Is Regional Lymph Node Metastasis of Head and Neck Paraganglioma a Sign of Aggressive Clinical Behavior: A Clinical/Pathologic Review.

BACKGROUND: Head and neck paraganglioma is a rare neoplasm of the paraganglia. It accounts for <1% of all head and neck tumors. It usually has benign clinical course; however, malignant paraganglioma can only be diagnosed by showing metastatic disease. We undertook a retrospective study to assess the clinical significance of regional lymph nodes metastases in head and neck paragangliomas. DESIGN: From 1993 to 2016, primary head and neck paragangliomas are identified. The patient clinical and histopathologic materials were reviewed. RESULTS: Sixty-five specimens from 62 patients (3 patients w

Mucoepidermoid carcinoma of the parotid gland with invasion of the jugular foramen region: A case report.

RATIONALE: Mucoepidermoid carcinoma of the parotid gland, being the most prevalent malignant neoplasm among salivary gland tumors, represents the leading incidence rate among primary parotid malignancies. This pathological entity predominantly affects young and middle-aged females, typically presenting as a painless parotid mass. Due to its variable disease progression and nonspecific clinical manifestations, preoperative misdiagnosis as a benign tumor is frequently encountered. However, when parotid mucoepidermoid carcinoma progresses to involve the jugular foramen region, a complex anatomica

Schwannoma of the trochlear nerve-an illustrated case series and a systematic review of management.

Schwannomas of cranial nerves in the absence of systemic neurofibromatosis are relatively rare. Among these, schwannomas of the trochlear nerve are even less common. They can be found incidentally or when they cause diplopia or other significant neurological deficits. Treatment options include observation only, neuro-ophthalmological intervention, and/or neurosurgical management via resection or sterotactic radiosurgery (SRS). In recent years, the latter has become an attractive therapeutic tool for a number of benign skull base neoplasm including a small number of reports on its successful us

Clivus metastases: report of seven patients and literature review.

BACKGROUND: Tumours of the clivus are rare and metastases involving this area have been previously described only as single case reports or included in series with other skull base tumours. Here, we describe seven such examples and review the pertinent literature. METHOD: Clinical, radiological and follow-up data of patients who had undergone surgery for clivus tumours at our Institution between January 1995 and December 2007 were retrospectively collected. A literature review was performed using PubMed. FINDINGS: Of 46 patients who underwent surgery for clivus bone tumours, seven proved to

[Dedifferentiated chordoma of the skull base. A case report].

Sarcomatous transformation in chordoma is a very rare condition and has been emphasized as a distinct entity because of its more aggressive clinical course. Here we describe a case of dedifferentiated chordoma arising from the skull base region of an 11-year-old boy, with tumor recurrence within one year. This tumor showed features of pleomorphic cell sarcoma with areas more typical of chordoma. Most of tumor cells expressed cytokeratin, epithelial membrane antigen, vimentin and S-100 protein, thus confirming the diagnosis of dedifferentiated chordoma.

The Clinical outcomes following pencil beam scanning proton therapy for skull base chordoma and chondrosarcoma: a single-institution experience.

BACKGROUND: Chordomas and chondrosarcomas of the skull base are rare, locally aggressive tumors that pose significant therapeutic challenges due to their proximity to critical anatomical structures. While maximal safe surgical resection remains the cornerstone of management, the infiltrative nature of these tumors often precludes radical excision, necessitating adjuvant radiotherapy. Given their relative radioresistance, high-dose radiotherapy exceeding 70 Gray Relative Biological Effectiveness (GyRBE) is required to achieve durable local control. This study evaluated long-term outcomes and to

The skull base in NF1 phenotypes on lateral cephalograms.

Neurofibromatosis type 1 (NF1) influences skeletal growth processes. We investigated the effect of the disease and associated neurogenic craniofacial tumors on the skull base. We examined skull base parameters on lateral cephalograms in patients with (N = 74) facial plexiform neurofibroma (FPNF) and 92 without and the same number of age- and sex-matched controls. The measurements were analyzed for mean differences and certain parameters (sex, age (< or ≥ 18 years), tumor type). The application of the skull base angle as a diagnostic parameter was explored. We found statistically significant el

Nasogastric Tube Feeding into Brain After Endoscopic Endonasal Transsphenoidal Surgery.

The endoscopic endonasal transsphenoidal (EET) approach for skull base tumors has become increasingly popular. We know that bone defects in the skull base can cause cerebrospinal fluid rhinorrhea, but for patients who need to be intubated through the nose, the tube can enter the brain through a skull base bone defect. Nasogastric tube feeding into the brain is a rare occurrence, and this situation can occur only in the case of a skull base defect. We treated a patient with an unusual complication after the EET approach for pituitary adenoma resection. This particular case suggests that bone de

Clinical Features, Management Considerations and Outcomes in Case Series of Patients with Parasellar Intracranial Aneurysms Undergoing Anterior Skull Base Surgery.

BACKGROUND: The coincidence of parasellar aneurysms (IAs) and anterior skull base (ASB) lesions, while rare, presents a management challenge. IAs embedded within, or adjacent to, ASB lesions are at risk in the perioperative period and may have unique presentations, natural histories, and outcomes. The objective of this study was to outline management options and nuances in patients with coexisting IAs and ASB lesions. METHODS: We retrospectively evaluated all patients who presented for management of IAs and ASB lesions from January 2006 to January 2014. Medical charts and imaging were reviewe

Immediate complications associated with high-flow cerebrospinal fluid egress during endoscopic endonasal skull base surgery.

Endoscopic expanded endonasal approaches (EEAs) to the skull base are increasingly being used to address a variety of skull base pathologies. Postoperative CSF leakage from the large skull base defects has been well described as one of the most common complications of EEAs. There are reports of associated formation of delayed subdural hematoma and tension pneumocephalus from approximately 1 week to 3 months postoperatively. However, there have been no reports of immediate complications of high-volume CSF leakage from EEA skull base surgery. The authors describe two cases of EEAs in which compl

Cavernous internal carotid artery aneurysm after radiotherapy presenting with external ophthalmoplegia.

Cranial radiotherapy could cause several types of vasculopathies, which include atherosclerotic occlusive diseases, moyamoya disease, and aneurysm formation. To our knowledge, radiation-induced aneurysms of the internal carotid artery (ICA) are extremely rare. Here, we report a 68-year-old woman who presented with external ophthalmoplegia caused by radiotherapy after the transsphenoidal surgery for metastastic tumor of the clivus region, and the angiography demonstrated a giant aneurysm of the cavernous ICA. After the ICA ligation, the patient recovered well without brain ischemia with a 6-mon

The lion's mane sign: surgical results using the bilateral fronto-orbito-nasal approach in large and giant anterior skull base meningiomas.

OBJECT: Concerns about extreme peritumoral edema and its ensuing surgical and perioperative complications led the authors to use the bilateral fronto-orbito-nasal approach to remove midline anterior skull base meningiomas that were larger than 4 cm. The authors hypothesize that extreme vasogenic edema exemplified by finger-like hyperintensities extending into the bifrontal white matter and external capsule and/or the extreme capsule, coined the lion's mane sign (LMS), would help identify patients with a challenging postoperative course. They hypothesize that the LMS would better predict sympto

Giant anterior clinoidal meningiomas: surgical technique and outcomes.

OBJECT: Surgery for giant anterior clinoidal meningiomas that invade vital neurovascular structures surrounding the anterior clinoid process is challenging. The authors present their skull base technique for the treatment of giant anterior clinoidal meningiomas, defined here as globular tumors with a maximum diameter of 5 cm or larger, centered around the anterior clinoid process, which is usually hyperostotic. METHODS: Between 2000 and 2010, the authors performed 23 surgeries in 22 patients with giant anterior clinoidal meningiomas. They used a skull base approach with extradural unroofing o

Surgical treatment of dumbbell-shaped hypoglossal schwannoma via a pure endoscopic transoral approach.

BACKGROUND: Dumbbell-shaped hypoglossal schwannomas with intradural and extradural extension are extremely rare, and complete removal of these tumors is very difficult. This report describes such lesions in three patients that were completely removed via a purely endoscopic transoral approach. METHOD: Three patients with intradural and extradural growth hypoglossal schwannomas (three women, aged 16, 42 and 43 years) were treated by direct surgery via a purely endoscopic transoral approach to the posterior fossa. RESULTS: In this series, radical resections of the dumbbell-shaped hypoglossal s

Diplopia as a presenting symptom in a gastric gastrointestinal stromal tumor.

Gastrointestinal stromal tumors are the most common mesenchymal neoplasm of the gastrointestinal tract. Distant metastasis of gastrotintestinal stromal tumors occurs in ∼50% of the cases and is usually found in the liver and peritoneum. We present a patient with diplopia which was due to a metastatic gastrointestinal stromal tumor of the clivus. Transsphenoidal resection of the tumor was performed and post-operative treatment with oral imatinib mesylate was done. One month after the surgery, treatment was started with imatinib and the patient's diplopia improved within 15 days. Follow-up compu
