Treatment stability with the eruption guidance appliance.

INTRODUCTION: Although the eruption guidance appliance has been used to correct Class II malocclusions for many years and its effects have been demonstrated, there is no study on the stability of the changes it produces. Therefore, the objective of this study was to investigate the long-term stability of cephalometric dentoskeletal and occlusal changes after eruption guidance appliance therapy. METHODS: Thirty-nine patients were evaluated. Occlusal evaluations were made with the peer asseessment rating index, and anterior tooth irregularity was evaluated with the Little irregularity index. Mixed dentition case report.

Malocclusions can be successfully corrected by phase I mixed dentition therapy and significantly reduce the need for comprehensive orthodontic treatment in the permanent dentition. Phase I orthodontic treatment is designed to correct the skeletal and dental malocclusions at an early age with 12 to 18 months of treatment. After phase I treatment, there is a supervision phase of typically 18 to 30 months to hold the correction achieved and to supervise the eruption of the remaining permanent teeth. Good management during the supervision phase is key to maintaining the phase I result and to minim

Skeletal, dental, and functional effects of headgear-activator therapy on Class II malocclusion in Japanese: a clinical case report.

Ten orthodontic patients in whom a headgear-activator appliance was used in the first phase of treatment for Class II malocclusion were clinically evaluated. All patients had abnormal perioral muscle function at the initial examination. Myofunctional methods were used before and during active treatment. Pretreatment and posttreatment cephalometric tracings were evaluated to examine the effect of the appliance on dental, skeletal, and soft tissue structures. Five of the 10 cases illustrated distal movement of the lower first molar, a phenomenon not mentioned previously in the literature. Growth

Severe facial asymmetry and unilateral lingual crossbite treated with orthodontics and 2-jaw surgery: 5-year follow-up.

A 33-year-old woman with severe facial asymmetry and unilateral lingual crossbite was treated with orthodontics combined with differential maxillary impaction and intraoral vertical ramus osteotomy. After 12 months of preoperative orthodontic treatment, 2-jaw surgery was performed. The total active treatment time was 18 months. Both her occlusion and facial appearance were significantly improved by the surgical-orthodontic treatment. The occlusion was stable after 5 years of retention. Posttreatment records after 5 years showed excellent results with good occlusion and long-term stability.

Severe unilateral scissors-bite with a constricted mandibular arch: Bite turbos and extra-alveolar bone screws in the infrazygomatic crests and mandibular buccal shelf.

A 33-year-old woman had a chief complaint of difficulty chewing, caused by a constricted mandibular arch and a unilateral full buccal crossbite (scissors-bite or Brodie bite). She requested minimally invasive treatment but agreed to anchorage with extra-alveolar temporary anchorage devices as needed. Her facial form was convex with protrusive but competent lips. Skeletally, the maxilla was protrusive (SNA, 86°) with an ANB angle of 5°. Amounts of crowding were 5 mm in the mandibular arch and 3 mm in the maxillary arch. The mandibular midline was deviated to the left about 2 mm, which was consi

Clinical and radiographic evaluation of a computer-generated guiding device in bilateral sagittal split osteotomies.

The bilateral sagittal split osteotomy (BSSO) is one of the main orthognathic surgery procedures used for managing skeletal mandibular excess, deficiency or asymmetry. It is known to be a technique-sensitive procedure with high reported incidences of inferior alveolar nerve injury, bad splits and post-surgical relapse. With the increasing use of computer-assisted techniques in orthognathic surgery, the accurate transfer of the virtual plan to the operating room is currently a subject of research. This study evaluated the efficacy of computer-generated device at maintaining the planned condylar

Evaluation of maxillary molar distalization with the distal jet: a comparison with other contemporary methods.

Maxillary molar distalization is an increasingly popular option for the resolution of Class II malocclusions. This communication describes the effects of one particular molar distalizing appliance, the distal jet, in a sample of 20 consecutively treated and growing subjects (11 females, nine males; mean starting age of 13) and compares these effects with those of similar devices. Pre- and postdistalization cephalometric radiographs and dental models were analyzed to determine the dental and skeletal effects. The distal jet appliances were constructed using a biomechanical couple to direct the

Outcome of Surgically Treated Fractures of the Condylar Process by an Endoscopic Assisted Transoral Approach.

PURPOSE: Fractures of the condylar process are frequent. Ideal management of these fractures, as discussed in the literature, is controversial. Some recent meta-analyses have favored open reduction and internal fixation using various approaches. A strictly transoral approach has been described to minimize scarring and risk of facial nerve injury but has restricted visibility. This retrospective study analyzed outcomes of patients with unilateral mandibular condyle fractures who were treated by open reduction and internal fixation through an endoscopic-assisted transoral approach. MATERIALS AN

Agenesis of the upper lateral incisors: Study of an orthodontic population and clinical illustration.

In his daily practice, the orthodontist is regularly asked to treat patients with one or more missing teeth. Considering their functional and esthetic specificities, and the relatively high frequency of agenesia, our interest was to focus on the permanent lateral upper incisors. Our study, based on an orthodontic population including 1000 patients, shows that about 7.8% of these patients present agenesis of at least one permanent tooth (out of which 3.6% are agenesis of the upper lateral incisor). Treatment options for these cases are multiple with several decisional factors to consider: the s

Three-dimensional virtual planning in orthognathic surgery enhances the accuracy of soft tissue prediction.

INTRODUCTION: Throughout the history of computing, shortening the gap between the physical and digital world behind the screen has always been strived for. Recent advances in three-dimensional (3D) virtual surgery programs have reduced this gap significantly. Although 3D assisted surgery is now widely available for orthognathic surgery, one might still argue whether a 3D virtual planning approach is a better alternative to a conventional two-dimensional (2D) planning technique. The purpose of this study was to compare the accuracy of a traditional 2D technique and a 3D computer-aided predictio

Stem Cells from the Apical Papilla-Laden Microgels in Combination with Mandibular Advancement for Adult Condylar Cartilage Development.

Angle's Class II malocclusion, characterized by mandibular retraction, significantly impacts occlusal function and facial aesthetics. In adults, the limited development potential of the condylar cartilage poses challenges to effective mandibular remodeling. Clinically, mandibular advancement (MA) can promote the development of condylar cartilage in adolescents, but its effect on adult condylar cartilage remains controversial. Herein, MA is achieved via three methods in adult rats to investigate the effect of MA on adult condylar cartilage development. MA could stimulate the growth of adult con

Changes of neuroplasticity in cortical motor control of human masseter muscle related to orthodontic treatment.

BACKGROUND: Orthodontic treatment is a common clinical method of malocclusion. Studies have found that neurons in the sensorimotor cortex of the brain undergo adaptive remodeling in response to changes in oral behavior or occlusion. OBJECTIVE: To explore whether orthodontic treatment could be sufficient to cause neuroplastic changes in the corticomotor excitability of the masseter muscle. METHODS: Fifteen Angle Class II malocclusion patients who were receiving orthodontic treatment participated in the study. Cortical excitability was assessed by electromyographic activity changes evoked by t

The effectiveness of pendulum, K-loop, and distal jet distalization techniques in growing children and its effects on anchor unit: A comparative study.

BACKGROUND: A common strategy to correct Class II malocclusions using a nonextraction protocol in children is to move the maxillary molars distally using molar distalization appliances, which usually derive their anchorage from maxillary premolars, causing mesialization of premolars and protrusion of incisors. OBJECTIVES: To evaluate the skeletal, dental and soft tissue changes produced by three different distalizing appliances, namely, pendulum, K-loop, and distal jet appliances. MATERIALS AND METHODS: Sixty-six children of mean age 14.13 years requiring molar distalization were divided int

Patient-reported outcome measures of accelerated upper canine retraction with low-amperage electrical stimulation: A randomized controlled trial.

BACKGROUND: Interest in accelerating orthodontic tooth movement has been increasing, with a particular focus on methods that prioritize high-quality patient-reported outcomes. Among these, low-amperage electrical stimulation (LAES) emerges as a promising technique. However, the patient-reported outcome measures related to its intraoral application remain largely unexplored. OBJECTIVES: This randomized controlled trial evaluated the clinical tolerance and patient-reported outcomes of an intraoral LAES device during maxillary canine retraction. Pain perception during the early phase (A1-A3) was
