Recovery and Return to Sport in Pediatric Osteochondral Defects of the Knee: Evaluating the Influence of Lesion Size.

BACKGROUND: Osteochondral defects (OCDe) of the knee in pediatric patients can significantly impact long-term joint function. This study aimed to evaluate the impact of lesion size on patient characteristics, treatment decisions, and outcomes. HYPOTHESIS: We hypothesized that larger lesions would be associated with worse outcomes and delayed return-to-sport (RTS), and that surgical modalities would vary by lesion size. METHODS: This IRB-approved retrospective study included 112 patients surgically treated for knee OCDe between January 2012 and December 2023 at an academic tertiary care cente

Does thePenicillin Allergy Label Affect Outcomes of Complicated Odontogenic Infections?

PURPOSE: Penicillins are a potent antibiotic in managing odontogenic infections, but 10% of the population is labelled as allergic to these drugs. This has limited their use and resulted in increased utilization of health care resources as well as complications associated with alternative antibiotics. The purpose of the study was to measure the association between patients labeled as penicillin allergic and treatment outcomes in a sample of patients treated for complicated odontogenic infections. Additionally, we sought to investigate antibiotic resistance patterns in these patients. MATERIAL

Open Versus Arthroscopic Treatment of Chronic Lateral Epicondylitis and Worker's Compensation.

PURPOSE: To compare the short-term outcomes between arthroscopic and open procedures for the treatment of lateral epicondylitis. Because a significant portion of patients have worker's compensation (WC), the outcomes in these subgroups were separately analyzed. METHODS: A retrospective analysis of patients who had surgical treatment of lateral epicondylitis by a single surgeon from 2010 to 2017 was performed. Patients who were symptomatic after 9 months of conservative treatments including nonsteroidal medications, steroid injections, and physical therapy were offered surgical intervention. T

Treatment of unresectable skull base meningiomas with somatostatin analogs.

OBJECT: The standard surgical treatment for meningiomas is total resection, but the complete removal of skull base meningiomas can be difficult for several reasons. Thus, the management of certain meningiomas of the skull base--for example, those involving basal vessels and cranial nerves--remains a challenge. In recent reports it has been suggested that somatostatin (SST) administration can cause growth inhibition of unresectable and recurrent meningiomas. The application of SST and its analogs is not routinely integrated into standard treatment strategies for meningiomas, and clinical studie

Comorbidity Trajectories Before and After Hip Fracture in Elderly Patients: A Korean Nationwide Database Cohort Study.

BACKGROUND: Hip fractures in older adults often signal frailty and are associated with poor outcomes, including mortality and disability. Although a high burden of comorbidities is common, little is known about how these conditions evolve before and after the fracture event. Understanding comorbidity trajectories can inform preventive and rehabilitative strategies. METHODS: We analyzed data from the Korean National Health Insurance Service-Senior cohort between 2005 and 2016. A total of 8,319 patients aged ≥ 65 years with surgically treated hip fractures were matched 1:2 with 16,638 controls

The association between allostatic load and lymphedema in breast cancer survivors.

PURPOSE: Allostatic load, a measure of physiological dysregulation secondary to chronic exposure to socioenvironmental stressors, is associated with 30-day postoperative complications and mortality in patients with breast cancer. This study aimed to examine the association between allostatic load (AL) at diagnosis and development of breast cancer-related lymphedema (BCRL). METHODS: Patients aged 18 years or older who received surgical treatment for stage I-III breast cancer between 2012 and 2020 were identified from The Ohio State University Cancer Registry. AL was calculated using biomarkers

Association of Homelessness with Before Medically Advised Discharge After Surgery.

BACKGROUND: Before medically advised (BMA) discharge, which refers to patients leaving the hospital at their own discretion, is associated with higher rates of readmission and death in other settings. It is not known if housing status is associated with this phenomenon after surgery. METHODS: We identified all admitted adults who underwent an operation by one of 11 different surgical services at a single tertiary care hospital between January 2013 and June 2022. Chi-square tests and t-tests were used to compare demographic and clinical features between BMA discharges and standard discharges.

Time-Dependent Risk of Atrial Fibrillation in Patients With Primary Aldosteronism After Medical or Surgical Treatment Initiation.

[Figure: see text]. Increased risk of atrial fibrillation was reported in patients with primary aldosteronism. However, data are limited regarding the time-dependent risk of atrial fibrillation in surgically or medically treated primary aldosteronism. From the National Health Insurance Claim database in Korea (2003–2017), a total of 1418 patients with primary aldosteronism (adrenalectomy [ADX], n=755, mineralocorticoid receptor antagonist n=663) were age- and sex-matched at a 1:5 ratios to patients with essential hypertension (n=7090). Crude incidence of new onset atrial fibrillation was 2.96%

Trigger Finger Treatment: Identifying Predictors of Nonadherence and Cost.

BACKGROUND: Evidence-based practices in medicine are linked with a higher quality of care and lower health care cost. For trigger finger, identifying patient factors associated with nonadherence to evidence-based practices will aid physicians in treatment decisions. The objectives were to (1) determine patient factors associated with treatment nonadherence, (2) examine the success rates of steroid injections, and (3) evaluate the economic consequences of nonadherence to treatment recommendations. METHODS: The authors used data from the Clinformatics DataMart database from 2010 to 2017 to cond

Moderating effects of immunosuppressive medications and risk factors for post-operative joint infection following total joint arthroplasty in patients with rheumatoid arthritis or osteoarthritis.

OBJECTIVE: Inconclusive findings about infection risks, importantly the use of immunosuppressive medications in patients who have undergone large-joint total joint arthroplasty, challenge efforts to provide evidence-based perioperative total joint arthroplasty recommendations to improve surgical outcomes. Thus, the aim of this study was to describe risk factors for developing a post-operative infection in patients undergoing TJA of a large joint (total hip arthroplasty, total knee arthroplasty, or total shoulder arthroplasty) by identifying clinical and demographic factors, including the use o

Evaluating components of dental care utilization among adults with diabetes and matched controls via hurdle models.

BACKGROUND: About one-third of adults with diabetes have severe oral complications. However, limited previous research has investigated dental care utilization associated with diabetes. This project had two purposes: to develop a methodology to estimate dental care utilization using claims data and to use this methodology to compare utilization of dental care between adults with and without diabetes. METHODS: Data included secondary enrollment and demographic data from Washington Dental Service (WDS) and Group Health Cooperative (GH), clinical data from GH, and dental-utilization data from WD

Clinical outcomes of extended venous thromboembolism prophylaxis after oncologic colorectal surgery.

BACKGROUND: Current guidelines recommend extended venous thromboembolism (VTE) prophylaxis after colorectal surgery for cancer. However, adherence to these guidelines has been low. Moreover, the effectiveness of extended VTE prophylaxis has not been evaluated using real-world data. METHODS: This study retrospectively analyzed a random 25% sample of the 2009-2022 IQVIA PharMetrics Plus for Academics database, an administrative claims database representative of the commercially insured population of the United States. Patients with cancer who underwent oncologic colorectal surgery were included

Income-Based Disparities in Outcomes Following Pediatric Appendectomy: A National Analysis.

BACKGROUND: Appendectomy remains a common pediatric surgical procedure with an estimated 80,000 operations performed each year. While prior work has reported the existence of racial disparities in postoperative outcomes, we sought to characterize potential income-based inequalities using a national cohort. METHODS: All non-elective pediatric (<18 years) hospitalizations for appendectomy were tabulated in the 2016-2020 National Inpatient Sample. Only those in the highest (HI) and lowest income (LI) quartiles were considered for analysis. Multivariable regression models were developed to assess
