Exercise following heart transplantation.

During the past 2 decades, heart transplantation has evolved from an experimental procedure to an accepted life-extending therapy for patients with endstage heart failure. However, with dramatic improvements in organ preservation, surgery and immunosuppressive drug management, short term survival is no longer the pivotal issue for most heart transplant recipients (HTR). Rather, a return to functional lifestyle with good quality of life is now the desired procedural outcome. To achieve this outcome, aggressive exercise rehabilitation is essential. HTR present unique exercise challenges. Preoper

The broadening application of chemodenervation in X-linked dystonia-parkinsonism (Part I): muscle afferent block versus botulinum toxin-A in cervical and limb dystonias.

Botulinum toxin (BoNT) is an established mainstay treatment for dystonia. However, its use, especially in developing countries, is significantly limited by its cost. Chemodenervation with muscle afferent block (MAB) using lidocaine-ethanol may provide a more cost-effective alternative to traditional BoNT injections. A study comparing MAB with BoNT type-A in cases of X-linked dystonia-Parkinsonism (XDP) having cervical dystonia indicated a modest and short-lived efficacy with MAB, while a more robust efficacy in dystonia and pain parameters, lasting up to 11 weeks, was observed in the two BoNT

Comparison of two experimental models of urodynamic evaluation in female rats.

PURPOSE: Urodynamic studies in small animals can be performed through urethral sounding or cystostomy. OBJECTIVE: To compare the two methods of urodynamic evaluation in female rats. METHODS: Ten female rats weighing on average 250 g, under anesthesia with urethane (1.25 mg/kg) were submitted in three repeats to an urethal catheter of 0,64 mm in external diameter for cystometric measurements of vesicle pressure(VP1) and contraction time (CT1). The catheter was extracted at a constant velocity of 0.05 cm/minute until complete exteriorization and determinations of maximal urethral pressure (UP1

[Function of the upper esophageal sphincter after denervation of recurrent laryngeal nerves and intramural nerves of the cervical esophagus in dogs].

The upper esophageal sphincter prevents reflux into the pharynx. If it functions improperly, aspiration pneumonia can result. We studied the functioning of the sphincter in unanesthetized dogs after denervation under anesthesia of the recurrent laryngeal nerves. The pressure of the sphincter at rest was measured by manometry with a transducer that measured pressure around the tip of a catheter. Then the pressure in response to inflation of a balloon to the diameter of 1.5, 2.0 and 2.5 cm at 5 and 10 cm aboral to the sphincter was measured. Next, the pressure during perfusion of 0.1 N HCl or Na

Vagus-dependent disruption of interdigestive canine motility by gastric distension.

The purpose of this study was to determine the effect of proximal gastric distension on interdigestive patterns of canine gastrointestinal motility and to examine the role of extrinsic nerves in regulating such an effect. Serosal electrodes were placed on the antrum, duodenum, and jejunum. Animals were studied before and after transthoracic vagotomy or after neural isolation of the entire jejunoileum (extrinsic denervation). Proximal gastric distension for 5 h was provided by inflating with air a thin complaint bag placed into the proximal stomach after the onset of phase III of the migrating

Long-term follow-up of selective sacral neurectomy.

Between 1976 and 1981, 22 patients underwent selective, complete sacral root section as a treatment for severe detrusor overactivity. Eighteen were followed up for more than 4 years (mean 6.2). Thirteen of the 22 patients had significant symptomatic improvement lasting for more than 4 years post-operatively (59%), 8 of whom had stable bladders. It was concluded that this operation still has a place in the treatment of detrusor overactivity.

Acute effects of spinal transection on EPSPs produced by single homonymous Ia-fibers in soleus alpha-motoneurons in the cat.

1. Excitatory postsynaptic potentials (EPSPs) generated in soleus motoneurons by single homonymous Ia-fibers were measured using intracellular recording and the spike-triggered averaging technique. Two groups of barbiturate-anesthetized adult cats were studied: one with the spinal cord intact and the other with the spinal cord severed at thoracic segment 13 (T13) several hours prior to recording. 2. In cord-transected cats, single homonymous Ia-fibers produced EPSPs in soleus motoneurons that were, on average, larger and faster rising relative to normal, as they are for those produced in media

Fibro-Adipogenic Progenitor Ablation Triggers Muscle Atrophy Through Cell Death-Induced Inflammation.

BACKGROUND: Skeletal muscle mass maintenance involves coordination of myofibers with mononuclear cell populations, including satellite cells, resident macrophages and fibro-adipogenic progenitors (FAPs). FAPs, identified by surface expression of PDGFRα, are important contributors to muscle homeostasis, as genetic ablation of FAPs induces rapid muscle atrophy. However, mechanisms underlying this response remain poorly understood. METHODS: We utilized tamoxifen-inducible FAP specific diptheria toxin (DTA) mice (PdgfraCre-ERT2/+; Rosa26^DTA/+) to evaluate consequences of FAP deletion, measuring

Relevance of a peripheral site of action outside the brain-blood barrier for the beneficial effects of CB2 receptor activation in experimental ALS in male mice.

BACKGROUND: Cannabinoid type 2 receptor (CB2) activation works against neurotoxic events that kill motor neurons in amyotrophic lateral sclerosis. Given that amyotrophic lateral sclerosis is a neuromuscular pathology with the skeletal muscle being also affected, we were interested in deciphering whether peripheral CB2 may contribute in these beneficial effects. METHODS: To this end, we used two CB2 agonists: (i) RO-6866945, which crosses the blood-brain barrier (BBB) and acts at both Central Nervous System (CNS) and peripheral compartments; and (ii) RO-6871304, which is a peripherally-restric

Spinal Cord Injury Versus Sciatic Nerve Injury: Divergent Neuromuscular Pathologies and Neural Circuitry Remodeling.

Spinal cord injury (SCI) and sciatic nerve injury (SNI) represent distinct neurotrauma models with different pathological outcomes. Although both injuries result in severe motor and sensory dysfunction in the lower limbs, their divergent mechanisms of muscle atrophy and neural circuit remodeling remain poorly understood. This study systematically compared electrophysiological and morphological alterations at multiple time points post-injury. At 4 weeks, SNI induced severe muscle atrophy and complete loss of compound muscle action potentials (CMAPs), whereas SCI caused mild atrophy and moderate

Conditioning Electrical Stimulation Accelerates Regeneration in Nerve Transfers.

OBJECTIVE: Compared to the upper limb, lower limb distal nerve transfer (DNT) outcomes are poor, likely due to the longer length of regeneration required. DNT surgery to treat foot drop entails rerouting a tibial nerve branch to the denervated common fibular nerve stump to reinnervate the tibialis anterior muscle for ankle dorsiflexion. Conditioning electrical stimulation (CES) prior to nerve repair surgery accelerates nerve regeneration and promotes sensorimotor recovery. We hypothesize that CES prior to DNT will promote nerve regeneration to restore ankle dorsiflexion. METHODS: One week fol

Nociception contributes to the formation of myogenic contracture in the early phase of adjuvant-induced arthritis in a rat knee.

It is unknown how joint contracture is generated in inflamed joints. This study aimed to clarify the role of nociception on the formation of joint contracture secondary to arthritis. Monoarthritis was induced by intra-articular injections of complete Freund's adjuvant (CFA) into rat knees. On day 5 after CFA injection, the passive extension range of motion (ROM) of knee joints were measured, both before and after myotomy of knee flexors, to evaluate the extent of muscular contribution to CFA-induced joint contracture. The steroidal anti-inflammatory drug dexamethasone could prevent ROM restric

Phenotypic spectrum of disorders associated with glycyl-tRNA synthetase mutations.

We describe clinical, electrophysiological, histopathological and molecular features of a unique disease caused by mutations in the glycyl-tRNA synthetase (GARS) gene. Sixty patients from five multigenerational families have been evaluated. The disease is characterized by adolescent onset of weakness, and atrophy of thenar and first dorsal interosseus muscles progressing to involve foot and peroneal muscles in most but not all cases. Mild to moderate sensory deficits develop in a minority of patients. Neurophysiologically confirmed chronic denervation in distal muscles with reduced compound mo

Surgical anatomy of the somatic terminal innervation to the anal and urethral sphincters: role in anal and urethral surgery.

PURPOSE: The gross anatomy of the pudendal nerve branches was studied to identify more precisely the neuroanatomical relationship in the region of the anal canal, bladder neck and proximal urethra. Such knowledge is essential for the development of surgical techniques that avoid nerve injury in sphincteroplasty for anal and urinary stress incontinence, and in pudendal canal decompression. MATERIALS AND METHODS: The pudendal nerve terminal branches were dissected in 7 female and 5 male formalin fixed cadavers, including 6 fully mature neonates and 6 adults, a mean age of 37.6 years. The nerves

Analysis of static and kinetic abnormalities typical of torticollis, and postoperative changes.

The clinical pattern of torticollis and surgical results were evaluated. Head posture and range of motion were measured. The authors use a newly designed device consisting of an orthogonal system to which head position is referred. Preliminary data were obtained on 24 patients with torticollis and 21 healthy control subjects. The examination of posture shows that the head usually twists in opposite directions simultaneously around a vertical and a sagittal axis, and the deflection is greater in one direction. Head position affect body posture, with the trunk often compensating for head deviati

Reconstruction of elbow flexion in arthrogryposis multiplex congenita type I: results of transfer of pectoralis major muscle with follow-up at skeletal maturity.

BACKGROUND: The purpose of this study was to analyze the results of a pectoralis major transfer to restore active elbow flexion in patients with extension elbow contracture in arthrogryposis. The hypotheses were: (1) this transfer ensures permanent useful elbow flexion; and (2) flexion elbow deformity will not progress during growth and after its cessation. METHODS: Unipolar transfer of the 3 distal parts of the pectoralis major muscle was used in 9 extremities of 5 patients (age range, 5 to 9 y; average age, 6.3 y) and the results were prospectively followed in the period of 13 to 16 years.

Effect of early intensive physical therapy on NLRP3 inflammasome activation and muscle atrophy in critical illness myopathy (PT-NLRP3-CIM): a two-centre randomized open-label study protocol.

BACKGROUND: Critical illness myopathy (CIM) is a prevalent cause of intensive care unit-acquired weakness (ICUAW). It is associated with prolonged mechanical ventilation and increased mortality and frequently evolves into postintensive care syndrome with long-term cognitive and physical impairments. Although several molecular mechanisms have been implicated in CIM, including muscle atrophy and altered contractility, its exact etiopathogenesis is not fully understood. Evidence suggests that activation of the NLRP3 inflammasome and upregulation of atrogenes significantly contribute to this condi
