Decision making under ambiguity in temporal lobe epilepsy: does the location of the underlying structural abnormality matter?

Previous studies have reported decision-making deficits in patients affected by mesial temporal lobe epilepsy (TLE). The aim of the study described here was to assess the specificity of these deficits by comparing performance of patients with neocortical TLE, patients with mesial TLE, and healthy controls. The mesial TLE group performed lower than healthy controls and the neocortical TLE group in decision making under initial ambiguity. Although patients with neocortical TLE and controls showed a significant learning effect over the blocks of the Iowa gambling task, performance of patients wit…

Medial temporal lobe structures are needed to re-experience remote autobiographical memories: evidence from H.M. and W.R.

The nature and extent of retrograde amnesia in patients with medial temporal lobe (MTL) lesions is currently under debate. While some investigators propose a temporally limited role for the MTL in episodic and semantic memory, others claim that MTL structures are needed for episodic memories of one's entire lifetime, and that only semantic memory becomes independent of the MTL. To address this issue, we tested two amnesic patients, H.M. and W.R., with bilateral MTL lesions on a series of remote memory tests that together distinguished episodic memory from semantic memory performance. Notably, …

High-grade atrioventricular block triggered by spontaneous and stimulation-induced epileptic activity in the left temporal lobe.

Cardiac bradyarrhythmias may play a pivotal role in the pathophysiology of sudden unexpected death in epilepsy (SUDEP). We describe a patient with left temporal lobe epilepsy in whom high-grade atrioventricular conduction blocks were triggered by both spontaneous and stimulation-induced epileptic activity in the left temporal lobe. Electrophysiological data obtained by surface and intracranial electrodes point to a cerebral cardioarrhythmogenesis in the left amygdala and anterior hippocampus.

Anterior medial temporal lobe volumes in polydipsic schizophrenic patients with and without hypo-osmolemia: a pilot study.

The volume of certain brain structures, particularly those in the anterior medial temporal lobe, may be reduced to a greater extent in hyponatremic/hypo-osmolemic polydipsic schizophrenics than other schizophrenic patients. To explore if volume reduction is specific to this particular brain region, and if it is fundamentally associated with polydipsia, we imaged the anterior hippocampi/subicula and adjacent temporal lobes in normal males (n = 10) and polydipsic schizophrenic patients with (n = 7) and without (n = 6) hypo-osmolemia. Bilateral hippocampal/subicular (p < .01), but not temporal lo…

Olfactory prodromal symptoms and unilateral olfactory dysfunction are associated in patients with right mesial temporal lobe epilepsy.

PURPOSE: We report a patient with mesial temporal lobe epilepsy (MTLE) with olfactory prodromal symptoms manifested as an unpleasant smell of onions, who was found to have an ipsilateral deficit of olfactory naming (olfactory agnosia). METHODS AND RESULTS: Preoperative olfactory testing revealed a selective right-sided olfactory deficit for naming of odors. Olfactory threshold was within the normal range. The patient has been seizure free after selective amygdalohippocampectomy for 4 months. No olfactory prodromal events have occurred since surgery. Olfactory testing 3 months after resection …

Anatomical aspects, technical nuances, and a case series of the resection of the inferior temporal gyrus as a strategy to access the basal surface of the temporal lobe and the lateral incisural space.

BACKGROUND: Pathologies of the basal surface of the temporal lobe (TL) and the lateral incisural space (IS) commonly present microsurgical challenges. Since the inferior temporal gyrus (ITG) is part of both the lateral and the basal surfaces, it has a close relationship with the basal surface of the TL and lacks major white matter tracts, reducing the risk of eloquence. This study aims to describe the technique of ITG resection, its applications in four surgical cases, and the relevant anatomical aspects. METHODS: An anatomical review of the TL and the IS was performed. Anatomical pieces fixe…

Modeling the microRNA regulation of TGF-β/SMAD signaling pathways for seizure control in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is the most prevalent type of focal epilepsy. Recent developments in sequencing, proteomics and network analysis tools provide new avenues for investigating potential molecular therapeutic targets. Both the TGF-β/SMAD signaling pathways and subsets of microRNAs (including miR-21a-5p, miR-142a-5p, and miR-10a-5p) have been shown to be altered in several preclinical models of epilepsy and were mathematically modeled in this study. Using prior systems-based findings, a changeover between 'seizure' and 'anti-seizure' cellular states has been identified upon inhibition …

Systems Biology of Mesial Temporal Lobe Epilepsy and Role of Iron-Related Gene Expression in Its Pathophysiology.

Mesial temporal lobe epilepsy (MTLE) is the most common form of drug-resistant epilepsy in adults, yet its molecular pathogenesis remains elusive. While iron dysregulation has been implicated in MTLE, transcriptome-level regulation of iron-related genes in MTLE brain, including regional, subcellular, and pathology-specific patterns, remains largely unexplored. We analyzed publicly available nuclear and cytoplasmic RNA-sequencing data from hippocampal and cortical tissues of patients with MTLE with and without hippocampal sclerosis and controls. We identified differential expression among 562 c…

Dynamic Analysis of Gray Matter and White Matter Functional Network Connectivity in Individuals with Temporal Lobe Epilepsy.

Accurately localizing the epileptic zone in temporal lobe epilepsy (TLE) is challenging. Emergent resting-state functional connectivity (rs-FC) analyses have demonstrated that the epileptic network of TLE likely involves both gray (GM) and white matter (WM). Clustering dynamic functional network connectivities (dFNCs) reveals time-varying dependencies of functional networks (FNs) and may provide additional insight into TLE, as interictal discharges are often transient and spread across FNs. In this study, we analyzed data from 103 participants from the Epilepsy Connectome Project (ECP), includ…

Persistent white matter disruption underlies apparent functional normalization in intractable temporal lobe epilepsy: Evidence from multimodal MRI.

BACKGROUND: Intractable temporal lobe epilepsy (ITLE) poses ongoing therapeutic challenges due to resistance to antiseizure medications and limited improvements in patient quality of life. Although neuroimaging studies have identified both functional and structural abnormalities in ITLE, their combined analysis and relevance to treatment refractoriness remain unclear. METHODS: We collected longitudinal rs-fMRI and DTI data from 29 patients with ITLE (pre-/post-treatment) and 25 healthy controls and extracted ALFF/ReHo and FA/MD/AD/RD features. Primary inferences were based on conventional gro…

Predictors of long-term seizure outcomes and perceived quality of life outcomes after temporal lobe epilepsy surgery.

BACKGROUND: Balancing seizure outcomes and cognitive deficits is always a challenge in both mesial and neocortical temporal lobe epilepsy cohorts. The present study evaluated the long-term seizure outcomes and perceived quality of life (QOL) in both of these groups, along with factors predictive of these outcomes. METHODS: A retrospective analysis of patients undergoing temporal lobe epilepsy (TLE) surgery from 2015 to 2023, with at least 1 year of follow-up (N = 175), is presented. Patients were grouped as MTLE (mesial temporal sclerosis only) and NTLE (neocortical) based on neuroimaging. Su…

Long-term seizure outcomes in patients with drug-resistant mesial temporal lobe epilepsy due to hippocampal sclerosis.

OBJECTIVE: Resective surgery is a significant therapeutic option for patients with drug-resistant mesial temporal lobe epilepsy (MTLE) due to hippocampal sclerosis (HS). Numerous studies have demonstrated the efficacy of epilepsy surgery in the short to median term. However, studies on long-term outcomes are limited in number. The objective of this study was to document the long-term prognosis and associated factors of patients undergoing resective surgery for MTLE. METHODS: A retrospective analysis was conducted on the data of patients with drug-resistant MTLE due to HS who underwent resecti…

Commonalities and distinctions of static and dynamic functional connectivity density between left and right temporal lobe epilepsy.

To comprehensively examine static and dynamic functional connectivity density (FCD) in left temporal lobe epilepsy (LTLE) and right temporal lobe epilepsy (RTLE). For 46 LTLE patients, 43 RTLE patients, and 53 healthy controls (HCs), T1-weighted structural images and resting-state functional images were collected. Static FCD and corresponding temporal dynamic FCD (dFCD) obtained via a sliding window approach were measured and compared. Additionally, seed-based functional connectivity (FC) analysis was executed. Relationships between cognitive scores and FCD or dFCD values were analyzed. Compar…

Distributed functional cross-network connectivity of the epileptogenic zone predicts late seizure relapse in temporal lobe epilepsy.

OBJECTIVE: Although many patients with temporal lobe epilepsy achieve seizure freedom in the first post-operative year, the likelihood of long-term seizure freedom declines substantially over time. Late seizure relapse may reflect ongoing epileptogenicity, potentially driven by persistent epileptic activity arising from the epileptogenic zone. We hypothesized that its more distributed connectivity profile across brain networks would distinguish patients experiencing late relapse. METHODS: We prospectively enrolled 209 patients with drug-resistant temporal lobe epilepsy across six centers who …

Developmental exposure to legacy environmental contaminants, medial temporal lobe volumes and spatial navigation memory in late adolescents.

Exposure to lead, mercury, and polychlorinated biphenyls (PCBs) has been causally linked to spatial memory deficits and hippocampal changes in animal models. The Inuit community in Northern Canada is exposed to higher concentrations of these contaminants compared to the general population. This study aimed to 1) investigate associations between prenatal and current contaminant exposures and medial temporal brain volumes in Inuit late adolescents; 2) examine the relationship between these brain structures and spatial memory; and 3) assess the mediating role of brain structures in the associatio

Photopharmacological activation of adenosine A1 receptor signaling suppresses seizures in a mouse model for temporal lobe epilepsy.

Up to 30 % of epilepsy patients suffer from drug-resistant epilepsy (DRE). The search for innovative therapies is therefore important to close the existing treatment gap in these patients. The adenosinergic system possesses potent anticonvulsive effects, mainly through the adenosine A1 receptor (A1R). However, clinical application of A1R agonists is hindered by severe systemic side effects. To achieve local modulation of A1Rs, we employed a photopharmacological approach using a caged version of the A1R agonist N6-cyclopentyladenosine, termed cCPA. We performed the first in vivo study with intr

Late-onset temporal lobe epilepsy: Enlarged amygdala-hippocampus indicates interictal epileptic activity at electroencephalography and memory impairment.

BACKGROUND: Late-onset temporal lobe epilepsy (TLE) sometimes manifests with enlargement of the amygdala or hippocampus. This study aimed to clarify the yet unknown clinical significance of amygdala-hippocampal enlargement in late-onset TLE. METHODS: We retrospectively analyzed the late-onset TLE (≥ 60 years old) patients admitted to Kyoto University Hospital between 2000 and 2019. To investigate whether the patient group has enlarged amygdala or hippocampus compared to the healthy control, we performed MRI volumetry with hippocampal subfield segmentation. Furthermore, to evaluate the associa…

Stereotactic radiofrequency thermocoagulation selectively targeting the subiculum for mesial temporal lobe epilepsy -Treatment concept and preliminary results.

OBJECTIVE: The recently emerging laser interstitial thermal therapy is becoming a substitute treatment for mesial temporal lobe epilepsy (MTLE) due to its less invasiveness, but it offers lower seizure-free rates than traditional open surgery. Another less invasive stereotactic ablation surgery, that is radiofrequency thermocoagulation, is also an alternative surgical procedure. In particular, magnetic resonance imaging-guided stereotactic radiofrequency thermocoagulation (MRgSRFTC) has shown excellent seizure outcomes. Recently, the subiculum has been reported to have a more important role in…
