Aberrant Right Subclavian Artery: A Case Report.

During routine dissection of a medical cadaver, an aberrant right subclavian artery (ARSA) was identified originating from the aortic arch. An ARSA, present in 0.2-2.5% of the population and often asymptomatic, increases the risk of surgical nerve injury during head, neck, and thoracic surgeries. ARSAs can also impinge on the esophagus and cause dysphagia or odynophagia, impinge on the trachea and cause dyspnea, or be impinged by the esophagus and/or trachea, limiting blood supply to the upper extremity. Most often, ARSAs follow a retroesophageal course, as in this case. In addition to the ARS

Neurologic complication after a roller coaster ride.

Neurologic complications after roller coaster rides are uncommon but potentially catastrophic. Physicians should have a high index of suspicion and prompt appropriate investigation. A 22-year-old healthy African American man presented with a 2-day history of constant occipital headache associated with vertigo, nausea, vomiting, and ambulatory dysfunction. Physical examination showed gait ataxia, slight dysmetria, and vertical nystagmus. Magnetic resonance imaging (MRI) of the brain showed early subacute ischemic infarct in the right cerebellum in the distribution of the right posterior inferio

Anatomical characteristics of two cases of aberrant right subclavian artery.

An aberrant right subclavian artery is a branching variation of the aortic arch. We encountered two female cadavers with an aberrant right subclavian artery during routine student dissection at our school. In both cases, the right subclavian artery was not a branch of the brachiocephalic trunk but originated directly from the distal part of the aortic arch as the last branch and ran between the esophagus and vertebral column, traveling to the upper limb. The right recurrent laryngeal nerve was absent, but a non-recurrent inferior laryngeal nerve branching from the vagus and traveling directly

Some variations of the circle of Willis, important for cerebral protection in aortic surgery--a study in Eastern Europeans.

BACKGROUND: During unilateral selective cerebral perfusion (SCP), via cannulation of the brachiocephalic trunk, the brain receives blood only through the right common carotid artery and the right vertebral artery. For perfusion of the contralateral (left) hemisphere it is counted on the competence of the circle of Willis (CoW). It is well known that variations of CoW are present in more than 50% of the people. Furthermore, these variations usually affect more than one vessel of the circle. The aim of the present work was to study the variations of CoW, which could have an impact on cerebral bl

Vessel wall inflammation in spontaneous cervical artery dissection: a prospective, observational positron emission tomography, computed tomography, and magnetic resonance imaging study.

BACKGROUND AND PURPOSE: Vessel wall inflammation (VWI) may be a pathogenetic factor in cervical artery dissection (CAD). We used contrast-enhanced high-resolution MRI (hrMRI) and positron emission tomography CT (PET-CT) to systematically investigate VWI in spontaneous CAD. METHODS: In this monocentric, prospective, observational study, all consecutive patients with acute, MRI-confirmed, spontaneous CAD admitted to our center between August 2007 and August 2009 were included. VWI was defined as perivascular contrast enhancement in hrMRI and increased perivascular [18F]-fluorodesoxyglucose upta

Endovascular treatment of ruptured posterior circulation dissecting aneurysms.

BACKGROUND: and aim: Dissecting aneurysms of the posterior circulation constitute a relatively uncommon subgroup of aneurysms. They account for 3-7% of cases of nontraumatic subarachnoid hemorrhage. Because of high risk, in most cases the patients require surgical or endovascular therapy. In this study we discuss the clinical efficacy of endovascular treatment with long-term follow-up in ruptured dissecting aneurysms of the posterior circulation. MATERIALS AND METHODS: This retrospective study was conducted at our institution between January 1995 and June 2005. Eight patients (4 male; 4 femal

Dissecting giant vertebro-basilar aneurysms.

OBJECT: Vertebro-basilar dissecting aneurysms (VDAs) are very rare in children. Strokes or subarachnoid hemorrhage are characteristic clinical manifestations, but often only non-specific symptoms are found. Radiological diagnosis may be difficult to obtain and therapy hard to perform. We report on a child who presented with an intracranial mass effect due to a giant VDA. METHODS: The patient was a 8-year-old girl with right hemiparesis, ataxia, dysphagia and dysphonia and worsening of her neurological deficits. Neuroimaging discovered a giant dissecting aneurysm arising from the left vertebra

Clinical application of high-resolution MRI in combination with digital subtraction angiography in the diagnosis of vertebrobasilar artery dissecting aneurysm: An observational study (STROBE compliant).

Vertebrobasilar artery dissecting aneurysm (VBA-DA) is associated with serious complications and poor prognosis in patients. High-resolution magnetic resonance imaging (HR-MRI) is a noninvasive method for the diagnosis of VBA-DA.VBA-DAs were classified according to the feature of HR-MRI in combination with digital subtraction angiography (DSA), and the clinical outcomes of different types of VBA-DAs were analyzed. Thirty-nine patients with 42 VBA-DAs were included and underwent HR-MRI, including three-dimensional T1 weighted image, three-dimensional T2 weighted image (3D-T2WI), three-dimension

Peritoneal shunt migration into the pulmonary artery--case report.

A 48-year-old man underwent ventriculoperitoneal shunting for hydrocephalus secondary to subarachnoid hemorrhage due to left vertebral artery dissection, which had been successfully treated by trapping. The peritoneal catheter was correctly positioned via a right upper abdominal incision, and symptoms related to the hydrocephalus disappeared. One month later, the patient began to complain of pain on the right side of the neck. Chest radiography revealed that the peritoneal end of the catheter had migrated into the right pulmonary artery. The catheter route was explored through a small neck inc…

Spontaneous retroclival hematoma presenting as a thunderclap headache. Case report.

An excruciating headache of instantaneous onset, or thunderclap headache, may be caused by a variety of serious disorders, including aneurysmal subarachnoid hemorrhage, cerebral venous sinus thrombosis, pituitary apoplexy, and carotid artery or vertebral artery dissection. The authors describe a patient with this type of headache who was found to have a spontaneous retroclival hematoma. A 49-year-old woman experienced an instantaneous excruciating headache. Results of computerized tomography (CT) scans of the head were normal, but on examination of the cerebrospinal fluid xanthochromia was fou…

Posterior circulation aneurysms. Results of direct operations.

Twenty-eight patients with aneurysms of the posterior circulation were managed by direct operations, 53.6% of these being done within 14 days of a subarachnoid hemorrhage. Transvenous cardiac pacing was used in 4 of the 13 patients with aneurysms of the basilar bifurcation in order to produce a short period of profound hypotension while final dissection and clip application was performed. There were nine patients with posterior-inferior cerebellar artery/vertebral-junction aneurysms, the only one dying having a large aneurysm. The operative technique used in the majority of the 13 patients wit

Management of the left subclavian artery during aortic arch replacement using a frozen elephant trunk approach: a review.

The frozen elephant trunk (FET) technique for total aortic arch replacement extends repair into the proximal portion of the descending thoracic aorta. Several techniques and modifications of total arch replacement have been described in the literature, and many of these iterations are related to facilitating the distal anastomosis while preserving flow to the left subclavian artery (LSCA), as well as maintaining posterior circulation of the brain via the vertebral artery, by reducing the circulatory arrest time during reconstruction. Because of the LSCA's posterior and deep anatomic location i

Evaluating the Role of Imaging Markers in Predicting Stroke Risk and Guiding Management after Vertebral Artery Injury: A Retrospective Study.

BACKGROUND AND PURPOSE: Traumatic vertebral artery injury (TVAI) is a severe consequence of head and neck trauma occurring when a disturbance to vessel wall architecture causes vessel dissection. This injury comes with a host of complications, including debilitating neurologic damage, though patients are often asymptomatic upon presentation. Still, the screening recommendations, treatment strategies, and predicted outcomes of TVAI remain largely undefined. Therefore, it is imperative to investigate the risk of life-threatening sequelae occurring secondary to vertebral artery injury to minimize

[Cardiovascular manifestations of Horton disease: an underestimated disease in cardiology].

Horton's disease is a giant cell arteritis well known for its presentation as temporal arteritis. It is, in fact, a systemic disease which affects over 1% of the general populations after 50 years of age. With the exception of the risk of blindness by occlusion of the ophthalmic artery, the cardiovascular manifestations of Horton's disease are not well known and probably underestimated by clinicians. The main complications are involvement of the large arteries, especially the thoracic aorta and subclavian and axillary arteries, the femoro-popliteal axis and supra-aortic arterial vessels. Durin
