Stent-supported reconstruction of the aortoiliac bifurcation with the kissing balloon technique.

BACKGROUND: Bilateral iliac artery obstructions involving the aortic bifurcation or high-grade stenosis of the abdominal aorta are not usually considered an indication for percutaneous treatment. The purpose of the present study was to evaluate the initial and long-term results of primary stent implantation for reconstruction of the aortic bifurcation. METHODS AND RESULTS: In 48 patients with obstructions of the aortoiliac segment, primary stent implantation was performed with the "kissing balloon technique." Preinterventional angiography showed bilateral stenoses (>85%) of the proximal commo

Correlation of subepithelial haze and refractive regression 1 month after photorefractive keratectomy for myopia.

PURPOSE: To relate myopic regression after photorefractive keratectomy (PRK) to subepithelial haze at the first postoperative month. METHODS: One hundred nineteen eyes of 119 patients underwent excimer laser PRK for treatment of myopia up to -8.00 D. Eyes were examined at 1, 3, 6, 9, and 12 months after surgery. All eyes received fluorometholone 0.1% for the first 5 postoperative months in a tapered dose. Dexamethasone 0.1% qid for 1 month was prescribed to all eyes with a spherical equivalent refraction less than plano, followed by an augmented dose of fluorometholone 0.1%. Eyes with myopia

[Perforation and rupture of coronary arteries].

Spontaneous rupture of coronary arteries as well as coronary perforation during percutaneous interventions are rare but potentially life-threatening incidents often resulting in emergency surgery. Frequency of acute perforation due to therapeutic catheterization varies according to the devices employed. With conventional balloon angioplasty it is estimated to be 0.1 to 0.2% whereas substantially higher rates of up to 3% have been reported with the use of so-called "new devices" (i.e. directional atherectomy, rotablation, excimer laser angioplasty or extractional atherectomy). [Correlation between semi-quantitative classification of corneal topography and the coefficient of Fourier transformation after non-mechanical perforating keratoplasty].

BACKGROUND: Semiquantitative classification of corneal topography after penetrating keratoplasty has the potential for focusing information about the areal dioptric power of the cornea. The purpose of this study was to objectify the procedure of manual semiquantitative classification using a Fourier transform of corneal topography power data and to correlate both methods. PATIENTS AND METHODS: Fifty patients each (30 keratoconus, 20 Fuchs dystrophy) underwent nonmechanical trephination (excimer laser MEL60, Aesculap-Meditec, Jena) in penetrating keratoplasty. All procedures (7.5-mm trephinati

Subjective and Objective Visual Quality Outcomes After KLEx vs KLEx Xtra: A 2-Year Prospective Randomized Trial.

PURPOSE: To compare the 2-year refractive correction outcomes and subjective and objective visual quality between keratorefractive lenticule extraction (KLEx) and KLEx combined with corneal cross-linking (KLEx Xtra). METHODS: Patients with myopia were recruited for KLEx or KLEx Xtra surgeries. Only the right eyes were selected. The cross-linking energy was 2.4 J/cm2. Visual acuity and optical quality (objective and subjective) were assessed preoperatively and at 1, 6, 12, and 24 months postoperatively. RESULTS: A total of 150 participants were enrolled, divided into two groups: the KLEx grou

PURPOSE: The purpose of this study was to identify preoperative and demographic risk factors of myopic regression 1 year after corneal refractive surgery. METHODS: A retrospective study of 2093 patients (2781 eyes) who underwent laser-assisted in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), or small incision lenticule extraction (SMILE) and experienced myopic regression of ≥0.5 D 1 year after surgery was conducted. RESULTS: The incidence of regression at 1 year was higher in the overall SMILE group (10.1%) and steep keratometry group (9.6%) ( P < 0.05). SMILE eyes with nor

PURPOSE: To analyze the outcomes of a new technique (corneal remodeling) to treat corneal ectasia. METHODS: Sixty-nine cases that underwent corneal remodeling were analyzed. Anesthetic drops were instilled and a femtosecond laser platform was used to perform an 8-mm diameter keratectomy (180°, 270°, or 360°). Once ablation was completed, the edges of the resection were sutured with 8 to 12 interrupted stitches. RESULTS: Three-year follow-up data are presented. The age of the population was 30.83 ± 12.65 years (range: 16 to 48 years). At 36 months postoperatively, 57.2% presented with uncorre
