Oral manifestations of internal malignancy and paraneoplastic syndromes.

Malignant tumors of visceral organs are a fundamental feature of familial cancer and paraneoplastic syndromes. In many instances, the presence of an internal and often occult malignancy may be forewarned by various external manifestations. Several of these findings are preferentially localized to the head and neck region, including the oral cavity proper. This places the dental practitioner in a unique position to detect these "markers" of occult neoplastic involvement. Because these markers may present before an established syndrome or cancer diagnosis, even representing the first expression

Jaw bone necrosis without previous dental extractions associated with the use of bisphosphonates (pamidronate and zoledronate): a four-case report.

Jaw bone necrosis is a clinical condition associated with defects in vascularization of the maxilla or the mandibular bone, usually present following head and neck radiotherapy and/or oral surgical interventions. Bisphosphonates are synthetic analogues of pyrophosphate used in the treatment of patients with hypercalcemia as a result of malignancy, bone metastasis and for the treatment of other disorders such as metabolic bone diseases, Paget's disease and osteoporosis. Over last 10 years, cases of jaw bone necrosis have been associated with the use of bisphosphonate therapy. In particular, Rug

Alcohol abuse and dependence: psychopathology, medical management and dental implications.

BACKGROUND: The authors review the clinical features, epidemiology, pathophysiology, medical management, dental findings and dental treatment of patients with alcoholism. LITERATURE REVIEWED: The authors conducted a MEDLINE search for 1995 through 2001 using the key terms of alcoholism, epidemiology, pathophysiology, treatment and dentistry. Reports selected for further review included those published in English in peer-reviewed journals. The authors gave preference to articles reporting randomized, controlled trials. CONCLUSIONS: Alcoholism is a chronic and progressive psychiatric illness t

Impacted teeth: prophylactic extractions or not?

Pathologic changes related to impacted teeth are infrequent. However, considerable damage to bone and adjacent teeth may result if they do occur. Four patients are described in whom different circumstances existed with impacted teeth. Extractions are indicated when the anatomic location of the impacted teeth is most favorable and when the patient's systemic and hematologic condition is least compromising, even if the teeth are asymptomatic at that time.

Malignant non-Hodgkin's lymphoma (NHL) of the jaws: a review of 16 cases.

Non-Hodgkin's lymphoma (NHL) is rarely found in the jaw. We present 16 cases and the purpose of this study was to analyze the clinical signs and symptoms. The treatment and the progression evolution are also mentioned. The diagnosis was usually difficult and was often misleading and so delays before the first bone biopsy were frequent. The therapy of this rare, diffuse, large cell lymphoma was very variable from one case to another but the majority of the patients were treated with a combination of chemotherapy and radiotherapy.

Histopathologic changes in soft tissue associated with radiographically normal impacted third molars.

BACKGROUND: The incidence of impacted or embedded third molars accounts for approximately 98%. Since 1948, there are studies reporting pathological changes in an asymptomatic dental follicle. Controversy still exists for removal of asmptomatic impacted teeth. Hence, this study was performed to histologically evaluate soft tissue pathosis in the pericoronal tissues of impacted third molars with pericoronal radiolucency measuring up to 2.5 mm on orthopantomographs. MATERIALS AND METHODS: Forty-one asymptomatic impacted third molars with follicular space of up to 2.5 mm on radiographs were inclu

Reconstruction of extensive composite posterolateral mandibular defects using nonosseous free tissue transfer.

BACKGROUND: The management of composite oromandibular defects involving the posterolateral mandible and surrounding soft tissue remains a reconstructive challenge. Although bony reconstitution restores continuity of the mandible, osteocutaneous flaps sometimes do not provide adequate soft-tissue coverage of these postablative defects. The purpose of this study was to evaluate the use of soft-tissue flaps for extensive posterolateral oromandibular defects. METHODS: Consecutive patients who underwent reconstruction of composite oromandibular defects following posterolateral mandibulectomy betwe

Chimaeric subscapular system free flap for complex oro-facial defects.

BACKGROUND: Ablation of locally advanced head and neck cancers generally results in large composite oro-facial defects. Due to the often-large segment of mandible missing, as well as the need to provide skin coverage and oral lining, reconstructive options are limited. We present our experience in oncologic head and neck reconstruction using chimaeric subscapular system free flaps. METHODS: We performed a retrospective chart review of patients presenting important through-and-through oro-facial defects following ablation of T3, T4a or T4b tumours in two university centres between 2005 and 201

Automatic quantification of speech intelligibility in patients after treatment for oral squamous cell carcinoma.

PURPOSE: Treatment of oral carcinomas often causes reduced speech intelligibility. It was the aim of this study to objectively evaluate the speech intelligibility of patients after multimodal therapy for oral squamous cell carcinoma (OSCC) with a computer-based, automatic speech recognition system. MATERIALS AND METHODS: The speech intelligibility of 59 patients after multimodal tumor treatment for OSCC, located at the lateral tongue, floor of the mouth, or the alveolar crest of the lower jaw, was objectively analyzed by a computer-based speech recognition system that calculates the percentag

Mandibular ameloblastoma: analysis of surgical treatment carried out in 60 patients between 1977 and 1998.

Ameloblastoma, a benign tumor of odontogenic type, represents 10% of all tumors of the jawbone. It is localized in the mandible in 80% of cases and in the upper jaw in the remaining 20%. In every case, the selection of the surgical treatment to be applied must consider some fundamental elements, including the age and general state of health of the patient, the clinicopathological variant, and the localization and extent of the tumor. In addition, it is necessary to evaluate whether the neoplasm to be treated is a primitive lesion or a recurrence. Although ameloblastoma has relative histologica

Burkitt's lymphoma involving the mandible: report of a case and review of Japanese cases.

A Japanese case of Burkitt's lymphoma is presented. The intraoral and panoramic images showed alveolar bone destruction with an infiltrative border, displacement of lower molars, root resorption, and loss of lamina dura and the follicular cortex of a developing tooth, similar to descriptions in previous reports. Computed tomography revealed that a primary lesion occupied the mandible and extended to the muscles of facial expression and to the sublingual and submandibular spaces. Another soft tissue mass was evident in the contralateral parapharyngeal space. Results of a survey of the Japanese

Point-of-care virtual surgical planning for jaw reconstruction: An update on the experience with 50 patients at a tertiary referral center.

BACKGROUND: Virtual surgical planning (VSP) has revolutionized the field of maxillofacial reconstruction. Traditionally, VSP is outsourced to an external company, which can incur considerable costs and delay surgery. Our institution has previously reported on the feasibility of point-of-care VSP (POC-VSP). Here, we present an update on the technical refinements made to our POC-VSP process, and provide an expanded series with specific endpoints including time from planning to product release, operative time, implant survival, and post-operative complications. METHODS: A retrospective cohort st

Oral rehabilitation of the cancer patient: A formidable challenge.

Rehabilitation of oral functions following surgery on the jaws is a goal that is often difficult to achieve. Removable dentures supported by remaining teeth or gum are often unstable and seldom satisfactory. On the other hand, endosseous (dental) implants offer a mechanism to provide stability to the dentures. This review, discusses factors related to the tumor, patient, treatment, and physicians which impact upon the feasibility and success of dental implants in patients with oral cancer.

Stylomandibular tunnel widening versus narrowing: a useful tool in evaluating suprahyoid mass lesions.

AIM: To evaluate whether qualitative and quantitative assessments of stylomandibular tunnel asymmetry are useful in lesion localization and differentiation. MATERIALS AND METHODS: The stylomandibular tunnel was measured in 60 control patients at CT to determine normal side-to-side variation. Twenty-one patients in the study group with suprahyoid neck masses were divided into two subgroups, those with widening and those with narrowing of the pathological side. Surgical and pathological findings in these subgroups were compared for site of origin and histology. RESULTS: Stylomandibular tunnel

Benign paediatric mandibular tumours: experience in reconstruction using vascularised fibula.

The majority of the paediatric oral and maxillofacial tumours are benign and the mandible is involved in one-third of these cases. A review of the literature reveals only a handful of studies pertaining exclusively to benign paediatric mandibular tumours. The basis of this study was to fulfil the need to assess the suitability of major mandibular reconstructions using a vascularised fibular graft in cases of benign tumours in children. From April 1999 to April 2011 we have managed 18 cases of benign paediatric mandibular tumours. All the reconstructions were done using vascularised fibular gra

Functional outcomes of fibula and osteocutaneous forearm free flap reconstruction for segmental mandibular defects.

OBJECTIVE/HYPOTHESIS: To demonstrate that the osteocutaneous radial forearm free flap provides equivalent functional outcomes and improved morbidity compared to the fibular free flap in mandibular reconstruction. STUDY DESIGN: Retrospective review. METHODS: There were 168 patients requiring free flap reconstruction of segmental mandibular defects between January 2001 and December 2008. Mean follow-up was 31 months for fibula free flap (FFF) (n = 117) and 20 months for osteocutaneous radial forearm free flaps (OCRFFF) (n = 51), reflecting an increasing use of forearms. RESULTS: OCRFFF were m

Osteosynthetic titanium mini-plate fixation of composite radial forearm flaps in mandibular reconstruction.

This paper describes the evolution and refinement, over a four-year period, of a simple technique for shaping and stabilising the bony component of a composite radial forearm free flap to reconstruct the mandible following segmental resection for neoplastic disease. The results of reconstruction in 30 consecutive patients are reported. The current method of adapting and stabilising the radial bony strut using a specially designed 32 hole osteosynthetic miniplate is described.
