BACKGROUND: The recent amalgamation of data by users of the Perinatal Problem Identification Programme (PPIP) throughout South Africa has culminated in the publication of the Saving Babies report.
OBJECTIVES: To determine the absolute rate of death from intrapartum-related birth asphyxia, and the contribution of intrapartum-related asphyxia to total perinatal mortality in South African hospitals, and to identify the primary obstetric causes and avoidable factors for these deaths.
METHODS: The amalgamated PPIP data for the year 2000 were obtained from 27 state hospitals (6 metropolitan, 12 to

OBJECTIVES: To examine whether infants who are being breast fed in their mother's bed are at increased risk of accidental suffocation.
MATERIALS AND METHODS: Review of all cases of unexpected infant death occurring in South Australia, Australia during 1996 was undertaken to ascertain whether any cases of sudden infant death had occurred in association with breast feeding in the parental bed. All infants had death scene examinations, history reviews and full autopsies performed.
RESULTS: Three of a total of 28 cases of unexpected infant death were identified where accidental asphyxia associat

To determine whether vascular endothelial damage occurs in neonates with asphyxia, we examined the plasma thrombomodulin level at birth in infants with and without asphyxia. The plasma thrombomodulin concentration in 11 asphyxiated infants was significantly elevated compared with that in 48 infants without asphyxia (38.1 vs 27.0 micrograms/l, p < 0.0001). The plasma thrombomodulin-to-serum creatinine ratio was also significantly elevated (0.62 vs 0.48 microgram/mumol, p = 0.0005). The plasma thrombomodulin concentration and the plasma thrombomodulin-to-serum creatinine ratio in infants without

Visual evoked potentials (VEPs) have proved to be accurate predictors of outcome in term infants with hypoxic-ischemic encephalopathy. Parallels between term asphyxia and hypoxic-ischemic injury in the preterm brain suggested the hypothesis that VEPs may predict the development of periventricular leukomalacia (PVL) and later cerebral palsy. 123 infants less than 32 weeks' gestational age were enrolled in the study. VEPs were done in the first 3 weeks of life (usually first week). VEPs did show a statistically significant association with PVL (p < 0.04) although false-positive recordings were t

The consequences of perinatal asphyxia on the rat brain were studied 80 min to 8 days after birth with hematoxylin-eosin and in situ DNA double-strand-breaks labeling histochemistry. Asphyxia was induced by immersing fetus-containing uterus horns, removed from ready-to-deliver Sprague-Dawley rats, in a water bath at 37 degrees C for various time periods (0-22 min). Spontaneous- and cesarean-delivered pups were used as controls. Perinatal asphyxia led to a decrease in the rate of survival, depending upon the length of the insult. No gross morphological changes could be seen in the brain of eith

OBJECTIVE: To determine risk factors for perinatal asphyxia.
DESIGN: Cohort study.
SETTING: Teaching hospital.
METHODS: All consecutive hospital births were evaluated during the study period. Asphyxia was defined on intrapartum and neonatal resuscitation criteria. Maternal, intrapartum and neonatal variables were recorded in all births. Data was analyzed after stratifying for live and stillbirths by univariate and logistic regression analyses.
RESULTS: Amongst 2371 births (55 fetal deaths and 2316 live births), there were 86 cases of perinatal asphyxia (35 fetal deaths and 51 live births),

Eighty-eight EEGs from 32 premature neonates with autopsy-verified periventricular-intraventricular hemorrhage (PVH-IVH) were compared with associated neuropathological findings. PVH-IVH was rarely an isolated lesion at autopsy. Twenty-seven infants (84%) had additional parenchymal brain lesions, such as periventricular leukomalacia (47%), ischemic neuronal necrosis (22%), pontosubicular necrosis (22%), cerebral infarction (13%), and/or cerebellar hemorrhage (13%). A significant correlation was found between the patient's most abnormal EEG and the severity of morphological changes. Infants wit

OBJECTIVE: To investigate whether or not postasphyctic cerebral hypoperfusion and decreased cerebral metabolism occur in the perinatally asphyxiated neonate, as has been reported in adults and newborn animals.
METHODS: Using near-infrared spectroscopy, we monitored changes in oxyhemoglobin (HbO2), deoxyhemoglobin (HbR), total hemoglobin (HbO2 + HbR, which represents changes in cerebral blood volume [CBV]), and cytochrome oxidase (Cytaa3, which indicates changes in oxidation level of this intracerebral mitochondrial enzyme). Thirty-one neonates (gestational age > 34 weeks), divided into three

Very scanty information is available in East, Central and Southern Africa on the incidence and risk factors associated with asphyxia of the newborn. A multicentre prospective study involving 4267 deliveries in eight countries was undertaken over a three month period, in maternity units of the central hospitals to determine the incidence; maternal, service and logistic risk factors for asphyxia of the newborn as determined by an abnormally low apgar score. 30% of births were by primigravida mothers, of whom 67% were teenagers. A birth by a teenager had a higher risk for low birth weight. Overal

Between 1988 and 1991 thirty-nine pregnant women suffering from HELLP-Syndrome were treated in our hospital. A retrospective analysis of clinical course and of the changes in the relevant laboratory values was performed. The incidence of the HELLP-Syndrome at Freiburg University Hospital was 0.85% for all pregnancies or 17% of the patients with toxaemia. 90% of the women were primipara, there was no prevalence of a certain group. The clinical course was characterized by the perseverance of symptoms for more than two weeks in 18% of the cases and a perinatal mortality of 25%, the latter mainly

There is increasing evidence that cerebral palsy (CP) in developed countries results mainly from antenatal factors, whereas reports from developing countries suggest that perinatal and postnatal factors may be more important because of less than optimal delivery conditions. The authors studied 103 Saudi children with CP and compared their antecedent factors with those of a control group. The major risk factors identified were a history of CP in a sibling and consanguinity of the parents. Low birthweight (less than 2000g), gestational age less than 32 weeks, twin pregnancy and respiratory distr

The CT findings in 120 cerebral palsied children are analysed. The 72.5% positive findings are correlated with the clinical types, as well as the aetiological basis for the cerebral palsy. The spastic type, 83.3% of the total number of children, had the highest positive findings. The yield was increased in children with seizures (91.3%) and those in the postnatal group (90%), as well as those with birth trauma and neonatal asphyxia (94%). The findings were those of atrophy in 30.8%, hydrocephalus, in 10%, infarct in 11.6%, porencephaly in 8.3% and others. The atropic changes and their patterns

Analyses were undertaken to determine the causes of cerebral palsy in a prospective study of 43,437 full-term children. Presumed causes were found for about 71% of the 34 quadriplegic and 40% of the 116 nonquadriplegic patients with cerebral palsy. Risk estimates based on predictive models, adjusted for multiple factors, suggest that 53% of the quadriplegic patients with cerebral palsy could be attributed to congenital disorders, 14% to birth asphyxia, and 8% to other identified disorders. Thirty-five percent of the nonquadriplegic patients with cerebral palsy could be attributed to congenital

Infants of diabetic mothers are thought to be at risk for perinatal asphyxia. We hypothesized that the following are significant risk factors for perinatal asphyxia: poor third-trimester glycemic control, diabetic vascular disease (nephropathy, retinopathy) appearing in pregnancy, pregnancy-associated hypertension, smoking, prematurity, fetal macrosomia, and maternal hyperglycemia and hypoglycemia within 6 hours preceding delivery. We prospectively studied 162 infants born to 149 diabetic mothers (White classes B through R-T). Perinatal asphyxia was defined clinically as fetal distress during

Plasma parathormone (PTH) and calcium concentrations were measured in 309 specimens collected from 190 newborns during the first 7 days of life. The patient material consisted of 51 preterm, 130 term, and 9 postterm infants, including 22 infants of diabetic mothers (IDM), 38 infants with hypocalcemia, and 25 asphyxiated infants. PTH was detectable, although in low concentrations, in cord blood samples despite the presence of elevated calcium concentrations. Postpartum, PTH concentrations in term, appropriate for gestational age (AGA) infants remained low during the first 2 days of life; a sign

Perinatal central-nervous-system lesions may be found in as many as one third of all subjects who died before, during, or after the time of birth. Today, hypoxia is considered to be an essential cause of such lesions; in contrast to views held previously, birth trauma, i.e., physical injury to an infant during its delivery, is of minor pathogenetic importance. Neurohistological studies showed that it is especially prolonged hypoxia - in addition to cerebral hemorrhage, damage to the cerebral parenchyma and medullary substance - which can cause lesion of the brain stem. A relatively frequent oc
