[Toxic damage of the optic nerve in treatment with ethambutol in a patient on chronic dialysis].

The authors give an account of a 57-year-old male patient who was regularly dialyzed for two years and treated because of suspect TB of the spine with antituberculotics, incl. ethambutol. In the course of 8-9 months he was given 17.9 mg/kg ethambutol per day. Because of the practically zero excretion by the damaged kidney the drug cumulated in the serum and reached toxic levels before dialysis. Gradual deterioration of eyesight developed (central acuity and colour sense) which resulted in bilateral atrophy of the optic nerves with concentric narrowing of the visual field.

[Bronchial fistula caused by Pott's abscesses. Report of 3 cases].

The authors report 3 cases of intrabronchial fistulisation of paravertebral abscesses due to tuberculous spondylitis of the thoracic spine. This complication is relatively rare (these 3 cases were in a series of 178 cases of Pott's disease) and is easy to diagnose due to its characteristic radio-clinical features. As mentioned in old publications, this type of fistulisation does not seem to have serious consequences. There are no particular aspects of treatment.

Anterior intervertebral disc herniations in children. Report of four cases.

Four boys with anterior lumbar disc herniations probably secondary to chronic trauma are reported. Anterior disc herniations may appear as separate lesions-as in our patients-or coexist with Scheuermann's disease. Although Scheuermann's disease, discitis and tuberculosis are regarded as the most common causes of intervertebral disc lesions in childhood, trauma should be excluded as a possible cause in all children with chronic back pain who show localised radiographic signs of narrowed disc spaces with erosive and sclerotic changes in the adjacent vertebrae. Localised mild platyspondly may be

[Urogenital tuberculosis. Apropos of 40 cases].

The authors report a series of 40 cases of genitourinary tuberculosis diagnosed and treated in the department of urology "B" of Avicenne hospital over a 7-year period. The objective of this study is to define the various diagnostic and therapeutic aspects of this disease. The patients were predominantly males (62.5%) with a mean age of 40 years. 25% of cases reported a history of extra-urinary tuberculosis. The very polymorphous clinical presentation is dominated by signs of cystitis (45%). Intravenous urography is frequently suggestive of the diagnosis based on the appearance and multiplicity

Long-term effect of bronchial artery embolization in Korean patients with haemoptysis.

OBJECTIVE AND BACKGROUND: Bronchial artery embolization (BAE) has been regarded as a bridging therapy in the management of massive haemoptysis until a more definite therapy can be pursued. The long-term effectiveness of BAE and the factors associated with failure to control bleeding in an Asian setting of tuberculosis are unknown and were investigated. METHODS: Over approximately 4 years, 139 patients received BAE to treat haemoptysis at a single centre, of these, 118 had been followed up for more than 1 year (median 23 months) and were retrospectively recruited into the study. Patients were

[Diagnostic value of ultrasonically guided lung aspiration in pneumonia].

To determine the diagnostic value of ultrasonic lung aspiration for patients with pneumonia, 60 patients with a tentative diagnosis of pneumonia were included in this study. After recording ultrasonographic findings, lung aspiration was done with a spinal needle and aspirated specimens were sent for Papanicolaou, May-Giemsa, acid fast, and Gram stains. The remaining specimens were sent for bacterial, mycobacterial and fungal culture. Twelve patients were excluded from the study because of the final diagnosis of non-infectious pulmonary diseases. In 28 cases of bacterial pneumonia, the diagnost

Successful management of Pott's disease in a young woman with 32 weeks of pregnancy: a rare case report.

Spinal tuberculosis (Pott's disease) during pregnancy is rare and poses diagnostic challenges due to overlapping symptoms with pregnancy-related changes, potentially leading to severe neurological deficits if untreated. We report an 18-year-old primigravida at 32 weeks gestation presenting with progressive lower limb weakness, urinary incontinence, and upper back pain. Neurological examination showed bilateral lower limb paralysis, hypertonia, exaggerated reflexes, and sensory loss up to T10. Contrast-enhanced MRI revealed tubercular spondylo-discitis at T3-T5 with partial T4 collapse, paraver

Myelin oligodendrocyte glycoprotein-antibody disease (MOGAD) with leukodystrophy-like presentation.

Myelin oligodendrocyte glycoprotein-antibody disease (MOGAD) is an immune-mediated demyelinating disorder, distinct from MS, which typically affects the optic nerve, spinal cord, brain and/or brainstem. Diffuse white matter involvement that resembles a leukodystrophy has been reported only in children. We present a woman aged 29 years with new right optic neuritis on a background of previously unexplained, longstanding, bilateral, reduced visual acuity. MR scan of the brain and orbits showed confluent, bilateral, posterior predominant white matter changes reminiscent of a leukodystrophy, along

[Myelitis Associated with Visceral Leishmaniasis Due to Leishmania infantum in Burkina Faso: A Case Report].

INTRODUCTION: Visceral leishmaniasis with Leishmania infantum (VL/LI) remains unreported in West Africa. We describe here an unprecedented autochthonous case of VL/LI in Burkina Faso, associated with myelitis. CASE REPORT: A teenage girl was hospitalized for prolonged fever, flaccid paraplegia, severe pancytopenia, hepatosplenomegaly, and immobilization-related pulmonary embolism with pleural effusion. Immunological, hematological, and microbiological investigations, including tuberculosis, particularly on pleural, bone marrow fluids, and salivary gland biopsy were non-diagnostic. Spinal MRI

Diffuse leptomeningeal glioneuronal tumor mimicking chronic inflammatory and tuberculous meningitis.

Diffuse leptomeningeal glioneuronal tumors (DLGNT) are rare central nervous system neoplasms characterized by the absence of a primary brain or spinal cord tumor. Patients exhibit common radiological and histopathological features; however, the clinical progression varies. We present the case of a 4-year-old girl initially diagnosed with constipation and failure to thrive, who subsequently developed neurological signs during hospitalization. Subsequently, she was referred with suspected tuberculosis meningitis, despite negative cerebrospinal fluid (CSF) culture results, and the patient presuma

Multifocal Skeletal Tuberculosis - A Case Report.

INTRODUCTION: Tuberculosis (TB) remains a pervasive infectious disease, and extrapulmonary manifestations account for a significant proportion of cases. Skeletal involvement is observed in about 10% of extrapulmonary TB instances. While spinal TB is relatively common, sacroiliac (SI) joint TB is a rare occurrence, particularly in immunocompetent individuals. CASE REPORT: This case report discusses an unusual presentation in a 19-year-old immunocompetent lady where the spine, acetabulum, and SI joint were concurrently affected. The patient, initially undergoing alternative medicine, exhibited

Neurobrucellosis complicated by primary pyogenic ventriculitis: a case report.

Neurobrucellosis is a serious focal brucella infection. Ventriculitis is a special form of central nervous infection where pyogenic infection of the ependymal linings hinders antibiotics' accessibility to the cerebrospinal fluids and leads to protracted infection. We present a case of a 37-year-old Shepherd who had low-grade fever for 5 months followed by a brief history of vomiting, abdominal pain, and gait imbalance. Investigations showed neutrophilic leukocytosis, high titers of serum anti-brucella antibodies, and lymphocytic pleocytosis. Mycobacterial tuberculosis workup was negative. Magn

Multimodality imaging workup of classical Hodgkin lymphoma in an 8-month-old child.

The incidence of Hodgkin lymphoma (HL) varies by age, most commonly affecting 15-19-year-olds. Cases in children less than 3 years old are exceedingly rare. We report a case of classical HL in an 8-month-old male; the youngest case reported thus far in the literature to our knowledge. Furthermore, while lymphadenopathy is a salient feature of HL, it was absent in our patient, who presented with immunodeficiency and delays in achieving neurologic milestones. A thorough radiologic workup demonstrated bilateral paravertebral masses, collapse of the T3 vertebrae, and severe spinal cord compression

A Case Report of Rare Presentation and Delay in the Diagnosis of Neuropathic Abdominal Pain Secondary to Asymptomatic Pott's Spine in Elderly Patients.

The presence of vertebral tuberculosis (TB) in developing countries and Southeast Asia is well known, but developed nations such as the USA and UK also claim a good share because of immigrants and the HIV population. We present a unique case series of two patients with chronic abdominal pain where various differentials and arduous investigation were employed. Finally, after a few months, we could locate the lower thoracic Pott's spine and commenced the treatment with successful resolution of symptoms. Surgeons and gastroenterologists should rule out the spinal cause of abdominal pain and also
