<title>BACKGROUND</title>Pleural TB (PTB) is treated with standard anti-TB therapy, but additional treatments may be necessary to mitigate restrictive functional sequelae (RFS). This study identifies effective adjunctive management options to prevent PTB sequelae.
<title>METHODS</title>We systematically reviewed studies from PubMed, Cochrane Library, EMBASE, CINAHL, Web of Science, and Ovid, focusing on PTB treatments like oral steroids, pleural drainage, intrapleural therapy, and pleuroscopy. Our primary goal was to assess the impact on pulmonary function test (PFT) results, ad

In this study, copper, zinc, magnesium, manganese, selenium, and iron were measured by inductively coupled plasma atomic emission spectrophotometry in patients with tuberculous (TB) pleurisy (n = 24) and in the control group (n = 20). Selenium, magnesium, and zinc were found to be significantly lower in TB pleurisy cases than those in the control group (p < 0.01, p < 0.001, and p < 0.001, respectively). There was no significant difference in the value of manganese and iron between TB pleurisy and the control group (p > 0.05). Copper levels were significantly increased in the serum of the patie

A 13-year-old boy presented with a history of respiratory infection that had progressed for 3 months. A chest X-ray showed pathological findings that suggested endothoracic fascia compromise (Skarby sign). The thorax computerized axial tomography scan revealed multiple opacities in the fascia. A biopsy was performed, and a pathological lymph node with caseous material was obtained. Thirty days later, Mycobacterium tuberculosis var. hominis was obtained from the culture. The diagnosis of tuberculosis of the endothoracic fascia was confirmed. Treatment included 9 months with isoniacide and rifam

In a patient with an undiagnosed pleural effusion, the first question to answer is whether the fluid is an exudate or a transudate. This is usually determined by means of Light's criteria, which differentiate transudative effusions from exudative effusions by measuring the levels of total protein and lactate dehydrogenase in the pleural fluid (PF) and serum. In patients under diuretic treatment, Light's criteria misclassify transudates as exudates, but the serum to pleural fluid albumin gradient usually remains above 12 g/L. When tests are done only in PF, protein concentration >30 g/L perform

BACKGROUND: Tuberculous pleural effusion (TPE) is usually characterized by lymphocytic predominance and high pleural fluid adenosine deaminase (pfADA), while parapneumonic pleural effusion (PPE) is usually characterized by neutrophilic predominance. However, in some cases, neutrophils can be predominant in TPE. In such cases, the differential diagnosis between TPE and PPE is challenging and has been rarely investigated. The aim of this study was to evaluate the accuracy of pfADA, pleural fluid lactate dehydrogenase (pfLDH) and other parameters, such as age/pfADA in the differential diagnosis o

Pleural fluids obtained from 26 patients with tuberculous pleurisy (T-group), 11 with parapneumonic pleurisy (B-group) and 21 with malignant pleurisy (M-group) were tested for their biologic parameters and cytokine concentrations. 1) The average age of T-group was over 10 years lower than that of M-group with a statistically significant difference. 2) The average CRP value of B-group and the positivity on PPD skin test of T-group were higher than those of the other groups, respectively. 3) Yellowish pleural fluids were mainly observed in T- and B-group, while bloody pleural fluids were mostly

BACKGROUND: Early and accurate diagnosis of tuberculous pleurisy (TP) remains a challenge. The aim of the present study is to evaluate the performance of the pleural fluid (PF) T-SPOT and interferon-gamma (IFN-γ) for TP diagnosis in high tuberculosis (TB) burden settings.
METHODS: In total, 214 and 217 subjects suspected of TP were prospectively enrolled in the Wuhan (training) cohort and Changchun (validation) cohort, respectively. All patients were examined with PF T-SPOT, IFN-γ, and other traditional tests simultaneously.
RESULTS: The receiver-operating characteristic (ROC) curve analysis

After the start of anti-tuberculous treatment, paradoxical worsening of tuberculous lesions has been described. However, abdominal tuberculosis as paradoxical response is relatively rare. This report describes the 26-year-old female who suffered from peritoneal tuberculosis while treating tuberculous pleurisy with anti-tuberculous medications. It was considered as paradoxical response, rather than treatment failure or else. She was successfully managed with continuing initial anti-tuberculous medications. When a patient on anti-tuberculous medications is presented with abdominal symptoms, the

Toll-like receptors (TLRs) play an important role in mediating the down stream signaling of immune response in tuberculosis. The predominance of Th1 response in tuberculous pleurisy prompted us to study the expression profiles of TLR2 and TLR4 on different immune cells and on subsets of T cells obtained from the site of infection. Our results showed that TLR2 was up-regulated on the monocytes from pleural fluid indicating a prominent role for this receptor in anti-tuberculous immunity. Notably, TLR2 and TLR4 expression were also enhanced on IFN-gamma secreting CD4(+)T cells. However, their exp

Extrapulmonary tuberculosis (EPTB) is a multifaceted disease that could potentially impact nearly all organs. Current global data indicate a significant variation in the proportion of EPTB among all tuberculosis cases, ranging from 15% to 53%. Clinicians in India express concerns about the efficacy of the cartridge-based nucleic acid amplification test (CBNAAT) in EPTB, as its yield frequently does not align with the findings of the World Health Organization meta-analysis. Hence, the present study was conducted to evaluate the diagnostic yield of CBNAAT in EPTB at a tertiary care hospital. Spe

BACKGROUND: The cantilever method is a standard for two-dimensional deformity correction, as in spondylitis tuberculosis kyphotic deformity. An accurate and secured pedicle screw placement as part of the correction tools is essential to accommodate reduction while preventing screw-related complications. Many literatures have described the pedicle screw misplacement in cases with "normal" bone quality (ie, scoliosis, Scheuermann's kyphosis, ankylosing spondylitis, trauma) or in the obviously abnormal bone such as osteoporosis. However, to our knowledge, the pedicle screw accuracy in cases of de

OBJECTIVES: A bronchopleural fistula after pneumonectomy is a relatively rare but very serious complication. The development of endoscopic methods of treatment opens a new page in treating this condition. The goal of this paper was to confirm that the atrial septal defect Amplatzer device can be used for bronchopleural fistula closure in properly selected patients.
METHODS: A retrospective study of 13 patients with bronchopleural fistula after pneumonectomy was performed. There were 11 men and 2 women aged 26-70 years. Right-sided fistulas occurred in 10 patients and left-sided fistulas occur

BACKGROUND: Tuberculous pleurisy (TP), a predominant form of extrapulmonary tuberculosis, presents significant diagnostic challenges attributable to the paucibacillary nature of pleural effusion (PE) specimens. Cell-free Mycobacterium tuberculosis (MTB) DNA in PE represents a promising biomarker for TP diagnosis. This study aimed to develop and assess a novel cell-free DNA (cfDNA)-CRISPR assay targeting MTB DNA in PE supernatants.
METHODS: Patients with suspected TP were prospectively enrolled at Beijing Chest Hospital. PE samples underwent centrifugation, with sediments tested by MTB/RIF Xpe
