Effect of voluntary soft tissue tension and articular conformity after total knee arthroplasty on in vivo anteroposterior displacement.

The in vivo relationship between the degree of voluntary soft tissue tension and articular conformity after total knee arthroplasty (TKA) and anteroposterior (AP) displacement was simultaneously investigated by analyzing LCS prostheses (posterior cruciate ligament-sacrificing rotating platform design) in 20 knees from 20 patients. AP displacement was measured using the KT-2000 arthrometer, at 30° and 75° flexion, while patients were conscious and under anesthesia; 30° flexion was regarded as high conformity and 75° as low conformity. Mean displacements at 30° and 75° were 5.1mm and 7.0mm, resp

Nursing assessment of injuries to the lower extremity.

When assessing injuries to the lower extremity, they can best be discussed if divided into two groups: (1) vehicular or crushing injuries; and (2) nonvehicular, torsion, or overuse injuries. Vehicular injuries require speedy, accurate assessment as they are often complex and frequently involve other systems. The patient is initially stabilized, then assessment of orthopedic problems is begun. The nurse's role is an assistive one. If the patient is admitted to the hospital, orthopedic care involves prevention of complications, reduction of pain, and assisting the patient in daily activities. No

Recurrent patellar dislocation patients with high-grade J-sign have multiple structural bone abnormalities in the lower limbs.

PURPOSE: To explore the relationship between preoperative J-sign grading and structural bone abnormalities in patients with recurrent patellar dislocation (RPD). METHODS: A retrospective study was conducted on RPD patients over 5 years. Patients were categorised based on J-sign grade into low (J- and J1+), moderate (J2+) and high groups (J3+). Trochlear dysplasia (TD) and osseous structures (femoral anteversion angle [FAA], knee torsion, tibial tuberosity-trochlear groove [TT-TG] distance, Caton-Deschamps index) were assessed and grouped according to risk factor thresholds. The χ2 test was us

Patellofemoral pain in children.

The primary inadequacy in the treatment of patellofemoral pain in children has been the inability to translate distinct problems recognized during physical examination into a specific clinical classification. Using a classification system based on etiology rather than symptoms allows the clinician to proceed with treatment based on a specific diagnosis. The majority of patellofemoral pain in children is caused by trauma and malalignment syndromes (or a combination of both) and can usually be managed successfully with nonoperative methods. Success in a rehabilitation program depends on a precis

Intraoperative Patellofemoral Kinematic Acquisition: The Design, Testing, and Validation of a Setup for Clinical Studies.

Background/Objectives: Abnormalities in patellar tracking, often overlooked in surgical planning, have been identified as a contributing factor to total knee arthroplasty (TKA) complications, including anterior knee pain, patellar subluxation, and dislocation. This study aims to evaluate the repeatability of a novel intraoperative setup for assessing patellofemoral kinematics and its interaction with prosthesis design and positioning during surgery. This setup may support personalized alignment techniques in TKA, potentially improving surgical outcomes. Methods: Kinematic data were collected u

Epidemiology of multiligament knee reconstruction.

BACKGROUND: The multiple-ligament-injured knee represents a special challenge, being an uncommon injury that is both severe and complicated to treat. Many studies have evaluated patients treated for this injury, but most are limited in their scope. The evaluation of this injury and its treatment using an administrative database might provide a different perspective. QUESTIONS/PURPOSES: Using a large administrative database, we determined (1) the number of multiligament knee reconstructions in New York State, (2) the rate of 90-day hospital readmission, and (3) the frequency of subsequent knee

[Influence of the quadriceps muscles on the patellofemoral contact in patients with low flexion patellofemoral instability after MPFL reconstruction].

INTRODUCTION: MPFL reconstruction represents one of the most important surgical treatment options for recurrent patellar dislocations at low flexion angles associated with low flexion patellofemoral instability. Nevertheless, the role of quadriceps muscles in patients with patellofemoral instability before and after patellofemoral stabilization using MPFL reconstruction has not been fully elucidated. The present study investigates the influence of quadriceps muscles on the patellofemoral contact in patients with low flexion patellofemoral instability (PFI) before and after surgical patellofemo
