To what extent have relaxed eligibility requirements and increased generosity of disability benefits acted as disincentives for employment? A systematic review of evidence from countries with well-developed welfare systems.

BACKGROUND: Reductions in the eligibility requirements and generosity of disability benefits have been introduced in several Organisation for Economic Cooperation and Development (OECD) countries in recent years, on the assumption that this will increase work incentives for people with chronic illness and disabilities. This paper systematically reviews the evidence for this assumption in the context of well-developed welfare systems. METHOD: Systematic review of all empirical studies from five OECD countries from 1970 to December 2009 investigating the effect of changes in eligibility require

Acceptance of virtual patients as a continuous professional development approach among practicing nurses in primary health care settings in a low-income country: a quasi-experimental posttest setup design.

BACKGROUND: Virtual patients are an educational technological approach used in healthcare education. Its distinctive features have rendered virtual patient technology appealing for the training of medical and healthcare students, particularly in the enhancement of clinical reasoning. Virtual patients are less often applied for continuous professional development for practicing healthcare providers, and there is a scarcity of studies exploring this possibility. This study aimed to assess the acceptability of nurses for using virtual patients as a continuous professional development approach. M

Long-term effects of severe acute malnutrition during childhood on adult cognitive, academic and behavioural development in African fragile countries: The Lwiro cohort study in Democratic Republic of the Congo.

INTRODUCTION: Little is known about the outcomes of subjects with a history of severe acute malnutrition (SAM). We therefore sought to explore the long-term effects of SAM during childhood on human capital in adulthood in terms of education, cognition, self-esteem and health-related disabilities in daily living. METHODOLOGY: We traced 524 adults (median age of 22) in the eastern Democratic Republic of the Congo, who were treated for SAM during childhood at Lwiro hospital between 1988 and 2007 (median age 41 months). We compared them with 407 community controls of comparable age and sex. Our o

Multicentre cross-sectional clinical evaluation study about quality of life in adults with disorders/differences of sex development (DSD) compared to country specific reference populations (dsd-LIFE).

BACKGROUND: Previous studies in quality of life (QOL) in individuals with disorders/differences of sex development (DSD) have been restricted to subpopulations of the condition. We describe QOL in adult persons with DSD compared to country specific references and assess the impact of diagnosis. METHODS: The multicentre cross-sectional clinical evaluation (dsd-LIFE) took place in 14 specialized clinics in six European countries. Adolescents (≥16 years) and adults having a DSD condition were included from 02/2014 to 09/2015. The main outcome QOL was measured by the WHOQOL-BREF (domains of physi

Current State of LGBTQIA+ Health Education in Low- and Middle-Income Countries: A Narrative Review of Educational Gaps in Transgender, Intersex/Differences of Sex Development (DSD), and Sexual and Gender Minority Healthcare.

Sexual and gender minorities, LGBTQIA+ individuals, transgender individuals, and intersex individuals represent a small proportion of the population and encompass diverse sexual preferences, gender identities, sexual characteristics, and gender expressions. These individuals often present with unique health needs due to biological differences in anatomy, physiology, or genetic sexual characteristics. Despite these differences, education regarding their healthcare is insufficient in medical curricula worldwide, specifically in low- and middle-income countries (LMICs). Medical students are often

Evaluating the Efficacy of Modified BeEAM(Bendamustine, Etoposide, Cytarabine, Melphalan) Regimen as Conditioning for Autologous Stem Cell Transplantation in Relapsed or Refractory Lymphoma: An Experience from Two Centers of a Developing Country.

Background: High-dose chemotherapy followed by Autologous SCT (stem cell transplantation) is a treatment of choice for relapsed and refractory lymphoma. Due to cost, toxicity, and shortage of Carmustine, we decided to conduct a phase 2 clinical trial to evaluate the safety and efficacy of Bendamustine instead of Carmustine in a previously used BEAM-like protocol. Materials and Methods:102 patients (median age,37) with Hodgkin(n=54) and non-Hodgkin lymphoma(n=48) were recruited and transplanted in two centers. After stem cell harvesting, a modified BeEAM regimen was administered to all the pati

Measuring progress from 1990 to 2017 and projecting attainment to 2030 of the health-related Sustainable Development Goals for 195 countries and territories: a systematic analysis for the Global Burden of Disease Study 2017.

BACKGROUND: Efforts to establish the 2015 baseline and monitor early implementation of the UN Sustainable Development Goals (SDGs) highlight both great potential for and threats to improving health by 2030. To fully deliver on the SDG aim of "leaving no one behind", it is increasingly important to examine the health-related SDGs beyond national-level estimates. As part of the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 (GBD 2017), we measured progress on 41 of 52 health-related SDG indicators and estimated the health-related SDG index for 195 countries and territories for

Predicting Risk of Tuberculosis (TB) Disease in People Who Migrate to a Low-TB Incidence Country: Development and Validation of a Multivariable, Dynamic Risk-Prediction Model Using Health Administrative Data.

BACKGROUND: Tuberculosis (TB) incidence remains disproportionately high in people who migrate to Canada and other countries with low TB incidence, but systematic TB screening and prevention in migrants are often cost-prohibitive for TB programs. We aimed to develop and validate a TB risk-prediction model to inform TB screening decisions in foreign-born permanent residents of Canada. METHODS: We developed and validated a proportional baselines landmark supermodel for TB risk prediction using health administrative data from British Columbia and Ontario, 2 distinct provincial healthcare systems

A randomized controlled trial comparing incidences of postoperative nausea and vomiting after laparoscopic cholecystectomy for preoperative intravenous fluid loading, ondansetron, and control groups in a regional hospital setting in a developing country.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a common complication in inpatient and outpatient settings. Multimodal approaches have been pursued to minimize this undesirable outcome. Despite consensus guidelines for the management of PONV have been updated and published for many years, data from our pilot study showed that patients with high-risk surgeries for PONV, laparoscopic cholecystectomy (LC), still hardly received perioperative PONV prophylaxis. This study aimed to compare the incidences of PONV in adult patients undergoing elective LC who were administered preoperative intr

Do phenotypic characteristics, parental psychopathology, family functioning, and environmental stressors have a role in the response to methylphenidate in children with attention-deficit/hyperactivity disorder? A naturalistic study from a developing country.

Little is known about the effect of clinical characteristics, parental psychopathology, family functioning, and environmental stressors in the response to methylphenidate in children with attention-deficit/hyperactivity disorder (ADHD) followed up in a naturalistic setting. Data from cultures outside the United States are extremely scarce. This is a longitudinal study using a nonrandom assignment, quasi-experimental design. One hundred twenty-five children with ADHD were treated with methylphenidate according to standard clinical procedures, and followed up for 6 months. The severity of ADHD s

Global prevalence of dyspnoea: a systematic review and meta-analysis.

BACKGROUND: Dyspnoea is a common subjective experience associated with decreased exercise tolerance and mortality. Although several studies have explored its prevalence in the general population, there has been limited synthesis over broad income settings, particularly low-income countries. This review and meta-analysis aims to address these gaps by reviewing the global prevalence of dyspnoea across low, middle, and high-income countries and associated factors. METHODS: A systematic review and meta-analysis were completed evaluating the prevalence of dyspnoea in the general population and ass

Factors associated with post spinal cord injury employment: A systematic review and meta-analysis.

PURPOSE: To identify factors associated with post spinal cord injury (SCI) employment, exploring differences between high-income (HIC) and low/middle-income (LMIC) countries. METHODS: This systematic review and meta-analysis included, Embase, CINAHL, PsycINFO, and Scopus. Articles discussing post-SCI employment were included and excluded if data were not disaggregated by "employed/unemployed" or did not include SCI-only sub-groups. Factors reported in three articles were considered for meta-analyses, using a random effects model to estimate odds ratios (OR) and mean differences (MD). RESULTS

Brain drain or brain gain? Effects of high-skilled international emigration on origin countries.

How does emigration of highly educated citizens of low-income countries to high-income countries affect the economies of the origin countries? The direct effect is "brain drain"-a decrease in the country's human capital stock. However, there may also be indirect "brain gain" effects. This review summarizes evidence that uses causal inference methods to reveal mechanisms that may lead to brain drain, gain, or circulation. Collectively, the weight of the evidence suggests that migration opportunities often increase human capital stock in origin countries and produce downstream beneficial effects

Rehabilitation Services for Young-Onset Dementia: Examples from High- and Low-Middle-Income Countries.

The WHO Dementia Global Action Plan states that rehabilitation services for dementia are required to promote health, reduce disability, and maintain quality of life for those living with dementia. Current services, however, are scarce, particularly for people with young-onset dementia (YOD). This article, written by an international group of multidisciplinary dementia specialists, offers a three-part overview to promote the development of rehabilitation services for YOD. Firstly, we provide a synthesis of knowledge on current evidence-based rehabilitative therapies for early-onset Alzheimer's
