Effects of Image Degradation on Deep Neural Network Classification of Scaphoid Fracture Radiographs: Comparison Study of Different Noise Types.

BACKGROUND: Deep learning models have shown strong potential for automated fracture detection in medical images. However, their robustness under varying image quality remains uncertain, particularly for small and subtle fractures, such as scaphoid fractures. Understanding how different types of image perturbations affect model performance is crucial for ensuring reliable deployment in clinical practice. OBJECTIVE: This study aimed to evaluate the robustness of a deep learning model trained to detect scaphoid fractures in radiographs when exposed to various image perturbations. We sought to id

Tripolar Dislocation: Elbow Terrible Triad-Essex-Lopresti Injury-Trans-scaphoid Perilunate Dislocation: A Case Report.

CASE: A 23-year-old man presented with a right upper limb injury after a 10-m fall. Initial evaluation demonstrated a terrible triad elbow injury associated with a trans-scaphoid perilunate dislocation. Elbow stabilization with radial head replacement and carpal fixation was performed. Radiographs on postoperative day 7 demonstrated an ipsilateral Essex-Lopresti injury, which had been initially missed, and revision surgery was performed to reconstruct the interosseous membrane. CONCLUSION: Surgeons should maintain a high degree of suspicion for an ipsilateral Essex-Lopresti injury in patients

Assessing the outcome of disorders of the hand. Is the patient evaluation measure reliable, valid, responsive and without bias?

The different attributes of the Patient Evaluation Measure (PEM) questionnaire were investigated in 80 patients with a fracture of the scaphoid. Assessments were made at 2, 8, 12, 26 and 52 weeks. Reliability was assessed by measurement of the internal consistency of the different questions in 275 completed PEM forms. Cronbach's alpha, which needs to lie between 0.7 and 0.9, was 0.9 for the PEM. Pain, tenderness, swelling, wrist movement and grip strength correlated with the PEM score confirming the validity of the assessment. Changes in the different variables between visits correlated signif

Functional outcome with scaphotrapeziotrapezoid arthrodesis in the treatment of Kienböck's disease stage III.

Scaphotrapeziotrapezoid (STT) arthrodesis for the treatment of Kienböck's disease is recommended as a wrist salvage procedure since the publication by Watson and colleagues in 1985. Stage IIIa/b of the Lichtman classification is the specific indication for this operative-procedure. This study reviews the results of 26 patients with stage III of Kienböck's disease treated with STT arthrodesis during a 6-year period. From 1993 to 1998, 26 patients (16 men, 10 women) were treated with STT arthrodesis for Kienböck's disease in stage IIIa/b. The mean follow-up was 35 months. Two-rung grip strength

Scapholunate ligament reconstruction using a bone-retinaculum-bone autograft.

This study was designed to evaluate the use of a locally available bone-retinaculum-bone graft in the reconstruction of the completely torn scapholunate (SL) ligament. Nineteen consecutive patients (14 with dynamic instability and 5 with static instability) underwent SL ligament reconstruction using an autogenous bone-retinaculum-bone graft taken from the third dorsal compartment region. All patients underwent arthroscopy to document a torn SL ligament. The bone plugs on the graft were fitted into the dorsal scaphoid and lunate, respectively, with the retinaculum periosteal soft tissue interve

Computer Modelling of Wrist Biomechanics: Translation into Specific Tasks and Injuries.

BACKGROUND: The carpus is a complicated and functionally challenged mechanical system, advancements in the understanding of which have been compromised by the recognition that there is no standard carpal mechanical system and no typical wrist. This paper covers components of a larger project that seeks to develop a kinetic model of wrist mechanics to allow reverse analysis of the specific biomechanical controls or rules of a specific patient's carpus. Those rules, unique to each patient, could be used to create a forward synthesis mathematical model to reproduce the individual's anatomical mot

16 detector multislice CT versus skeletal scintigraphy in the diagnosis of wrist fractures: value of quantification of 99Tcm-MDP uptake.

To compare the measured uptake of 99Tcm-methylene diphosphonate (99Tcm-MDP) in those scaphoid fractures seen on both 16 detector multislice CT and scintigraphy, with those seen only on scintigraphy. Over a 12 month period a total of 51 patients with suspected fracture underwent both conventional 99Tcm-MDP scintigraphy and 16 detector multislice CT on the same day. The 99Tcm-MDP uptake was then quantified in patients with identified fracture. This was measured by placing a region of interest (ROI) over the fracture site and the mean and maximum number of counts were compared with those in a sim

The influence of anatomical shape variations of wrist bones on bone orientation values in CT scans.

INTRODUCTION: Four-Dimensional Computed Tomography (4DCT) shows promise in diagnosing scapholunate ligament (SL) lesions. Wrist motion analysis requires local coordinate systems (LCS) for carpal bones, which might be affected by bone shape variations. These variations can affect the extraction of scapholunate angle (SLA) and capitolunate angle (CLA), indicative for SL lesions. This study characterizes the impact of anatomical shape variations on LCS determination and subsequent SLA and CLA estimates. METHODS: A statistical shape model (SSM) was created for the scaphoid, lunate, and capitate u

[Effect of dia- and percondylar humeral fractures on the growth of the carpal bones in children].

Using surface measurements obtained from anterior-posterior X-rays, the carpal bones on the side of the previous supracondylar fracture were compared with those on the uninjured side. A disparity in growth was found between the two sides. Most often, an increased growth of the scaphoid was seen on the side of the fracture. Analysis of the data obtained from the measurements of all carpal bones did not lead to any definite conclusions. Of the seven carpal bones studied, a greater length (1 to 3 mm.) was found 86 times and a greater width (1 to 4 mm.) 53 times. Decreased length (1 to 4 mm.) was

Musculoskeletal Disorders of the Elderly.

Elderly of ages greater than or equal to 65 years represent nearly 20% of the US population. Physiological functions of nearly all organ systems decline and the prevalence of major diseases increase with aging. Progressive musculoskeletal disabilities of the elderly with vestibular and proprioceptive neural dysfunction lead to poor balance, unsteady gait, and frequent falls that are the leading cause of fatal and non-fatal injuries in those greater than or equal to 65 years of age. Osteoporosis, defined by a marked decrease in mineral bone density, as quantified by dual-energy x-ray absorptiom

The scapho-capitate syndrome: a case report with follow-up of three years.

The scapho-capitate or Fenton syndrome is characterized by an associate fracture of the scaphoid and capitate with rotation of 90° or 180° of the capitate's head. We report a case of this syndrome which occurred in a fifteen years old professional motorcyclist who came to our observation following a high-energy trauma that occurred during the track tests. Through a dorsal access the fractures of the capitate and scaphoid were reduced and stabilized with Kirschner wires. The patient was radiographically and clinically evaluated three years after surgery; complete healing of scaphoid fracture an

Scaphoid screw fixation perpendicular to the fracture plane: Comparing volar and dorsal approaches.

INTRODUCTION: To percutaneously fixate a midwaist scaphoid fracture, both volar and dorsal approaches are considered valid options although they may have different screw insertion angles relative to the scaphoid fracture plane influencing fixation stability. In this virtual simulation study, we investigated the accessibility of placing a screw perpendicularly to the fracture plane in transverse and horizontal oblique scaphoid midwaist fracture models and compared standard volar and dorsal approaches. MATERIAL AND METHODS: Computed tomography scans of 38 healthy wrists were used to obtain virt
