RATIONALE: Harlequin syndrome is an extremely rare benign condition characterized by unilateral facial flushing and sweating.
PATIENT CONCERN: An 11-year-old boy presented with complaint of a right neck mass of 1-month duration.

Communications between the transverse cervical nerve (C2 and 3) and the cervical branch of the facial nerve have been reported. We describe a case in which the transverse cervical nerve joined the marginal mandibular branch of the facial nerve. It was found during a selective neck dissection and to our knowledge is the first report of such a communication. We discuss the clinical relevance particularly in relation to confusion with the marginal mandibular branch of the facial nerve when operating in the submandibular region.

The aim of this study was to investigate pulsatile tinnitus as a presenting symptom in cervicocephalic arterial dissection (CCAD). Of the 136 consecutive patients with confirmed CCAD, 16 presented with pulsatile tinnitus. Between January 1981 and June 1990, 36 previously untreated patients with squamous cancer of the base of the tongue were treated with radiation therapy.
