Vestibular and oculomotor disturbances caused by industrial solvents.

Hydrocarbon solvents, xylene, styrene, methylchloroform, and trichlorethylene, given intravenously to rabbits produce a positional nystagmus. Due to their additional influence on rotatory nystagmus, one may conclude that their mode of action takes place in the central nervous system. Optokinetic (OKN) responses in rabbits, cats, and humans were also influenced by styrene. OKN responses have not yet been tested for the other solvents. A comparison is made with the effects of alpha-chloralose and the GABA-antagonists bicuculline and picrotoxin which produced similar disturbances. The GABA agonis

The Campomelic syndrome. Temporal bone histopathologic features and otolaryngologic manifestations.

The campomelic syndrome is characterized by dwarfism, craniofacial anomalies, bowing of the tibiae and femora, cutaneous dimpling overlying the tibial bend, respiratory distress, and early death. Otolaryngologic manifestations include flat facies with a broad nasal bridge, low-set ears, cleft palate, mandibular hypoplasia, and tracheobronchial malacia. The underlying pathologic feature appears to be disturbance in cartilage growth involving the affected bones and the respiratory tract cartilage. The cause is unknown. We report clinical and histopathologic features in two cases of this syndrome

An in vitro model system to study gene therapy in the human inner ear.

The confined fluid-filled labyrinth of the human inner ear presents an opportunity for introduction of gene therapy reagents designed to treat hearing and balance dysfunction. Here we present a novel model system derived from the sensory epithelia of human vestibular organs and show that the tissue can survive up to 5 days in vitro. We generated organotypic cultures from 26 human sensory epithelia excised at the time of labyrinthectomy for intractable Meniere's disease or vestibular schwannoma. We applied multiply deleted adenoviral vectors at titers between 10(5) and 10(8) viral particles/ml

Pathogenesis of Meniere's disease and Meniere's syndrome.

Meniere's disease can only be studied in patients, since it does not occur spontaneously in animals nor can it be induced in them. However, aspects of the disease such as endolymphatic hydrops can be usefully studied in animals. A study of the natural history (epidemiology) of Meniere's disease demonstrated the three major symptoms (triad) to be vestibular symptoms, auditory symptoms, and aural pressure. Bilaterality occurs in at least one out of 3 patients, and may approach 50% over full lifespans. Aural pressure (74.1%) was common, as was positional vertigo (85.9%) during and/or between atta

Sudden hearing loss: a clinical survey.

After a brief statistic survey of general aspects the variety of clinical findings in sudden hearing loss is demonstrated by numerous observations. These illustrate the questionable role of supposed causes, the different modes of onset and course of sudden hearing loss, the audiometric findings and concomitant symptoms. The situation of both ears prior to the acute loss seems to be of great importance and offers a frame for classification, which is of clinical relevance. These features and others are discussed with regard to common theories on the etiology of sudden hearing loss. The difficult

Assessment of auditory and vestibular function and gene therapy in the Snell's waltzer mouse model of human deafness and balance dysfunction.

The Snell's waltzer mouse (Myo6sv/sv) serves as a model for human deafness and vestibular behavioral impairment, caused by a spontaneous 130 bp recessive deletion in the Myo6 gene. In this study, we characterized the auditory and vestibular phenotypes of Myo6sv/sv mice. These mice exhibit profound hearing loss, with cochlear hair cell stereocilia beginning to fuse soon after birth, ultimately leading to disorganization of hair bundles and degeneration of hair cells. Mice also exhibit behavioral phenotypes characterized by severe imbalance, hyperactivity with bouts of circling, and delayed spat

Increased Adrenergic Receptor Expression in Meniere Disease.

HYPOTHESIS: There is increased expression of adrenergic receptors in the vestibular end organs of Meniere disease. BACKGROUND: Meniere disease (MD) is an inner ear condition characterized by episodic vertigo, hearing loss, tinnitus, and aural fullness. It is believed to be due to an imbalance in inner ear fluid, often leading to hydrops; however, the exact mechanism is unclear. Pupillometry data have suggested a role for autonomic dysfunction, specifically sympathetic hyperactivity, but few studies have investigated the molecular pathways involved in this. METHODS: Vestibular end organs from

Vestibular neuromodulation for chronic insomnia after traumatic brain injury: a case series.

OBJECTIVE: Sleep dysfunction is common after traumatic brain injury (TBI) and can be difficult to manage due to medication side effects and complex neuropsychiatric comorbidities. Noninvasive electrical vestibular system stimulation (VSS) is an emerging neuromodulation therapy that has demonstrated benefit for primary chronic insomnia in adults without known brain injury, but has not been described for chronic insomnia in individuals with TBI. RESULTS: We present a retrospective case series of 5 adult veterans with chronic TBI and moderate-to-severe insomnia (Insomnia Severity Index [ISI] ≥15

Electrical stimulation of the dorsolateral prefrontal cortex inhibits vestibular signalling in humans: A BOLD fMRI study.

BACKGROUND: Low-frequency sinusoidal galvanic vestibular stimulation (sGVS) can induce perceptions of sway and nausea through entraining vestibular afferent firing to the sinusoidal stimulus. As recently shown, concurrent dorsolateral prefrontal cortex (dlPFC) stimulation via transcranial alternating current stimulation (tACS) greatly attenuates these vestibular perceptions. OBJECTIVE: Given that both vestibular afferents and dlPFC efferents project to the insular cortex, it was reasoned that the insula is the most likely area for the top-down inhibitory interaction to take place. METHODS: T

Non-additive effects of electrical stimulation of the dorsolateral prefrontal cortex and the vestibular system on muscle sympathetic nerve activity in humans.

Sinusoidal galvanic vestibular stimulation (sGVS) induces robust modulation of muscle sympathetic nerve activity (MSNA) alongside perceptions of side-to-side movement, sometimes with an accompanying feeling of nausea. We recently showed that transcranial alternating current stimulation (tACS) of the dorsolateral prefrontal cortex (dlPFC) also modulates MSNA, but does not generate any perceptions. Here, we tested the hypothesis that when the two stimuli are given concurrently, the modulation of MSNA would be additive. MSNA was recorded from 11 awake participants via a tungsten microelectrode in

Exploration of effects of galvanic vestibular stimulation on circadian rhythms and its associations with sleep and spatial memory in patients with breast cancer: The ICANSLEEP-2 protocol.

BACKGROUND: Patients with breast cancer (BC) exhibit circadian rhythm disruptions, mainly of rest-activity rhythm (RAR), of which sleep is an essential component, and cortisol rhythm. Sleep complaints such as insomnia and cognitive impairments are prevalent in BC. In general population, sleep is known to contribute greatly to cognition. Thus, improving RAR (and particularly sleep) could help limiting cognitive impairments in BC patients. It has recently been suggested that, in addition to its essential role in spatial memory, the vestibular system contributes to RAR synchronization. Its stimul

Using principal component analysis to determine which vestibular stimuli provide best biomarkers for separating Alzheimer's from mixed Alzheimer's disease.

Alzheimer's disease (AD) is often mixed with cerebrovascular disease (AD-CVD). Heterogeneity of dementia etiology and the overlapping of neuropathological features of AD and AD-CVD make feature identification of the two challenging. Separation of AD from AD-CVD is important as the optimized treatment for each group may differ. Recent studies using vestibular responses recorded from electrovestibulography (EVestG™) have offered promising results for separating these two pathologies. An EVestG measurement records responses to several different physical stimuli (called tilts). In previous researc

Early Outcomes of Simultaneous Translabyrinthine Resection and Cochlear Implantation.

OBJECTIVES/HYPOTHESIS: Hearing rehabilitation after translabyrinthine resection of a vestibular schwannoma (VS) has largely been based on the transfer of acoustic stimulus to the contralateral ear, typically through a contralateral routing of signal hearing aid or bone-anchored hearing aid (BAHA). Cochlear implant, either as a subsequent surgery or simultaneously, has become a more common treatment option; however, there is still relatively limited data available on its success. The purpose of this study is to evaluate the early outcomes of simultaneous cochlear implantation in patients with s
