Proximal row carpectomy for scapholunate dissociation.

Thirty-one patients underwent proximal row carpectomy for static scapholunate dissociation without degenerative arthritis. We report the radiographic and clinical results including wrist range of motion, grip strength, use of pain medication, pain severity and patient satisfaction. Mean postoperative flexion/extension was 74° (range 5°-150°) compared with 137° (range 70°-175°) for the uninvolved side. Mean grip strength was 22 kg for the involved side and 39 kg for uninvolved side. Fifteen of 31 patients reported moderate or severe pain after proximal row carpectomy, requiring wrist arthrodesi

Percutaneous screw fixation for unstable scaphoid fractures.

BACKGROUND: The technique of percutaneous screw fixation, with increasing popularity, has been successfully conducted in non-displaced, stable scaphoid fractures resulting in shortened immobilization duration and prompt functional retrieval. The purpose of this study was to evaluate the surgical technique and to explore the potential benefits of using percutaneous screw fixation in unstable scaphoid fractures. METHODS: Eleven of 97 patients with scaphoid fractures surgically treated between 1994 and 2002 were enrolled in this study. All were acute unstable fractures and underwent closed reduc

[Dorsal Screw Osteosynthesis of a Scaphoid Nonunion associated with Congenital Scaphotrapezial and Lunotriquetral Coalitions: A Case Report].

We present the case of a 24-year-old man who sustained a scaphoid fracture in the presence of congenital scaphotrapezial and lunotriquetral coalitions. As the fracture progressed to a nonunion, a screw osteosynthesis was performed. The altered biomechanics caused by the two coalitions necessitated a dorso-proximal surgical approach, and two screws were implanted to prevent rotational instability. The scaphoid fracture healed entirely.

Functional Morphology of the Scaphoid in Extant African Apes, Humans and Fossil Hominins.

OBJECTIVES: The morphology of the hominoid scaphoid has played a key role in functional and evolutionary hypotheses related to the emergence of hominin bipedalism and tool use. However, the scaphoid's complex morphology is challenging to comparatively analyze via traditional 2D linear measurements. This study quantifies scaphoid morphology utilizing 3D geometric morphometrics (3D GM) in African apes, humans and extinct hominins to provide a more holistic functional understanding of scaphoid morphology. MATERIALS AND METHODS: We use 3D GM, including anatomical and sliding semi-landmarks, to qu

Magnetic resonance imaging analysis of the distribution of cartilage damage in scaphoid nonunion.

This study aimed to evaluate the distribution and severity of cartilage damage using magnetic resonance imaging in patients with scaphoid nonunion without advanced wrist arthritis. We retrospectively analysed MRI of patients who underwent osteosynthesis for scaphoid nonunion. Cartilage damage was assessed in eight wrist regions using a modified Whole-Organ Magnetic Resonance Imaging Score. The frequencies of regions affected by any cartilage damage (Score ≥2.5) depicted on MRI were analysed. The study included 32 patients (31 men and one woman), with a mean age of 32 years (SD 11). The distal

Imaging assessment of dorsal scaphoid displacement in patients with scapholunate ligament tears: what is the best option for quantitative assessment?

OBJECTIVE: To compare four different methods for the quantitative assessment of dorsal scaphoid displacement in patients with scapholunate ligament tears. METHODS: A total of 160 consecutive patients who underwent CT arthrography to evaluate a suspected scapholunate ligament tear were prospectively included in this study approved by the local ethics committee. MR images were available for 65 of these patients. Two readers independently evaluated the dorsal scaphoid displacement on conventional radiographs with the dorsal tangential line (DTL) method, the posterior radioscaphoid angle (PRSA) o

Complications of distal radial and scaphoid fracture treatment.

Fractures of the distal radius and the scaphoid are common injuries in adults. In recent years, surgical fixation of these types of fractures has increased in response to improved patient outcomes and evolving fixation techniques. Potential soft-tissue, neurovascular, or osseous complications, including tendon injuries, carpal tunnel syndrome, loss of fracture reduction, and osteonecrosis, can increase the time the patient requires immobilization and can lead to poor patient outcomes. Prompt recognition and diagnosis of these complications may improve patient outcomes and satisfaction.

Avascular necrosis of the scaphoid in children treated by splint immobilisation: a report of two cases.

Avascular necrosis of the scaphoid following a fracture in children is rare and there is no established treatment protocol in the literature. We present two boys with nonunion and avascular necrosis of the scaphoid treated by simple immobilisation. Both cases healed with painless wrists and full movements. Our cases confirm that an ununited scaphoid fracture in children may heal with conservative treatment, even when an MRI scan suggests avascular necrosis. Unlike in adults, operative treatment need only be considered in children when conservative treatment fails.

Scaphoid nonunion in break-dancers: a report of 3 cases.

Most injuries resulting from break-dancing are minor, such as sprains and strains, but there is great potential for dance participants to sustain severe and life-threatening conditions, such as cervical cord injuries. Break-dancing carries many of the risks of conventional dance and gymnastics, but unlike those forms of activity, it is usually performed without trained supervision, as is supplied by coaches in other sports. It is necessary for clinicians to inquire more thoroughly into the nature of the activities that result in both unusual and common injuries in break-dancers and to educate

Dorsal transscaphoid transtriquetral perilunate dislocation with pseudoarthrosis of the scaphoidy.

All perilunate fracture-dislocations combine ligament ruptures, bone avulsions, and fractures in a variety of clinical forms. Two mechanisms of the injury may result in a carpal dislocation: direct and indirect. Most dorsal perilunate dislocations are the result of an indirect mechanism of injury, usually consisting of an extreme extension of the wrist joint, associated with variable degree ulnar deviation and radiocarpal or midcarpal supination. Many factors may explain the occurrence of one or another type of injury: age, differences in direction and magnitude of the deforming forces, and di

Distal scaphoid resection arthroplasty for scaphoid nonunion with radioscaphoid arthritis.

The authors compare the functional outcome of 8 patients treated by distal scaphoid resection arthroplasty for scaphoid nonunion with symptomatic wrist arthritis before surgery and after a minimum follow-up of 6 months. There was a significantly better range of radial deviation and grip strength at the time of re-examination. Significantly fewer patients complained of resting pain. By contrast the change of radiolunate angle measured before the surgery and at the time of re-examination was statistically insignificant. Our preliminary results show that distal scaphoid resection arthroplasty see

Age dependent T2 changes of bone marrow in pediatric wrist MRI.

OBJECTIVE: Hyperintensity of the bone marrow on fluid-sensitive sequences can be seen on magnetic resonance imaging (MRI) during childhood, even in the absence of bone pathology. They can be related to hematopoietic marrow, normal and abnormal bone remodeling. We sought to investigate whether hyper intensity of the bone marrow on MRI of the wrist is age-dependent and to evaluate if this signal follows a consistent age-related pattern. MATERIALS AND METHODS: Thirty-one wrist 1.5 T MR images of children (7-18 years) without suspected bone pathology were evaluated for foci of hyperintense bone m

Multifocal giant cell tumor of the carpus: Unusual presentation. Case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Giant cell tumors (GCTs) of bone in the carpus are rare. Carpal GCTs are usually solitary lesions; multifocal involvement is exceptional. These lesions have a higher risk of local recurrence after intralesional curettage than those in other body areas. CASE PRESENTATION: We present a case of a 28-year-old male with a six-month history of a palpable mass in the dorsal aspect of the left wrist. Physical examination revealed a 2 cm, mildly tender mass. Magnetic resonance revealed a large intermediate signal lesion involving completely hamate bone and the distal porti
