Anti-GM1 antibodies and impaired blood-nerve barrier may interfere with remyelination in multifocal motor neuropathy.

Multifocal motor neuropathy has pure motor manifestation and nonremittent clinical courses. Antiganglioside antibodies, though variable in titers, are characteristically elevated in the majority of these patient. In our cases, pathological findings at the site of conduction block suggested impaired remyelination and disruption of blood-nerve barrier. These findings lead us to postulate that antibodies toward gangliosides or toward unknown antigens containing gangliosides initiate motor-specific demyelination. The lesion, once produced, may persist as a result of impaired remyelination caused b

Experimental nerve thermal injury.

Thermal injury to rat sciatic nerve was produced by local heating (47 and 58 degrees C) and studied physiologically and morphologically. Unmyelinated nerve fibres showed a greater direct vulnerability to hyperthermia; first manifest as a reversible conduction block of C fibre action potentials and at higher temperatures by immediate and selective axonal degeneration. By contrast, lower grade nerve thermal injury resulted in delayed, selective loss of myelinated fibres. Evidence from this study suggests that this is secondary to a heat-induced angiopathy, immediately and diffusely manifest in t

High-dose intravenous immunoglobulin therapy in multifocal motor neuropathy.

We treated five consecutive patients with multifocal motor neuropathy (MMN) with high-dose intravenous immunoglobulin (IVIg). Four patients had increased levels of anti-asialo-GM1 IgM and two of anti-GM1 IgM as well; one patient had no reactivity. We treated them twice with 0.4 g/kg IVIg for 5 consecutive days at a 2-month interval, followed by maintenance infusions up to 6 to 12 months. All patients with high anti-asialo-GM1 had a consistent clinical improvement starting 3 to 10 days after the first IVIg course; in one patient, recovery was complete and persistent for 12 months without additi

AAEM minimonograph #40: clinical neurophysiology of the respiratory system.

Disorders of nerve and muscle are frequent causes of respiratory insufficiency, but current methods often fail to adequately assess the problem. Phrenic nerve conduction and needle electromyography of the diaphragm are of great assistance in identifying the nature and site of the disorder: the various disturbances of central drive, axonal or demyelinating neuropathies of the phrenic nerves, and certain myopathies. These studies may be performed on adults, children, or infants, and in outpatient, general ward, or critical care settings. It is hoped these techniques will stimulate further develo

Peroneal muscular atrophy with parkinsonism, ptosis, and congenital strabismus.

Peroneal muscular atrophy (PMA) may be occasionally associated with other neurodegenerative features including parkinsonism. We report the association of PMA of neuronal type with parkinsonism, ptosis and congenital strabismus in a 62-year-old Sicilian woman. The complete syndrome was present only in the proband, but variously combined features were present in ten other family members over four generations, with likely autosomal dominant inheritance. Although a similar syndrome of PMA, ptosis, parkinsonism and dementia was already reported, this family showed a previously undescribed combinati

Giant somatosensory evoked potentials in a patient with the anterior spinal artery syndrome.

We studied a previously healthy 25-year-old woman with the anterior spinal artery syndrome, a rare thoracocervical myelopathy with multiple potential etiologies. Quantitative and clinical sensory examination showed dissociated loss of pin-prick and temperature discrimination below the level of the lesion, with normal light touch, vibratory, and position sense. Magnetic resonance imaging was consistent with cervical spinal cord infarction. Median SEPs showed normal Erb's potential with absent spinal N13- and normal scalp N20- latency. Tibial SEPs showed normal lumbosacral responses and normal s

Sciatic nerve regeneration across gaps within collagen chambers: the influence of epidermal growth factor.

The effects of Epidermal Growth Factor (EGF) on axonal regeneration of a sectioned sciatic nerve within collagen tubes were investigated in 15 rats. Following baseline electrophysiologic assessment, bilateral 7-mm nerve gaps were created and repaired by interposition of collagen tubes, into which EGF (left side) or type I collagen (right side) was instilled. After 4 or 8 weeks, axonal regeneration, measured by electrophysiologic and histologic means, was identical for the EGF and control legs. The conclusion is that EGF does not influence nerve regeneration within a collagen chamber.

The absence of brachial plexus injury in stroke.

Brachial plexus injury and proximal mononeuropathy have been reported as a potential complications in the hemiplegic shoulder after a stroke. The diagnosis of brachial plexus injury and proximal mononeuropathy in the hemiplegic extremity is complicated by the upper motor neuron findings on physical examination and by the diffusely abnormal electrodiagnostic test results frequently seen in hemiplegic limbs. This study investigated the incidence of brachial plexus injury and proximal mononeuropathy after a thromboembolic stroke. Hemiplegic patients (n = 50) underwent physical examination, needle

Neurophysiological investigation of hands damaged by vibration: comparison with idiopathic carpal tunnel syndrome.

Forty-seven men with numbness and paresthesiae in their hands after long-term occupational exposure to vibrating hand-held tools were examined neurophysiologically and clinically. The vibration thresholds of the finger tips were assessed and fractionated neurography of the median nerve motor and sensory fibres carried out. They were compared with control groups of healthy subjects and patients with idiopathic carpal tunnel syndrome. The patients exposed to vibration had major increases in their finger vibration thresholds, shown with both the Goldberg-Lindblom vibrameter and the Lundborg vibro

Focal weakness following herpes zoster.

Three patients presented with focal weakness of an arm which followed segmental herpes zoster affecting the same limb. Neurophysiological investigations suggest that the site of the lesion lay at the root, plexus, or peripheral nerve level. This reflects the various ways in which the virus may affect the peripheral nervous system.

Case-of-the-month: perineuritis presenting as mononeuritis multiplex.

Perineuritis unassociated with other significant pathological changes is a rare finding. A patient is described with mild, non-insulin dependent diabetes mellitus who presented with the clinical picture of mononeuritis multiplex, and in whom perineuritis but no occlusive vascular disease was found on sural nerve biopsy. Treatment with prednisone and plasma exchange resulted in some improvement. We conclude that the focal perineuritis, rather than the diabetes, was responsible for the clinical picture mimicking mononeuritis multiplex. In conjunction with previous reports, this suggests that per

Acute quadriplegic myopathy: a complication of treatment with steroids, nondepolarizing blocking agents, or both.

We studied two patients who were given high-dose intravenous steroid therapy and were intubated for status asthmaticus. Both became quadriplegic and wasted within 2 weeks. EMG had myopathic abnormalities. Muscle biopsy revealed severe atrophy of most muscle fibers, with disorganization of myofibrils and selective loss of thick (myosin) filaments. Immunohistologic stains for myosin isoforms confirmed the decrease or absence of this protein. Both patients clinically improved over several months.

[Minimal-intensity carpal tunnel syndrome. Diagnostic sensitivity of various electrophysiological tests].

The most sensitive tests for the electrophysiological diagnosis of the carpal tunnel syndrome (CTS) were studied in asymptomatic hands contralateral to those objectively involved. The presence of dromic potential of "double peak" by stimulation of fingers 1 and 4, with detection of equidistant points of Median and Radial and Median and Cubital respectively, in the wrist, was the test demonstrating the earliest and most constant alterations. The detection of alterations with a frequency of subclinical character is of interest in the quantitative evaluation of CTS and in the recognition of the w

Clinical and electrophysiologic findings in chronic neuropathy of Lyme disease.

We evaluated 25 patients with Lyme disease and chronic peripheral neuropathy. All had immunologic evidence of exposure to Borrelia burgdorferi and no other identifiable cause of neuropathy. Neuropathic symptoms began a median of 8 months (range, 0 to 165) after erythema migrans and had been present for a median of 12 months (range, 2 to 168) prior to evaluation. Twelve patients (48%) had generally symmetric distal, nonpainful paresthesia, and another 12 (48%) had generally asymmetric radicular pain. One patient (4%) had asymptomatic neuropathy. The most common physical finding was multimodal s

Autosomal recessive form of hereditary motor and sensory neuropathy type I.

We studied pathologic changes in sural nerve biopsies from four patients with probable autosomal recessive (AR) hereditary motor and sensory neuropathy (HMSN) type I with a median motor nerve conduction velocity greater than 10 m/sec, comparing them with the pathologic features in autosomal dominant (AD) HMSN type I. The four recessive and two sporadic cases showed segmental demyelination. However, the classic onion bulbs of concentric Schwann cell processes, which occur in AD type I, were rare; many axons, also of a smaller size, were surrounded by onion bulbs of basal laminae. Schwann cells

Facial nerve injuries associated with orthognathic surgery: a review of incidence and management.

While facial nerve injuries associated with both extraoral and intraoral orthognathic surgery are rare, the results of such an injury can be devastating to the patient. A review of the literature shows that the majority of such injuries fall into the first-, second-, and third-degree injury categories. Prevention cannot be stressed enough; however, early recognition of an injury with prompt electrodiagnostic testing can assist with determining prognosis and treatment planning. When close observation is indicated, a variety of medical therapies have been suggested. For more severe injuries, ner

Neurophysiologic monitoring during surgery of peripheral and cranial nerves, and in selective dorsal rhizotomy.

Physiologic monitoring may warn of impending neural damage and intraoperative assessment may provide critical information used to direct the surgical procedure itself. Intraoperative techniques utilize methods modified from the diagnostic neurophysiologic laboratory. Simultaneous multimodality recording of different signal sources is of particular value for several types of procedures. Intraoperative studies must be tailored to each patient's clinical condition and to the specific surgical risks. Since studies have supported the benefits of intraoperative monitoring for peripheral nerve and cr

Proximal slowing of nerve conduction velocity in carpal tunnel syndrome.

Slowing of median nerve conduction velocity (NCV) over the forearm segment is often seen in patients with carpal tunnel syndrome (CTS). In order to evaluate the extent and significance of this retrograde slowing of NCV, we examined 132 CTS hands from 84 patients diagnosed by both clinical and electrophysiological findings and both hands of 50 normal subjects. The mean median motor NCV was relatively slow in the CTS group (55.8 +/- 4.4 m/s) as compared with normal subjects (60.0 +/- 3.4 m/s, p less than 0.001). Mean median motor NCV was slower in CTS with low median motor amplitudes (less than

Zinc deficiency and peripheral neuropathy in chicks.

Zinc-deficient chicks develop an arthritic-like neuromuscular disorder. They walk with a stilted gait and tend to remain in a squat position, bearing little weight on the legs. The purpose of this study was to determine the basis of the syndrome by making electrophysiologic measurements of nerve function. Chicks were fed low zinc (6 mg/kg) and zinc-adequate (50 mg/kg) diets, the latter ad libitum and pair-fed. At the end of 3 weeks, sciatic nerve function was determined in vivo by use of an electrodiagnostic system. Motor nerve conduction velocity was significantly lower in chicks fed the low

The effects of simple hysterectomy on vesicourethral function.

Many women who present with symptoms of bladder dysfunction relate the onset to the operation of simple hysterectomy. A group of 42 women undergoing simple hysterectomy was studied prospectively. Urinary symptoms, urodynamic findings and sacral reflex latencies (SRLs) were assessed pre- and post-operatively. After hysterectomy the incidence of urinary symptoms increased from 58.3 to 75.0%. Vesicourethral dysfunction was altered in 30.6% of patients, 72.7% of whom had evidence of pelvic neuropathy as detected by SRLs. The results show that simple hysterectomy is associated with a significant in
