OBJECTIVE: Pheochromocytoma and paraganglioma (extra-adrenal pheochromocytoma) are rare neuroendocrine tumors that arise from the neuroendocrine cells. Chronic hypoxia is known as a possible cause, and a strong link between cyanotic congenital heart disease and these tumors has been reported. However, reports of phechromocytoma/paraganglioma in Fontan patients were scarce. We herein report seven cases of phechromocytoma/paraganglioma after Fontan operation at a single tertiary center.

BACKGROUND: This study analyzed the change in liver fibrosis markers after the Fontan operation and investigated their clinical usefulness as an index of congestive liver fibrosis.
METHODS: The study enrolled 24 patients who underwent the Fontan operation between January 1994 and December 2008. An enlarged thymus adversely affected pulmonary artery development and perfusion in a 4-month-old boy with asplenia syndrome and a single ventricular circulation with aorto-pulmonary shunt. Steroid administration for 4 weeks successfully reduced thymus volume, concomitant with improvement of pulmonary vasculature and perfusion. Given the importance of pulmonary physiology in the Fontan circulation, the effects of thymic hyperplasia on pulmonary circulation should be taken into consideration in the overall management of candidates for Fontan surgery.

Surgical specimens of the atrial wall were microscopically investigated 7 years after total cavopulmonary connection. The intima of the morphologically right atrium was thicker in the part used for the high-pressured venous channel than in the other portion placed for the low-pressured atrial cavity supporting the systemic circulation. The number of myocardial cells was smaller, and the area of interstitial fibrosis was greater, in both circumstances, when compared with the normal right atrium.
