Developmental changes in activation and effective connectivity in phonological processing.

The current study examined developmental changes in activation and effective connectivity among brain regions during a phonological processing task, using fMRI. Participants, ages 9-15, were scanned while performing rhyming judgments on pairs of visually presented words. The orthographic and phonological similarity between words in the pair was independently manipulated, so that rhyming judgment could not be based on orthographic similarity. Our results show a developmental increase in activation in the dorsal part of left inferior frontal gyrus (IFG), accompanied by a decrease in the dorsal p

16p11.2 deletion syndrome.

The 16p11.2 BP4 and BP5 region, is a recurrent ∼600kb copy number variant (CNV), and deletions are one of the most frequent etiologies of neurodevelopmental disorders and autism spectrum disorder with an incidence of approximately 1/2000. Deletion carriers have delays in early neurodevelopment that most specifically impair speech, phonology and language in 70%. Intelligence quotient is shifted 1.8 standard deviations lower than family controls without the deletion. Other common neurobehavioral conditions include motor coordination difficulties (60%) and autism (20-25%). Unprovoked seizures are

The MECP2 duplication syndrome.

In this review, we detail the history, molecular diagnosis, epidemiology, and clinical features of the MECP2 duplication syndrome, including considerations for the care of patients with this X-linked neurodevelopmental disorder. MECP2 duplication syndrome is 100% penetrant in affected males and is associated with infantile hypotonia, severe to profound mental retardation, autism or autistic features, poor speech development, recurrent infections, epilepsy, progressive spasticity, and, in some cases, developmental regression. Most of the reported cases are inherited, however, de novo cases have

Brainstem dysgenesis: report of five patients with congenital hypotonia, multiple cranial nerve involvement, and ocular motor apraxia.

This paper reports three females and two males with a distinctive congenital syndrome characterized by severe congenital hypotonia, facial diplegia, jaw ankylosis, velo-pharyngeal incoordination, pyramidal tract signs, and ocular motor apraxia. Patients were followed up at ages ranging from 20 months to 16 years. All cases of this syndrome are sporadic, without dysmorphological features, chromosomal, or MRI brain abnormalities. Electrophysiological studies indicate the brainstem as the site of the neurological dysfunction. Post-mortem CNS study of one of the patients demonstrated neuronal depl

[Two cases of MERRF (myoclonus epilepsy associated with ragged red fibers) showing different clinical features in the same family].

A 73-year-old woman (patient 1) developed progressive mental deterioration at age 63, and seizures at age 70. On examination, she showed severe dementia, tonic clonic convulsion, hypotonia and muscular wasting. There was neither myoclonus nor cerebellar ataxia. Brain CT revealed a low density area in the right occipital lobe. A 44-year-old man (son of the patient 1) developed unsteady gait at age 15, muscle twitching at age 18 and then noticed speech disturbance at age 35. He had no history of convulsive seizure. Neurological examination showed cerebellar ataxia, myoclonus in the extremities a

[Oral and written affective expression in children of low socioeconomic status].

Descriptive data on affective expression of 58 children (33 girls and 25 boys) of low socioeconomic status (Graffar index), with ages between 8 and 12 are presented. Intelligence was assessed by means of Raven Progressive Matrixes Test, all subjects exhibiting mean level. Evaluated were the six forms of anxiety and the four hostility forms defined by the Gottschalk method of verbal content analysis. Hope scores, positive and negative, were also obtained from the same verbal samples. The oral sample consisted in speech produced spontaneously during 5 minutes, in response to a standard instructi

Applications of artificial intelligence to aid early detection of dementia: A scoping review on current capabilities and future directions.

BACKGROUND & OBJECTIVE: With populations aging, the number of people with dementia worldwide is expected to triple to 152 million by 2050. Seventy percent of cases are due to Alzheimer's disease (AD) pathology and there is a 10-20 year 'pre-clinical' period before significant cognitive decline occurs. We urgently need, cost effective, objective biomarkers to detect AD, and other dementias, at an early stage. Risk factor modification could prevent 40% of cases and drug trials would have greater chances of success if participants are recruited at an earlier stage. Currently, detection of dementi

Real-Time Conversation Comprehension in Younger and Older Adults: The Effects of Noise, Visual Speech, Speaker Switch, and Context.

This study investigated older and younger adults' conversational speech understanding using an established topic monitoring task to assess real-time semantic processing. Twenty-three younger adults (mean age = 26.5 years) and 19 older adults (mean age = 71.3 years) monitored for topic targets in 24 dialogues (2-3 minutes each). The impact on monitoring response time and accuracy of two message-level factors (when speakers switch; contextual predictability) was assessed while manipulating two signal-level factors (presentation modality; noise level). Results showed that presentation modality af

Associations between the medial olivocochlear reflex, middle-ear muscle reflex, and sentence-in-noise recognition using steady and pulsed noise elicitors.

The middle-ear muscle reflex (MEMR) and medial olivocochlear reflex (MOCR) modify peripheral auditory function, which may reduce masking and improve speech-in-noise (SIN) recognition. Previous work and our pilot data suggest that the two reflexes respond differently to static versus dynamic noise elicitors. However, little is known about how the two reflexes work in tandem to contribute to SIN recognition. We hypothesized that SIN recognition would be significantly correlated with the strength of the MEMR and with the strength of the MOCR. Additionally, we hypothesized that SIN recognition wou

Associations between speech recognition at high levels, the middle ear muscle reflex and noise exposure in individuals with normal audiograms.

It has been hypothesized that noise-induced cochlear synaptopathy in humans may result in functional deficits such as a weakened middle ear muscle reflex (MEMR) and degraded speech perception in complex environments. Although relationships between noise-induced synaptic loss and the MEMR have been demonstrated in animals, effects of noise exposure on the MEMR have not been observed in humans. The hypothesized relationship between noise exposure and speech perception has also been difficult to demonstrate conclusively. Given that the MEMR is engaged at high sound levels, relationships between s

Spurring Innovation in AAC Technology through Collaborative Dreaming and Needs Finding with Individuals with Developmental Disabilities Who Use AAC.

Millions of individuals who have limited or no functional speech use augmentative and alternative communication (AAC) technology to participate in daily life and exercise the human right to communication. While advances in AAC technology lag significantly behind those in other technology sectors, mainstream technology innovations such as artificial intelligence (AI) present potential for the future of AAC. However, a new future of AAC will only be as effective as it is responsive to the needs and dreams of the people who rely upon it every day. AAC innovation must reflect an iterative, collabo

AI enabled sign language recognition and VR space bidirectional communication using triboelectric smart glove.

Sign language recognition, especially the sentence recognition, is of great significance for lowering the communication barrier between the hearing/speech impaired and the non-signers. The general glove solutions, which are employed to detect motions of our dexterous hands, only achieve recognizing discrete single gestures (i.e., numbers, letters, or words) instead of sentences, far from satisfying the meet of the signers' daily communication. Here, we propose an artificial intelligence enabled sign language recognition and communication system comprising sensing gloves, deep learning block, a

A case report: Autosomal recessive microcephaly caused by a novel mutation in MCPH1 gene.

Autosomal Recessive Primary Microcephaly (MCPH-MIM 251200) is distinguished by congenital decrease in occipito-frontal head circumference (OFC) of at least 2 standard deviations (SD) below population average in addition to non-progressive mental retardation, without any prominent neurological disorder. Mutations in MCPH1, which encodes the protein microcephalin have been detected in this disorder. Here we report a consanguineous Iranian family with 2 children affected with microcephaly. Despite the severe mental retardation observed in the male patient, the female patient had normal intelligen
