Semon Lecture: 'Laryngectomy Practice Based on Personal Research', Royal Society of Medicine, 5 November 2020, London, UK.

I wish to thank the Semon Committee for inviting me to deliver the 2020 Semon lecture. This is a very special honour, as is evidenced by the list of distinguished lecturers dating back to the inaugural lecture delivered at University College London in 1913. I am not the first South African to deliver the Semon lecture, having been preceded by my previous chairman Sean Sellars in 1993, and by Jack Gluckman in 2001, who was South African raised and educated and who subsequently became the chairman of otolaryngology in Cincinnati, USA.

A Word of Caution: Watch and Wait in the Total Mesorectal Excision Era.

BACKGROUND: Watch and wait strategies offer organ preservation for selected patients with rectal cancer following neoadjuvant chemoradiotherapy (nCRT). Long-term outcomes following nCRT and total mesorectal excision (TME) provide important context for evaluating these approaches. We report regional surgical, pathological, and long-term oncological outcomes following nCRT and TME, with oncological outcomes stratified by treatment response. METHODS: A multicentre retrospective cohort study included patients with rectal cancer treated with curative-intent nCRT followed by TME across four Western

Guideline-concordant antibiotic prophylaxis on surgical site infection rates following colorectal surgery: real-world data from Thailand.

PURPOSE: Antibiotic prophylaxis (AP) is a key component in preventing surgical site infections (SSIs). The existing gap between clinical guidelines and real-world practice may influence SSI rates. This study aimed to compare SSI rates between patients receiving guideline-concordant AP based on the 2020 Thailand guideline (group A) and those receiving broader-spectrum and/or prolonged prophylaxis (group B). METHODS: We reviewed a database of adult patients who underwent elective colorectal resection between January 2022 and April 2024 at the largest university hospital in Thailand. SSIs were d

Low Chimney Cutaneous Ureterostomy (LCCU): a novel technique for supra-vesical urinary diversion.

BACKGROUND: Supra-vesical urinary diversion is rarely needed in current pediatric urology practice. Developments in reconstructive surgery, pediatric endoscopy, and anesthesia have promoted early definitive management of majority of indicated congenital obstructive uropathies. Ureterostomy is an effective alternative for decompressing upper urinary tract; however, none of current techniques provide an optimal diversion. End ureterostomy (EU) and low-loop ureterostomy (LLU) do not allow urinary bladder cycling especially in bilateral cases and those with solitary kidneys. EU cannot drain the ur

Role of endoscopic third ventriculostomy in children in a high-burden resource-limited setting: A prospective study.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is an established alternative to ventriculoperitoneal shunting for pediatric hydrocephalus, but its success in younger children remains variable, particularly in resource-limited settings. METHODS: We conducted a prospective observational study (July 2019-June 2024) at a tertiary pediatric surgical center in Northern India, including children aged 3-60 months undergoing ETV. Demographics, etiology, intraoperative findings, complications, and outcomes were analyzed. Primary outcome was 6-month ETV success (alive, clinically well, shunt-free).

Evaluating the Impact of Robotic Ileal Pouch-anal Anastomosis: A Case-matched Analysis From a High-volume Center.

OBJECTIVE: To compare robotic-assisted proctectomy with ileal pouch-anal anastomosis (R-IPAA) outcomes and laparoscopic proctectomy with IPAA within a specialized robotic surgery center, using matching techniques to minimize potential confounding factors. BACKGROUND: Minimally invasive approaches, particularly laparoscopy, have improved outcomes for patients with inflammatory bowel disease and familial adenomatous polyposis undergoing IPAA. Robotic-assisted surgery offers potential technical advantages, but its definitive superiority over laparoscopy in this context remains under debate. MET

Management of obstructed colorectal carcinoma in an emergency setting: An update.

Colorectal carcinoma is common, particularly on the left side. In 20% of patients, obstruction and ileus may be the first clinical manifestations of a carcinoma that has advanced (stage II, III or even IV). Diagnosis is based on clinical presentation, plain abdominal radiogram, computed tomography (CT), CT colonography and positron emission tomography/CT. The best management strategy in terms of short-term operative or interventional and long-term oncological outcomes remains unknown. For the most common left-sided obstruction, the first choice should be either emergency surgery or endoscopic

Permanent cutaneous vesicostomy: a pragmatic approach to safely manage lower urinary tract dysfunction in pediatric patients with chronic and life-limiting conditions and neuropathic bladders.

INTRODUCTION: Lower urinary tract dysfunction (LUTD) in cerebral palsy (CP) and other neuromuscular diseases can present with chronic retention that leads to hydronephrosis, recurrent urinary tract infections (UTI), and stone formation. Whenever the conservative treatment of LUTD fails for any reason, it is considered to be complicated LUTD, in which a surgical approach is warranted. Cutaneous vesicostomy (CV) is a simple, well-tolerated, and potentially reversible procedure that protects the upper tracts. We describe our experience using CV for this complex population. MATERIALS AND METHODS:

Case Report: Esophageal squamous cell carcinoma in a 13-year-old boy with a history of esophageal atresia with tracheoesophageal fistula.

In adults, esophageal cancers are a global health concern. Esophageal squamous cell carcinoma (ESCC) accounts for approximately 90% of esophageal carcinomas. The prognosis of esophageal cancers remains dismal, with a five-year survival rate below 20%. It typically affects older patients, and for now, ESCC after esophageal atresia has not been reported in patients younger than 18 years. We present an exceptional case of an ESCC in a 13-year-old boy with a history of esophageal atresia and corrective surgery in infancy. After the surgery the patient was lost to surgical follow up for over ten ye

Emergency surgical treatment of colonic acute diverticulitis: a multicenter observational study on behalf of the Italian society of colorectal surgery (SICCR) Lombardy committee.

Colonic diverticulitis can be treated conservatively, but some cases require surgery. Patients can undergo Hartmann's procedure (H) or resection with primary anastomosis (RA), with or without diverting stoma. This multicenter observational retrospective study aims to evaluate the adherence to current guidelines by assessing the rate of RA and H in Lombardy, Italy, and to analyze differences in patients' features. This study included data collected from nine surgical units performing emergency surgery in Lombardy, in 2019 and 2021. Data for each year were retrospectively collected through a sur

A Single-Center Comparative Study of Open Transabdominal and Laparoscopic Transanal Ileal Pouch-Anal Anastomosis with Total Mesorectal Excision. Has the Bar Been Raised?

BACKGROUND: Most ulcerative colitis (UC) patients requiring surgery undergo transabdominal ileal pouch-anal anastomosis (IPAA) performed minimally invasively or open. Although one multicenter study demonstrated acceptably low morbidity after transanal pouch, our initial single-center experience with transanal IPAA (ta-IPAA) was associated with an unacceptably high rate of anastomotic leak. The aim of this study was to compare the short-term outcomes of ta-IPAA and transabdominal IPAA with growing experience of transanal proctectomy and determine whether one approach offered any advantage or be
