Long-term outcome of ablation for anal HSIL: Achieving HSIL-free status is key-A UK-based retrospective cohort study.

INTRODUCTION: Treating anal high-grade squamous intraepithelial lesions (HSIL) reduces squamous cell carcinoma (SCC) rates in high-risk groups such as men having sex with men (MSM), especially those living with HIV (LWH). Follow-up data after HSIL treatment is lacking. METHOD: A retrospective analysis was conducted of consecutive male patients with biopsy-proven HSIL undergoing high resolution anoscopy (HRA)-based ablation and subsequent surveillance at a UK specialist centre until either latest HRA or cancer diagnosis. The primary objective was the proportion becoming HSIL-free after treatme

[Cylindric Abdominoperineal Rectum Exstirpation with Partial Vulvar and Vaginal Resection as well as Perineal and Vaginal Defect Reconstruction by a Vertical Rectus Abdominis Myocutaneous (VRAM) Flap].

Introduction Patients with low rectal cancer or anal cancer undergoing abdominoperineal excision (APE) benefit from extended surgery and the subsequent avoidance of surgical "waisting" at the level of the puborectalis muscle. The method of cylindrical APE was introduced by T. Holm and led to a reduction of intraoperative perforations and involvement of circumferential resection margins, and subsequently reduced the risk of local recurrence. The use of myocutaneous flaps reduces perineal wound complications, which occur in up to 60% of patients with primary closure of perineal defects, especial

Conservative treatment by irradiation of epidermoid carcinomas of the anal margin.

PURPOSE: Few series have described treatment results of anal margin tumors as defined in the UICC 87 classification. The purpose of this article is to describe experience with an uncommon condition in a single cancer center. METHODS AND MATERIALS: From 1971 to 1995, 32 patients with carcinoma of the anal margin were irradiated with a curative intent, and 31 were followed-up for more than 6 months (mean = 4.5 years). There were 9 T1, 15 T2, 7 T3, and 1 T4 (vulvar), with 26 N0 and 6 N1. There was a minor invasion of the lower canal in 17 patients. The histological types were 24 squamous cell an

Case report: role of granulocyte colony stimulating factor in radiotherapy.

Two patients who received granulocyte colony stimulating factor (G-CSF) before or during radiotherapy are reviewed. Both patients developed granulocytopenia after multi-agent chemotherapy. One patient had breast cancer with multiple distant metastasis and received palliative radiotherapy to the spine and hip joint. The other patient had locally advanced anal carcinoma and received concurrent 5-fluorouracil, mitomycin, and radiation therapy. Both patients completed the planned course of radiotherapy without interruption, after administration of G-CSF.

Ten year follow-up study of women presenting to a genitourinary medicine clinic with anogenital warts.

One hundred heterosexual women presenting at our clinic in 1979 with anogenital warts, were reviewed 10 years later. Median duration of warts following initial clinic attendance was 2 months (range 0-120 months). In 1979 cervical PAP smear results were available for 76 patients; cervical intraepithelial neoplasia (CIN) was seen in 15/76 (19.7%) women; 3 (4%) women had low grade CIN, 12 (15.7%) women had high grade CIN. Nineteen women had had treatment for CIN between 1979 and 1989, 7 laser ablation, 9 cone biopsy, 2 laser ablation and cone biopsy, and one woman laser ablation, cauterization an

Screening colposcopy in genitourinary medicine.

A prospective study of new female patients attending the Genitourinary Medicine Department in Doncaster was carried out to look at the value of colposcopic examination at their first attendance. Of the 100 randomized patients studied 41 showed evidence of colposcopically detected cervical abnormality. Twenty-seven patients had colposcopically directed biopsies. These showed evidence of cervical intraepithelial neoplasia (3 cases), flat condyloma (16 cases), exophytic wart (2 cases), chronic inflammation (2 cases) and normal epithelium (4 cases). Colposcopic cervical abnormalities were signific

Efficacy of chemical and/or surgical therapy against condylomata acuminata: a retrospective evaluation.

In order to estimate treatment resistance of condylomas, standardized records of 230 males aged 17-80 years (mean 27) attending at an STD clinic were analysed retrospectively. Anal warts occurred in 38 (17%) men; in 19 (50%) intranal warts were extensive enough to require further referral to the Department of Surgery. Urinary meatal warts occurred in 30 men; 9 of them (30%) were referred to the Department of Urology. The remaining 202 (88%) men were treated in the STD clinic with simple surgery and/or home-treatment with 0.5% podophyllotoxin-ethanol twice a day for one or more 3 day sessions.

Enhanced detection of high-risk human papillomavirus by in situ hybridization with expansion from 7 to 18 genotypes in anal biopsies.

OBJECTIVES: High-risk human papillomavirus (HPV) plays a central role in the pathogenesis of anal high-grade squamous intraepithelial lesions (HSILs). In situ hybridization (ISH) is commonly used for high-risk HPV detection in surgical specimens. Recently, ISH panels with expanded HPV genotype cocktails have become available. This study evaluated the detection rate of an expanded ISH probe cocktail targeting 18 high-risk HPV genotypes (HR18) compared with a 7-HPV genotype panel (HR7) in anal tissue microarray samples. METHODS: A retrospective study was performed on 245 patients with anal biop

AIMS: COVID-19 pandemic caused a significant disruption in healthcare services, leading to a reduction in routine check-ups as well as a shift towards virtual care. This resulted in many patients delaying or avoiding seeking medical attention for symptoms, which led to delay in diagnosis, especially for conditions such as anal cancer that require a physical examination and diagnostic endoscopy. This study aimed to highlight the impact of healthcare disruptions and resultant impact on advanced stage of anal cancer diagnosis. MATERIALS AND METHODS: This is an audit of all patients at a large ac
