The impact of COVID-19 pandemic on blood supplies and transfusion services in Eastern Mediterranean Region.

BACKGROUND: Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) has spurred a global health crisis. The safety and supply of blood during this pandemic has been a concern of blood banks and transfusion services as it is expected to adversely affect blood system activities. We aim to assess the situation in the Eastern Mediterranean Region (EMR) during the first months of the pandemic. MATERIALS AND METHODS: A survey was designed to address blood supply, transfusion demand, and donor management during the coronavirus disease-19 (COVID-19) pandemic. Medical directors of different blood

Clinical presentation and molecular identification of four uncommon alpha globin variants in Thailand. Initiation codon mutation of α2-globin Gene (HBA2:c.1delA), donor splice site mutation of α1-globin gene (IVSI-1, HBA1:c.95 + 1G>A), hemoglobin Queens Park/Chao Pra Ya (HBA1:c.98T>A) and hemoglobin Westmead (HBA2:c.369C>G).

Alpha thalassemia is the most common genetic disease in the world with the prevalence of carriers ranging from 5-50% in several populations. Coinheritance of two defective α-globin genes usually gives rise to a symptomatic condition, hemoglobin (Hb) H disease. Previously, it has been suggested from several studies in different populations that nondeletional Hb H disease (--/α(T)α or --/αα(T)) is generally more severe than the deletional type (--/-α). In this report, we describe four rare nondeletional α-thalassemia mutations in Thai individuals, including initiation codon mutation (HBA2:c.1del

Sickle cell disease in Germany - trends in epidemiology, healthcare utilization and disease burden from 2015 to 2024: a claims data analysis.

BACKGROUND: Sickle cell disease (SCD) is a rare inherited hemoglobinopathy associated with substantial morbidity and high healthcare needs. In Germany, population-based evidence on SCD epidemiology and real-world care patterns remains limited, despite increasing relevance in the context of migration and the introduction of nationwide newborn screening in 2021. METHODS: We conducted a retrospective, non-interventional study using anonymized German statutory health insurance claims data from the InGef database (2015-2024). Patients with SCD were identified using ICD-10-GM codes D57.0, D57.1, D5

Haptoglobin gene polymorphism and iron profile in sickle cell disease patients with inflammation in Yaounde, Cameroon.

BACKGROUND: Major sickle cell syndromes are the most common hemoglobinopathy in the world. The sickle cell patients are subjected to several factors causing inflammation, and the genetic identification of each individual allows to focus the possibility of allelic variations influence of a specific gene and then the polymorphism. This study aims at determining the distribution of HP gene (OMIM#140100) and their involvement on hematological parameters and the iron profile in the sickle cell patients presenting an inflammation condition during major sickle cell syndromes in Cameroun. METHODS: A

Screening for β-Thalassemia Trait in Pregnant Women With Microcytic Hypochromic Anemia and Its Impact on Pregnancy Outcomes.

Background Thalassemia is a common autosomal recessive hemoglobinopathy and a major public health concern in India. In pregnancy, β-thalassemia trait often presents as microcytic hypochromic anemia and may mimic iron deficiency anemia, leading to misdiagnosis and inappropriate management. Early and accurate differentiation is essential to prevent unnecessary iron therapy and reduce adverse maternal and neonatal outcomes. The study was designed to evaluate the prevalence, diagnostic differentiation, and pregnancy outcomes associated with β-thalassemia trait in this population. Methods This pros

Cerebral Hemodynamic Responses to Disease-Modifying and Curative Sickle Cell Disease Therapies.

BACKGROUND AND OBJECTIVES: Sickle cell disease (SCD) is a hemoglobinopathy resulting in hemoglobin-S production, hemolytic anemia, and elevated stroke risk. Treatments include oral hydroxyurea, blood transfusions, and hematopoietic stem cell transplantation (HSCT). Our objective was to evaluate the neurologic relevance of these therapies by characterizing how treatment-induced changes in hemoglobin (Hb) affect brain health biomarkers. METHODS: In this interventional study, adults with and without SCD underwent a 3T-MRI at Vanderbilt University Medical Center at 2 time points before and after

Indications and complications of splenectomy for children with sickle cell disease.

BACKGROUND: Sickle cell anemia (SCA), which is characterized by high hemoglobin (Hb) F level and persistent splenomegaly into the older age group (up to 18 years of age) or even adults, is one of the commonest hemoglobinopathies in the Eastern Province of Saudi Arabia. This makes them liable to develop splenic complications requiring splenectomy. This is a review of our experience in the management of 134 children with SCA who had splenectomy as part of their management at our hospital, with emphasis given to the indications and complications of splenectomy. PATIENTS AND METHODS: The medical …

[Course of bone tissue after bone marrow allograft in adolescents with sickle cell disease].

PURPOSE OF THE STUDY: Allogenic B.M.T. has been investigated as a curative treatment of anemia in S.C.D. Furthermore, correction of functional asplenia has been observed after B.M.T.; it remains to be determined if such a treatment may reserve other organ damage and particularly bone abnormalities and osteonecrosis frequently associated with S.C. hemoglobinopathies. MATERIAL AND METHODS: The aim of this retrospective review was to evaluate the radiologic and MRI results of 2 patients with avascular necrosis before the B.M.T. Roentgenographic diagnosis of osteonecrosis before B.M.T. included, …

Sonographic Assessment of Splenic Manifestations in Sickle Cell Disease Patients and Its Relation to Hematological Parameters: A Cross-Sectional Study in Basra, Iraq.

Background Hemoglobinopathies, such as sickle cell disease (SCD), are inherited disorders of hemoglobin (Hb) synthesis. SCD presents with complex clinical manifestations, including anemia, painful episodes, and organ damage due to recurrent vaso-occlusion. The spleen is one of the first organs affected in SCD patients, with hyposplenism typically occurring by age five. However, older patients may show variations in splenic size, which can complicate the diagnosis. Objective This study aimed to investigate the radiological manifestations of the spleen in patients with SCD using sonographic asse…

Laparoscopic splenectomy and/or cholecystectomy for children with sickle cell disease.

BACKGROUND: In 1991, Delaitre reported the first laparoscopic splenectomy (LS). Since then LS has become the procedure of choice to treat hematological diseases requiring splenectomy. The Eastern province of Saudi Arabia is known to have a high incidence of hemoglobinopathies including sickle cell disease (SCD), which is known to be associated with complications necessitating splenectomy and/or cholecystectomy. This report describes our experience with LS and/or laparoscopic cholecystectomy (LC) for children with SCD. PATIENTS AND METHODS: The medical records of all children with SCD who had …

Co-inheritance of alpha-and beta-thalassemia in Khuzestan Province, Iran.

BACKGROUND AND AIMS: beta-thalassemia is one of the most frequent hemoglobinopathies and single gene disorders in Iran. About 13 beta globin mutations encompass 70-90% of mutations spectrum in Iran, the rest are rare or unknown. People who do not produce enough alpha globin protein chains have alpha-thalassemia. This is commonly found in Africa, the Middle East, India, Southeast Asia, southern China, and occasionally the Mediterranean region. There are normally four alpha globin genes, two on each chromosome 16. Individuals who have one or two abnormal alpha globin genes have alpha-thalassemia…

Diagnostic and therapeutic ERCP in the pediatric age group.

The role and value of endoscopic retrograde cholangiopancreatography (ERCP) in the pediatric age group is not well established, because pancreatic and biliary diseases are less common in children. This however is not the case in areas like the Eastern Province of Saudi Arabia where sickle cell disease (SCD) and other hemoglobinopathies are common, with increased frequency of cholelithiasis and choledocholithiasis. The purpose of this study was to evaluate the indications, findings, safety and therapies of ERCP in children. One hundred and twenty five children had diagnostic and/or therapeutic …
