Biomechanical behavior of various post-and-core systems in severely compromised mandibular molars: A 3D finite element analysis.

STATEMENT OF PROBLEM: Selecting a post-and-core system for endodontically treated posterior teeth remains challenging. Cast post-and-core (CPC) and fiber post with composite resin core (FPCC) systems are used to restore structurally compromised teeth. Their biomechanical interactions with the remaining tooth structure under functional loading remain poorly understood. PURPOSE: The purpose of this finite element analysis study was to assess how CPC and FPCC restorations influence internal stresses and interfacial stress distributions in a restored mandibular first molar under functional loadin

A range of condylar hypoplasia exists in Treacher Collins syndrome.

PURPOSE: Temporomandibular joint malformation is a complex deformity in Treacher-Collins syndrome (TCS); however, it is not well characterized. This study aimed to better clarify this pathology by volumetrically assessing the mandibular condyle in patients with TCS compared with normal controls and the relative contribution of the condyle to hemimandibular volume. MATERIALS AND METHODS: A retrospective, cross-sectional analysis of pediatric patients with TCS and unaffected controls was performed. The study sample was comprised of Treacher Collins patients. The predictor variable in this study

[The pedicled submental flap].

INTRODUCTION: Facial reconstruction technique can be simple or complicated. The choice of performing a flap is made according to several criteria. Since its first description the submental flap has been used in several indications. The aim of this article was to document maxillofacial surgeons on this flap technique. ANATOMY: The submental artery is a major branch of the facial artery. This artery runs medial to the mandible and ends next to the mandibular symphysis. The submental artery supplies the submandibular lymph nodes, submandibular salivary gland, mylohyoid and digastric muscles, and

Repair of periodontal furcation defects in beagle dogs following reconstructive surgery including root surface demineralization with tetracycline hydrochloride and topical fibronectin application.

This study examines the effects of root surface demineralization and topical fibronectin as adjuncts to reconstructive periodontal surgery. In 14 beagle dogs, horizontal periodontal defects were surgically induced around the mandibular premolars followed by a 6-week period without plaque control. Reconstructive surgery of the defects was subsequently carried out. The root surfaces were debrided and superficially demineralized with citric acid or tetracycline hydrochloride, with or without subsequent application of fibronectin. Mucoperiosteal flaps were raised to cover most of the crowns and su

Nicotine abuse and 30-day perioperative outcomes after fibula free flap reconstruction: An ACS-NSQIP study.

BACKGROUND: Fibula free flap (FFF) reconstruction is a cornerstone of maxillofacial and mandibular reconstruction, particularly for oncologic and traumatic defects. Although smoking is a well-established risk factor for impaired wound healing and microvascular complications, its specific impact on FFF outcomes remains debated. In this multicenter retrospective study, we evaluate the association between smoking and 30-day perioperative outcomes in FFF reconstruction. METHODS: A retrospective cohort study was conducted using data from the American College of Surgeons National Surgical Quality I…

Eagle's syndrome with stylohyoid chain pseudoarthrosis and thyrohyoid ligament ossification: A case report and literature review.

RATIONALE: Eagle's syndrome is caused by anatomical variations or ossification of the stylohyoid chain (SHC) and presents with complex, nonspecific symptoms, often leading to delayed diagnosis. Complete ossification of the SHC is exceptionally rare, and reporting such cases may enhance clinical awareness and improve diagnostic accuracy. PATIENT CONCERNS: A 57-year-old man presented with persistent right mandibular pain lasting over 4 months. The pain intensified with mouth opening, swallowing, or head tilting, and radiated beneath the right earlobe. Conservative treatments including anti-infl

Iatrogenic Atrophic Mandible Fracture: Management Considerations and Digital Workflow.

Atrophic jaws are frequently seen in edentulous patients due to early tooth loss, leading to progressive alveolar bone resorption, bone fragility, and increased susceptibility to fractures, even with low-energy trauma. Surgical management of these fractures presents additional challenges, such as reduced bone density, fragment instability, reduced blood flow, and lack of natural occlusal support. With advances in implant-supported rehabilitation, new perspectives have emerged, but risks remain, especially in areas with severe resorption. Iatrogenic fractures can occur during implant placement

Long term complications of costochondral graft reconstruction in temporomandibular joint ankylosis of the young- a systematic review.

Costochondral graft has been a popular reconstruction choice in the past for temporomandibular joint ankylosis in young individuals. However, accounts of growth hampering complications have also been observed. Our systematic review aims to compile all existing evidence to determine the occurrence of these unfavourable clinical outcomes as well as factors affecting them to provide a better judgement on further use of these grafts. A systematic review was conducted following PRISMA guidelines where databases like PubMed, Web of science and Google Scholar were searched for the purpose of data ext

Effect of surgically guided axonal regrowth into a 3-way-conduit (isogeneic trifurcated aorta) on functional recovery after facial-nerve reconstruction: Experimental study in rats.

BACKGROUND: The "post-paralytic syndrome" after facial nerve reconstruction has been attributed to (i) malfunctioning axonal guidance at the fascicular (branches) level, (ii) collateral branching of the transected axons at the lesion site, and (iii) intensive intramuscular terminal sprouting of regenerating axons which causes poly-innervation of the neuromuscular junctions (NMJ). OBJECTIVE: The first two reasons were approached by an innovative technique which should provide the re-growing axons optimal conditions to elongate and selectively re-innervate their original muscle groups. METHODS
