A Scoping Review of Certified Nurse-Midwife and Certified Midwife Care in the United States: Assessing Outcomes Across Six Patient Care Domains.

UNLABELLED: Policy Points Certified nurse-midwife (CNM)/certified midwife (CM) care is associated with outcomes that are comparable or improved compared to physician care across multiple domains of health care quality, especially safety and effectiveness. CNM/CM care is consistently associated with lower rates of intrapartum interventions and improved birth outcomes and patient satisfaction. Integration of CNM/CM care remains limited across many US health systems due to scope of practice restrictions and institutional policies. Growing, diversifying, and integrating CNM/CM care offers a critic

Impact of a Multicomponent Intervention to Build Capacity of Public Health Workers to Make Algorithmic Diagnosis and Management of High-Risk Pregnancies in Uttar Pradesh, India: Protocol for a Matched-Control, Before-After, Quasi-Experimental Study With a Mixed Methods Design.

BACKGROUND: In India, 20%-30% of pregnancies are high-risk, contributing to 75% of perinatal mortality and morbidity. An effective approach to reduce this mortality and morbidity is early identification, effective management, and timely referral of high-risk pregnancies (HRPs). The Integrated High-Risk Pregnancy Tracking and Management program aims to enhance the capacity of auxiliary nurse midwives (ANMs), medical officers (MOs), and specialist gynecologists by (1) providing algorithmic, color-coded, detailed yet simple HRP protocols, customized for each role; (2) offering live training; (3)

Exclusively Pumping Women's Milk Production and Pumping and Feeding Dynamics-An Online Survey.

Background/Objectives: Women who exclusively pump (EP) expend more time and effort on pumping and feeding infants their expressed breast milk (EBM) than those who directly breastfeed. However, little is known about their 24 h milk production (MP) or pumping and feeding dynamics. Methods: An online survey of 195 EP women explored the experiences, characteristics, MP and pumping and feeding dynamics of EP women. Quantitative data were compared by prematurity/neonatal unit admission, milk supply and parity, and self-reported MP was compared with MP in a predominantly breastfeeding reference group

Maternal Anxiety and Breastfeeding Self-Efficacy in the Neonatal Intensive Care Unit: A Narrative Review of Their Relationship with Breastfeeding Outcomes.

Mothers of infants cared for in the neonatal intensive care unit (NICU) experience substantial psychological burden related to preterm birth, mother-infant separation, and stressors inherent to the intensive care environment. In this context, maternal anxiety and breastfeeding self-efficacy have been identified as key psychosocial factors influencing the initiation and continuation of breastfeeding. This narrative review examines the relationship between maternal anxiety and breastfeeding self-efficacy during the NICU stay and evaluates how this interaction is reflected in breastfeeding outcom

Comparison of mortality and morbidity rates of calves born from embryo transfer and artificial insemination in Hanwoo cattle.

Hanwoo cattle producers have considerable concerns about the higher incidence of perinatal mortality and morbidity in calves derived from in vitro embryo transfer (IVP-ET) than in those derived from artificial insemination (AI). This study aims to compare the mortality and morbidity rates between calves born via IVP-ET and AI in Hanwoo cattle, hypothesizing that calves conceived via IVP-ET will not exhibit higher rates compared to those conceived through AI. This study evaluated a propensity score-matched sample of 310 Hanwoo calves (155 IVP-ET and 155 AI calves) selected from an initial scree

Enhancing gestational diabetes mellitus education: a randomized controlled trial of King's theory and FMEA-PDCA integration for improved patient outcomes.

BACKGROUND: Gestational Diabetes Mellitus (GDM) is a common pregnancy complication that poses significant risks to both maternal and neonatal health, including preterm birth and macrosomia. Traditional GDM health education methods often lack clinical effectiveness due to insufficient personalized feedback and timely evaluation. This gap arises from the inadequate integration of nursing theories and quality management tools. This study explores the efficacy of combining King’s Theory of Goal Attainment with the Failure Modes and Effects Analysis (FMEA) and the Plan-Do-Check-Act (PDCA) cycle as

Sustaining the Continued Effectiveness of an Antimicrobial Stewardship Program in Preterm Infants.

BACKGROUND: There are wide variations in antibiotic use in neonatal intensive care units (NICUs). Limited data are available on antimicrobial stewardship (AS) programs and long-term maintenance of AS interventions in preterm very-low-birth-weight (VLBW) infants. METHODS: We extended a single-centre observational study carried out in an Italian NICU. Three periods were compared: I. "baseline" (2011-2012), II. "intervention" (2016-2017), and III. "maintenance" (2020-2021). Intensive training of medical and nursing staff on AS occurred between periods I and II. AS protocols and algorithms were m

Priorities to improve woman-centred gestational diabetes mellitus care: A qualitative study to compare views between clinical and consumer end-users.

BACKGROUND: Immigrants worldwide have a two-fold higher risk of gestational diabetes mellitus (GDM) than women of the host country. Providing culturally appropriate woman-centred GDM care to attenuate adverse maternal and neonatal health outcomes is a persistent challenge for health services. Underpinned by the Knowledge to Action Framework, understanding and comparing the views of patients from different ethnic backgrounds and healthcare professionals (HCPs) about current and optimal GDM care can highlight priority areas to improve woman-centred care. This qualitative study aimed to compare t

The effects of premature infant oral motor intervention (PIOMI) on oral feeding of preterm infants: A randomized clinical trial.

OBJECTIVE: Given the increase in the birth and survival rate of the premature infants, a need for supportive health care services becomes more evident. The goal of the present study was to examine the effectiveness of the Premature Infant Oral Motor Intervention (PIOMI) in the feeding progression and early intervention. This study was a double-blind randomized clinical trial. METHODS: This clinical trial included premature infants in the neonatal intensive care units (NICUs) of two hospitals in Tehran, who were randomly assigned into intervention and control group, each containing 15 infants.

Outcome of Pregnancies in Female Patients With Inflammatory Bowel Diseases Treated With Vedolizumab.

BACKGROUND AND AIMS: Vedolizumab is an IgG1 anti-α4β7 integrin antibody approved for the treatment of inflammatory bowel diseases [IBD], but without clear safety data during conception, pregnancy and nursing. Animal studies showed that mucosal vascular addressin cell adhesion molecule 1 [MAdCAM-1] is expressed by maternal vessels in the placenta and recruits α4β7-expressing cells that are considered important for maternal/fetal tolerance. Blocking this interaction by vedolizumab might affect this process. We aimed to evaluate pregnancy outcomes in vedolizumab-treated female IBD patients. METH

Exploring standardisation, monitoring and training of medical devices in assisted vaginal birth studies: protocol for a systematic review.

INTRODUCTION: Assisted vaginal birth (AVB) can markedly improve maternal and neonatal outcomes arising from complications in the second stage of labour. Historically, both forceps and ventouse devices have been used to assist birth; however, they are not without risk and are associated with complications, such as cephalohaematoma, retinal haemorrhage and perineal trauma. As new devices are developed to overcome the limitations of existing techniques, it is necessary to establish their efficacy and effectiveness within randomised controlled trials (RCTs). A major challenge of evaluating complex

Heterogeneity of Multimedia Exposures to Neurotoxic Elements (Al, As, Cd, Pb, Mn, and Hg) in Breastfed Infants from Porto Velho, Brazil.

Infant exposure to neurotoxic elements is a public health issue that needs monitoring with regard to breast milk composition. We studied six neurotoxic elements in breast milk samples at different stages of lactation in mothers from Porto Velho, Brazil. We used a flow-injection mercury system (FIMS) to determine total Hg concentrations and an inductively coupled plasma optical emission spectrometer (ICP-OES) to determine the concentrations of Al, As, Cd, Pb, and Mn in 106 donors of a human milk bank. Association rules analyses were applied to determine the pattern of binary and ternary mixture

A multicentre, randomised controlled, non-inferiority trial, comparing nasal high flow with nasal continuous positive airway pressure as primary support for newborn infants with early respiratory distress born in Australian non-tertiary special care nurseries (the HUNTER trial): study protocol.

INTRODUCTION: Nasal high-flow (nHF) therapy is a popular mode of respiratory support for newborn infants. Evidence for nHF use is predominantly from neonatal intensive care units (NICUs). There are no randomised trials of nHF use in non-tertiary special care nurseries (SCNs). We hypothesise that nHF is non-inferior to nasal continuous positive airway pressure (CPAP) as primary support for newborn infants with respiratory distress, in the population cared for in non-tertiary SCNs. METHODS AND ANALYSIS: The HUNTER trial is an unblinded Australian multicentre, randomised, non-inferiority trial.

Wide Variation Found in Care of Opioid-Exposed Newborns.

OBJECTIVE: Standardized practices for the management of neonatal abstinence syndrome (NAS) are associated with shorter lengths of stay, but optimal protocols are not established. We sought to identify practice variations for newborns with in utero chronic opioid exposure among hospitals in the Better Outcomes Through Research for Newborns (BORN) network. METHODS: Nursery site leaders completed a survey about hospitals' policies and practices regarding care for infants with chronic opioid exposure (≥3 weeks). RESULTS: The 76 (80%) of 95 respondent hospitals were in 34 states, varied in size (

Metachronous Paget's disease of the breast: case report.

BACKGROUND: Congenital Choanal Artesia (CCA) is a rare cause of upper airway obstruction, yet it is the most common congenital anomaly of the nose. While the unilateral condition could be undiagnosed, bilateral CCA may be life-threatening, especially for newborns. Some CCA may be associated with other congenital abnormalities, which leads to a systematic screening during the diagnostic assessment. The diagnosis is easy. However, surgical management is still controversial. METHODS: We conducted a retrospective study gathering data on management of CCA over 42 months. We reported epidemiologica

A randomised controlled trial of morphine versus phenobarbitone for neonatal abstinence syndrome.

BACKGROUND: The incidence of neonatal abstinence syndrome (NAS) has increased 10-fold over the last decade in Glasgow. In the Princess Royal Maternity Hospital, it now accounts for 17% of special care baby unit (SCBU) admissions. OBJECTIVE: To compare opiate replacement therapy (morphine sulphate) with the present standard treatment (phenobarbitone) for management of NAS. The primary study end point was duration of pharmaceutical treatment. Secondary end points were the requirement for additional drugs and the requirement for SCBU admission. DESIGN: Double blind, randomised controlled clinic

The effect of tolerance to noninherited maternal HLA antigens on the survival of renal transplants from sibling donors.

BACKGROUND: During pregnancy and nursing, a baby's developing immune system is intimately exposed to the mother's antigens. To determine whether this exposure is of clinical benefit to patients who later receive an allograft as an adult, we analyzed the outcome of primary renal transplantations from sibling donors. METHODS: We retrospectively studied graft survival and rejection episodes in 205 patients who had received renal transplants at nine centers between 1966 and 1996 from sibling donors bearing maternal or paternal HLA antigens not inherited by the recipient. The sibling donors were c

Transcutaneous bilirubinometry in Chinese, Malay and Indian infants.

The objective of this study was to investigate the correlation of transcutaneous bilirubinometry (TcB) and plasma bilirubin concentrations in full-term Chinese, Malay and Indian infants. TcB was performed with the Minolta Airshields bilirubinometer on Chinese, Malay and Indian fullterm infants. The readings were taken on the chest (sternum) and forehead (glabella) when capillary blood was sampled for bilirubin determination. Five hundred and forty TcB indices in 253 Chinese infants, 282 in 169 Malay infants, and 182 in 120 Indian infants were obtained over the sternum and forehead. A good corr
