We evaluated 16 children with congenital pseudoarthrosis of the tibia treated with contralateral fibular graft, with the aim to report the difficulties and clinical results in the affected limb after consolidation. Sixty-three percent of the children had characteristics of neurofibromatosis. Consolidation was achieved after the main surgery in 37%of patients, and the remainder, after multiple procedures. Bone healing was investigated histologically in a rat fibular osteotomy model subjected to microgravity (shuttle flight STS-29) and the tail suspension microgravity simulation model. A biomechanical study was designed to test the hypothesis that isolated plantar flexion of the ankle can produce avulsion fractures of the medial and lateral malleoli without fracture of the posterior articular surface of the tibia. The elastic fixation of the distal tibiofibulary syndesmosis by means of Engelbrecht's hook allows weight bearing and free dorsal flexion. The example of an 40-year-old long distance runner (and surgeon) shows that this system can endure extreme stress: during the 9 months until the metal was removed, he took part in 14 contests with excellent results. He ran at least 100 km per week, together more than 3700 and classical marathons, 100 km runs and mountain marathons. When the metal was removed there was no sign of metallotic changes of the tissue at the interface between hook and plate.

BACKGROUND: Although a variety of surgical techniques are available for the treatment of ankle degenerative disease, arthrodesis is a common treatment especially for unilateral posttraumatic arthritis in young patients. A clear trend toward internal fixation for ankle arthrodesis is evident, but fusion site compression is known to play an integral role in primary bone healing.

Follow-up checks were performed on 106 patients, between four and 16 years after osteosynthesis on the upper ankle joint. Good to very good results by Weber's criteria were recorded from 57.5 per cent. That percentage had been much higher in other comparable studies. Twenty-five consecutive in-patients who had suffered from rheumatoid arthritis for at least five years were investigated radiologically, histologically and biochemically for evidence of metabolic bone disease. Dietary intake of vitamin D was universally well below recommended levels.
