Understanding stroke risk phenotypes in pediatric patients with sickle cell disease and concurrent moyamoya arteriopathy: insights from 61 cases at a single institution.

OBJECTIVE: Children with sickle cell disease (SCD) are at risk of developing cerebral vasculopathies, increasing the risk of stroke. While some vasculopathies are medically managed, surgical revascularization might be needed in patients who also have moyamoya arteriopathy (MMA). The aim of this study was to characterize stroke timing and phenotype between children with homozygous SCD (HbSS) alone and those with HbSS and concurrent MMA. METHODS: A retrospective review of clinical and radiological information for pediatric patients (age < 21 years) with HbSS and cerebrovascular sequelae at a si

Long-Term Clinical and Angiographic Outcomes of Off-Pump Versus On-Pump Coronary Artery Bypass Grafting.

INTRODUCTION: This study aimed to compare long-term outcomes in patients undergoing off-pump coronary artery bypass grafting (OPCAB) versus on-pump coronary artery bypass grafting (ONCAB) in a single-center propensity-matched population. METHODS: Between January 2010 and June 2022, 2964 patients were analyzed and divided into two groups: 1671 (56.3%) patients receiving OPCAB and 1293 (43.7%) patients receiving ONCAB. Propensity score matching was performed resulting in 842 pairs. RESULTS: In the matched cohort, OPCAB was associated with a reduced incidence of early stroke (odds ratio: 0.40;

The Role of Encephaloduroarteriosynangiosis in Moyamoya Disease: A Consecutive Case Series From Pakistan.

Background Moyamoya disease is a rare cerebrovascular disorder characterized by the occlusion of cerebral arteries due to stenosis. Although the disease lacks a specific treatment approach, various surgical options have been discovered to address ischemic issues, including indirect bypass, direct bypass, and their combination, which aim to enhance cerebral blood flow. Indirect revascularization via encephaloduroarteriosynangiosis (EDAS) is deemed effective in preventing additional strokes, although its use for preventing future hemorrhagic stroke in Moyamoya disease remains a subject of contro

Predictors of Futile Recanalization After Endovascular Therapy for Acute Large Vessel Occlusive Intracranial Atherosclerosis: A Secondary Analysis of RESCUE-ICAS.

BACKGROUND AND OBJECTIVES: Futile recanalization in endovascular thrombectomy (EVT) occurs when recanalization is achieved but patients nevertheless experience a poor functional outcome. Variables associated with futile recanalization after endovascular therapy in intracranial atherosclerosis-related large vessel occlusion have not been previously described. METHODS: This was a secondary analysis of Registry of Emergent Large Vessel Occlusion due to Intracranial Stenosis (RESCUE-ICAS), a multicenter prospective observational cohort study conducted at 25 EVT-capable centers in North America, E

Differential outcomes of type A dissection with malperfusion according to affected organ system.

BACKGROUND: The management of malperfusion in patients with acute Stanford type A aortic dissection is controversial. We sought to determine the rate of resolution of malperfusion following primary repair of the dissection and to identify anatomic sites of malperfusion that may require additional management. METHODS: We reviewed the hospital records of patients who presented to our institution with acute type A aortic dissection. Patient demographics, operative details and post-operative course were retrospectively extracted from our institutional electronic database. Depending upon the anato

Omental transplantation for neurodegenerative diseases.

Up to date, almost all researchers consider that there is still no effective therapy for neurodegenerative diseases (NDDs) and therefore, these diseases are incurable. However, since May 1998, we know that a progressive ischemia in the medial temporal lobes and subcommissural regions can cause Alzheimer's disease; because, in contrast to this, its revascularization by means of omental tissue can cure or improve this disease. Likewise we observed that the aging process, Huntington's disease, Parkinson's disease, and Amyotrophic lateral sclerosis; all of them are of ischemic origin caused by cer

Embolic Stroke: A Rare but Probably Real Cause of Aneurysmal-Like Subarachnoid Hemorrhage.

BACKGROUND: Ischemic stroke is being increasingly recognized as a possible cause of spontaneous isolated convexity subarachnoid hemorrhage (SAH). However, it is a much less established cause of cisternal, aneurysmal-like SAH. Only 3 case reports of concomitant cisternal SAH and perforator infarcts exist in the literature, raising the possibility of perforating artery rupture as a potential mechanism. In contrast, embolic stroke is not recognized as a cause of aneurysmal-like SAH. CASE DESCRIPTION: In 2 patients with embolic cerebral infarctions mimicking intracranial aneurysm rupture, diagnos

Local cerebral hemodynamic changes through the angiographic stages of moyamoya disease.

In order to elucidate the cerebral hemodynamic changes that occur in Suzuki's six angiographic stages of moyamoya disease, local cerebral blood flow (LCBF) during the stable state and CO2 responsiveness of LCBF (L-CO2R: delta %LCBF/delta PaCO2) were measured by the Xenon CT-CBF method. Nineteen patients with moyamoya disease (mean age: 36.8 +/- 11.6 years) and 11 age-matched normal volunteers were studied. The LCBF during the steady state at all stages was not significantly different from that in normal volunteers. At stage 6, however, the LCBF was slightly decreased in the anterior part of th

[Importance of carotid artery studies prior to coronary surgery].

The incidence of cerebral complications following extracorporal cardiac operations is reported to be approximately 2%. One of the possible reasons behind these complications is the presence of significant carotid stenosis as coexistent disease to the cardiac illness requiring surgery. Because of the common etiology carotid stenosis coexists mainly with a coronary artery disease. The authors make known their own screening methods based on the correct exploration of medical history and on the proper physical examinations. Coexistent significant carotid stenosis was revealed in 3.6% of 1056 patie

Noninvasive tomographic study of cerebral blood flow and oxygen metabolism in vivo. Potentials, limitations, and clinical applications in cerebral ischemic disorders.

The non-invasive continuous inhalation technique of C15O2 and 15O2 coupled with positron emission tomography (PET) provides brain images that are thought to represent local cerebral blood flow (CBF) and oxygen extraction fraction (OEF). Experimental studies in baboons have confirmed that C15O2 inhalation allows tomographic measurement of CBF. The numerous difficulties involved in PET absolute quantitation are stressed, as well as some limitations inherent to the 15O inhalation model. However, the values for local CBF, OEF and CMRO2 obtained in normal young subjects are satisfactory in view of

Randomized Controlled Trial of Conventional Versus MicroNet-Covered Stent in Carotid Artery Revascularization.

OBJECTIVES: The aim of this study was to compare procedure-related ipsilateral cerebral embolism with a conventional (Acculink, Abbott Vascular) versus a MicroNet-covered (CGuard, InspireMD) stent in carotid artery stenting (CAS). BACKGROUND: The MicroNet-covered stent may reduce periprocedural cerebral embolism in CAS, but level 1 evidence is lacking. METHODS: A total of 100 consecutive patients were randomized 1:1 to filter-protected CAS using the Acculink or the CGuard device. The study was powered for its primary endpoint of at least 50% reduction in ipsilateral diffusion-weighted magnet

[Post-traumatic occlusion of the anterior cerebral artery].

A rare case with cerebral infarction in the region of the anterior cerebral artery after minor head injury is reported. A 44-year-old male sustained a blow in the occipital area and became unconscious for a moment when he was driving a car and was involved in a rear-end collision. There being no significant signs, he drove home. About 30 minutes after the traffic accident, he felt weakness in the right limbs and speech difficulty and immediately visited our clinic. The patient was alert, but right hemiparesis and mild motor aphasia were present. There was no apparent evidence of head injury. A

Challenges and Strategies in Reoperative Microsurgical Management of Cerebral Aneurysms after Clipping: Illustrative Cases and Review of the Literature.

BACKGROUND: Cerebral aneurysm clipping generally provides durable occlusion, but residual aneurysm regrowth or recurrence after apparently complete treatment may rarely require reoperation. Reoperative microsurgery is challenging because previous clips, dense scarring, altered vascular anatomy, and complex aneurysm-wall pathology may restrict exposure and vascular control. We reviewed technical challenges and operative strategies in microsurgical retreatment after clipping, supplemented by two illustrative cases. METHODS: We present two illustrative cases comprising three previously clipped a

Infraslow scalp electroencephalogram is closely linked to transient neurological events in Moyamoya disease.

BACKGROUND: Transient neurological events (TNEs) commonly occur following direct revascularization for moyamoya disease (MMD). Although changes in the cerebral blood flow (CBF) may be linked to TNEs, the underlying mechanism remains unclear. Herein, we investigated the relationship between TNEs and EEG to elucidate the underlying mechanisms and pursue the clinical utility of EEG as a TNE biomarker. METHODS: Twenty-six hemispheres in 22 patients with MMD aged ≥15 years who underwent revascularization were analyzed. Scalp electroencephalography (EEG) and single-photon emission computed tomograp

The effect of white matter lesions on cognition after carotid revascularization.

BACKGROUND: Cerebral white matter lesions (WML) are associated with cognitive impairment, and carotid revascularization with cognitive worsening or improvement. We assessed the relation between WML severity and changes in cognition after carotid endarterectomy or stenting. METHODS: Patients with symptomatic carotid artery stenosis, enrolled in the International Carotid Stenting Study (ISRCTN25337470), underwent detailed neuropsychological examinations (NPEs) before and after 6 months. Cognitive results were standardized into z-scores, from which a sum score was calculated. The primary outcome

Scintigraphic evaluation of myocardial and cerebral blood flow in patients with a history of coronary artery bypass grafting.

This paper deals with a comparative scintigraphic evaluation of the changes that occur in coronary and cerebral circulation in patients with coronary artery disease (CAD) after coronary artery bypass grafting (CABG) under conditions of cardiopulmonary bypass (CPB) or beating heart. Twenty-nine CAD patients who underwent CPB were examined. Of these, 14 patients were operated on using CPB (the first group) and 15 patients on the beating heart using the myocardial "stabilizer" Octopus (the 2nd group). The patient groups matched in terms of the age, sex, the clinical and angiographic factors. Perf
