[Massive hepatobiliary fascioliasis].

UNLABELLED: Massive hepatobiliary fascioliasis. Report of two cases. INTRODUCTION: Liver fascioliasis is the most frequent parasitic disease of the biliary tract in Mexico. Massive infestation is common in sheep and bovines and rarely occurs in humans. Only two previous cases of massive fascioliasis has been reported and we present here in two additional cases. Case reports. Both cases presented in alcohol-related liver disease patients who had eaten watercress Nasturium officinalis. Upper abdominal pain, obstructive jaundice, and choledochal dilatation were the salient manifestations. Diagno

[Hydatid cyst in the hepatic hilum causing a cavernous transformation in the portal vein].

Portal cavernomatosis consists in the substitution of the portal vein by many fine, twisting venules leading to the liver. This phenomenon is produced as a consequence of anterior thrombosis of the portal vein and is associated with chronic pancreatitis, cancer of the pancreas, intraabdominal sepsis and cholelithiasis. The symptomatology may be nul or present as obstructive jaundice or portal hypertension. Diagnosis is made by Doppler echography. The treatment is portal shunt when symptomatology is produced. In patients with cholelithiasis requiring surgery, the shunt is advised prior to bilia

Squamous cell carcinoma of the ampulla of Vater: a case report.

A wide variety of neoplastic lesions may involve the ampulla of Vater, but squamous cell carcinoma has not been reported. Here we report a case of squamous cell carcinoma of papilla Vater. A 72-year-old Chinese female was manifested by obstructive jaundice and biliary tract infection. Duodenoscopic study revealed a polypoid mass with deep and broad ulcer on the ampulla of Vater. Histological examination revealed a metastatic squamous cell carcinoma. The gynecologist had examined the patient carefully and revealed a negative tumor survey. Other sources were also intensively studied but which re

Endoscopic diagnosis and management of hepatobiliary tuberculosis.

Primary hepatobiliary tuberculosis is difficult to diagnose and treat. We describe a woman with obstructive jaundice, a common hepatic duct stricture, and granulomatous hepatitis diagnosed with biliary tract tuberculosis when acid-fast bacilli were found in bile aspirated during endoscopic retrograde cholangiopancreatography (ERCP). The hilar stricture resulted from either primary biliary tuberculosis or secondary inflammation related to tuberculous periportal adenitis and persisted despite adequate medical treatment for tuberculosis, apparently the result of postinflammatory stenosis. After 2

Therapeutic pancreaticobiliary endoscopy.

The remarkable advances achieved in the performance of therapeutic endoscopic retrograde cholangiopancreatography have placed endoscopy in the forefront in the schema for treating pancreaticobiliary disorders. Since the introduction of sphincterotomy and its impact on the management of stone disease, we have seen rapid development of alternative methods for treating obstructive jaundice, neoplasia of the biliary tree, and benign disorders. In addition, endoscopists are treating strictures and fistulas of the bile duct and pancreas, gallbladder disease, and more spectacularly, gallstone pancrea

[Disputable problems of surgical tactics in acute cholecystitis].

On the grounds of literature data and their own experience, the authors came to the conclusion that clear-cut identification of the character of the pathological process is of principal importance in deciding on the time of operative intervention for acute cholecystitis. Patients with a pronounced clinical picture of acute cholecystitis and those with signs of peritonitis must be operated on as emergencies, "postponed" operations produce poor results. Indications for an emergency operation in patients with acute cholecystitis complicated by obstructive jaundice and other conditions must be con

[Intensive lymphoid hyperplasia of the peripancreatic ganglia and extrahepatic cholestasis in Sjögren's syndrome. Report of an unusual case].

The diagnostic triad of Sjögren's syndrome is keratoconjunctivitis sicca, xerostomia with o without salivary gland enlargement and the presence of a connective tissue disease usually rheumatoid arthritis). Is postulated an altered immune response of lymphocytes to ductal antigens glands and the result is an chronic inflammatory process with eventual fibrosis and hyposecretion of the affected glands. Lymphoproliferation is characteristic of the Sjögren's syndrome, the lungs, kidneys and salivary glands are the most affected: in this paper is presented the disease complex and not described of Sj

Neonatal jaundice: the surgical viewpoint.

There is good evidence that neonatal hepatitis, biliary hypoplasia, biliary atresia and choledochal cyst are different stages of one disease process for which the term infantile obstructive cholangiopathy has been suggested. Thanks to the work of Kasai and the operation of hepatic portoenterostomy the surgical outlook has greatly improved, although in North America it still leaves much to be desired. One cannot procrastinate too long in the hope that the patient's condition will improve spontaneously, because the surgical results are much better when the operation is performed before the patie

[Hepatitis-B-surface antigen and panarteritis (author's transl)].

A case of an acutely beginning histologically proved panarteritis is described which was initiated by hepatitis B caused by blood transfusions. After one year of steroid therapy the arteritis was no longer seen histologically, Australia-antigen became negative. Terminally the patient developed an apoplexy, renewed gastric bleeding, septicemia with obstructive jaundice, nose bleeding, increasing renal insufficiency, and cardiac failure. The Australia-antigen reappeared in the serum. It could be assumed that the panarteritis had progressed. Immune complexes of Australia-antigen and corresponding

Haemagglutination in acute hepatitis and other diseases.

A haemagglutination test using day-old chick erythrocytes was applied to the sera of 210 patients with hepato-biliary disease and to those of 31 with other diseases.Thirty-six per cent of 58 patients with infective hepatitis gave positive results and this increased to 60% when sera taken during the first 14 days of symptoms were examined. Three of eight patients with infective mononucleosis gave positive titres. Results were consistently negative in 12 patients with acute obstructive jaundice and a raised titre excludes this condition. Positive results were seen in four of 14 patients with cir

[Ticlopidine-induced cholestasis].

UNLABELLED: We report a case of ticlopidine-induced cholestatic hepatitis. CASE SUMMARY: An 82-year-old man suffered a myocardial infarction in February 1998. Because of persistent angina pectoris a coronary stent was implanted in May 1998. At this time medication with 1 x 250 mg ticlopidine was started in addition to the preexistent medication of aspirin 1 x 100 per day, metoprolol fumarate 1 x 95 mg per day and isosorbide dinitrate 1 x 100 mg per day. Three weeks after starting ticlopidine the patient complained of itching, and on day 28 painless jaundice developed. At this time, the serum

[Perioperative complications and recent results of Frey procedure in the treatment of chronic pancreatitis].

OBJECTIVE: To analyze the perioperative complications and recent results of the Frey procedure in the treatment of chronic pancreatitis. METHODS: Between February 2009 and September 2012, 104 patients with chronic pancreatitis underwent the Frey procedures. This study included 91 male and 13 female patients, with a mean age of (49 ± 11) years (range, 16 to 75 years). The most common symptoms were abdominal pain in 97 patients, diarrhea in 10 patients, obstructive jaundice in 5 patients, and 5 patients had no symptoms. Nine patients had history of pancreatic surgery. RESULTS: There was no mor

Management of gallstone ileus.

BACKGROUND/PURPOSE: Gallstone ileus is an uncommon complication of cholelithiasis in the elderly with a high morbidity and mortality rate. This study aims to clarify the current surgical management. METHODS: In a retrospective survey over the past 11 years there were 9 patients with gallstone ileus, all elderly (mean age, 77 years), among 2242 cholecystectomies (0.4%) and 243 operated small intestinal obstructions (3.7%). Urgent laparotomy confirmed gallstone obstruction and a cholecystoduodenal (89%) or cholecystocolonic (11%) fistula. The operation included enterolithotomy alone (3 high-ris

Hepatocellular carcinoma embolus to the common hepatic duct with no detectable primary hepatic tumor.

Obstruction of the common bile duct (CBD) by direct extension of tumor is occasionally found in patients with hepatic neoplasms. Tumor embolus to the CBD is very rare, however, when no primary hepatic tumor is found. The patient described herein was a 74-year-old man who presented with a new onset of jaundice, nausea, anorexia, and epigastric pain. There was a history of dark urine and clay-colored stools, but no fever. Endoscopic retrograde cholangiopancreatography (ERCP) showed partial obstruction of the common hepatic duct and dilated intrahepatic bile ducts. A computed tomography (CT) scan

Cutaneous presentation of metastatic pancreatic cancer.

Pancreatic cancer typically presents with a cholestatic picture of jaundice, bloating and bowel changes, secondary to the obstruction of the biliary system. In tail of pancreas metastases, this clinical presentation may be absent, meaning that the disease progression can be more insidious. Cutaneous spread in pancreatic malignancies is often iatrogenic due to direct spread from the tumour site during diagnostic sampling or surgery. Rarely, cutaneous spread can be noted at distant sites, which is associated with widespread disease dissemination and poor prognostic outcomes. Our case focusses on

Giant Cell Hepatitis in Copper Toxicosis.

Cholestatic jaundice in new born and infants results from biliary obstruction or hepatocellular dysfunction. Biliary atresia (BA) and Idiopathic neonatal hepatitis comprises the major aetiology. Cholestasis due to toxins is rare in infants. Indian childhood cirrhosis (ICC) and ICC like diseases have been described in infants. Herein, authors are describing a case of infantile cholestasis presenting at 4 months of age who was diagnosed to have copper related hepatotoxicosis on liver biopsy. Copper tumblers were used for preparation of formula milk that likely was the source of exogenous copper

[Hepato-bilio scintigraphy in the examination of patients with acute cholecystitis before laparoscopic cholecystectomy].

The role of hepato-bilio scintigraphy in examination of 47 patients with acute cholecystitis was evaluated. Laparoscopic cholecystectomy was performed in all the patients. The existence of a functionally "dead" gallbladder in most cases presupposes the existence of marked inflammatory and infiltrative alterations of the neck of the gallbladder, a chronic decline of liver function in patients with calculous cholecystitis complicated with mechanical jaundice and pancreatitis. The effectiveness of hepato-bilio scintigraphy in detection of extra liver biliary tracts obstruction is proved. Hepato-b

Arthrogryposis, renal dysfunction, cholestasis syndrome in a neonate: an uncommon association of common problems.

A male infant born out of non-consanguineous marriage to a primigravida presented to us as his third hospitalisation with ichthyotic lesions all over the body, cholestatic jaundice, multiple joint contractures and a history of recurrent sepsis. Blood and urine investigations revealed Fanconi syndrome, hypothyroidism and direct hyperbilirubinaemia with elevated liver enzymes and normal gamma glutamyl transpeptidase levels. The combination of arthrogryposis, renal dysfunction and cholestasis led to the suspicion of arthrogryposis, renal tubular dysfunction, cholestasis (ARC) syndrome, which was
