patients, including loss-of-function tyrosinase mutations that result in systemic hypopigmentation and retinal dysgenesis. Isolated visual deficits have been identified in OCA1 mice, including photophobia and impaired depth perception; however, the full scope of OCA1 visual dysfunction has yet to be invest…

In-Depth Investigation into the Neurotoxic and Neuroglioma Risks of 6PPD Exposure in Zebrafish Larvae: Benchmark Dose Analysis, Molecular Mechanisms, and Adult Pathological Effects.

As an antioxidant widely used in tire rubber, (1,3-dimethylbutyl)-N'-phenyl-p-phenylenediamine (6PPD) has been recognized as an emerging contaminant with unclear neurotoxic potential. Using zebrafish (Danio rerio) as a model, we systematically evaluated the neurodevelopmental, behavioral, and molecular effects of 6PPD at environmentally relevant (5 μg/L) and sublethal (300 μg/L) concentrations. 6PPD exposure caused developmental abnormalities, including microphthalmia, exophthalmos, cardiac malformations, and hypopigmentation. Notably, benchmark dose (BMD) modeling identified the eye as a sens…

UV induces common cutaneous amyloid-like melanosomal protein aggregates.

Misfolding of aggregation-prone proteins underpins diseases known as proteinopathies. One of these proteins, alpha-synuclein, is a component … llular aggregate removal contributes to melanocyte senescence, which conversely exacerbates chronic hypopigmentation and photoaging-related discoloration. These findings identify melanosomal proteinopathy as a common contributor to melanocyte dysfunction and suggest aggregate-focused management approaches.
