Differential diagnosis of psoriasis.

Psoriasis is a common chronic skin disorder estimated to affect about 2% of the Western population. The disease creates a significant stigma for patients and is a major economic burden. Psoriasis has a large spectrum of clinical features and evolution. Clinical features of chronic psoriasis are generally sufficient to make the diagnosis. Diagnostic doubts, however, may arise in several clinical variants and atypical cases or when the psoriatic lesions are localized in particular sites.

Febrile ulceronecrotic Mucha-Habermann disease: a case report and a review of the literature.

We report a case of febrile ulceronecrotic Mucha-Habermann disease (FUMHD) in a 27-year-old woman. After 20 days of a mild eruption, extensive polymorphous, papular and ulcerohemorrhagic skin lesions gradually developed, associated with intermittent high temperature, and constitutional symptoms. The initial treatment with acyclovir was not successful, the skin lesions still progressed distally and individual lesions evolved from necrotic papules and bullae to erosions and ulcers. Skin biopsies showed the typical histopathological changes of PLEVA. The patient was treated with systemic predniso

[Papulois lymphomatoide].

Clinical study and bibliographic review of the lymphomatoid papulosis (Macaulay-1968) and parapsoriasis varioliformis acute (Mucha-1916). A personal case is reported after a review of nosological location of these diseases and the variable prognosis. The authors reach the following conclusions: 1st) From the clinical point of view the small number of lesions in the lymphomatoid papulosis bring about a different clinical picture from that of parapsoriasis varioliformis acuta in here usually there is a considerable number of lesions. 2nd) There are concomitant lesions of parapsoriasis gutata in

CARD14-Associated Papulosquamous Eruption (CAPE): An Updated Review of Pathogenesis and Treatment.

CARD14-associated papulosquamous eruption (CAPE) is a rare genetically mediated inflammatory skin disorder caused by gain-of-function variants in the CARD14 gene. It typically presents in infancy or early childhood with clinical features resembling both psoriasis and pityriasis rubra pilaris, and often exhibits a poor response to conventional therapies. An increasing number of studies support favorable outcomes with biologic medications, particularly interleukin (IL)-12/23 and IL-17 inhibitors, for the treatment of CAPE. This literature review summarizes the etiology and pathophysiology of CAP…

KLK11-Related Disorder of Cornification Presenting as Inflammatory Skin Disease: A Familial Case Report and Literature Review.

Mendelian disorders of cornification (MeDoC) are a group of heterogeneous inherited skin disorders characterized by hyperkeratosis or scaling. Clinical assessment and skin biopsy are primarily used to differentiate MeDoC from overlapping diseases, including inflammatory skin diseases, and to identify the specific type of MeDoC; however, the results can sometimes be inconclusive. We present two individuals from a family who were initially misdiagnosed with familial pityriasis rubra pilaris, however clinical exome sequencing revealed a likely pathogenic variant in KLK11, a gene recently identifi…

Rhabdomyomatous mesenchymal hamartoma presenting as telangiectasia in a 57-year-old man.

Rhabdomyomatous mesenchymal hamartoma (RMH) is a rare congenital malformation of the dermis and subcutaneous tissue. Usually, RMH occurs in the midline of the face and neck region. We described a case of RMH presenting as telangiectasia in a 57-year-old man with a history of pityriasis lichenoides chronicus. Histopathological examination revealed a subepidermal haphazard proliferation of striated muscular tissue perpendicular to the epidermis. These bundles of striated muscular tissue were admixed with adnexal structures. The diagnosis was consistent with RMH. RMH is more common in the neonata…

[Febrile ulceronecrotic Mucha-Habermann disease].

Pityriasis lichenoides et varioliformis acuta (PLEVA) is an inflammatory skin disease that is unknown to pediatricians. The ulceronecrotic febrile form is a rare and potentially lethal variant. We report the case of a 7-year-old boy with a papulovesicular eruption lasting for 4 weeks, secondarily associated with ulcers and necrotic crusts, fever, and systemic signs. After exploring infectious causes such as chickenpox, we discussed and confirmed PLEVA with histological analysis. Systemic steroids and methotrexate improved the symptoms. It is necessary to mention PLEVA in case of prolonged papu…

Ecchymotic Kaposi's sarcoma.

We report two patients with ecchymotic patches that did not suggest the diagnosis of Kaposi's sarcoma. The principal complaint of one patient was facial and periorbital edema with bilateral periorbital ecchymosis. On his trunk were patches resembling pityriasis rosea-like Kaposi's sarcoma. Both types of lesions proved to be Kaposi's sarcoma on histologic examination. The second patient had scattered ecchymotic lesions, with typical lesions of Kaposi's sarcoma elsewhere on his body. The ecchymotic lesions showed a large amount of extravasated red blood cells and no evidence of amyloid. This var…

Mycoses in India--study in Madras.

The incidence of mycoses is high in Madras accounting for over 13% of dermatoses diagnosed in a three-year period. Dermatophytoses (tinea corporis and tinea cruris) and pityriasis versicolor were most common in May and October. A correlation was observed between these infections and environmental temperature, humidity and rainfall. Most cases of all dermatophytoses except tinea capitis were recorded between 10 and 30 years of age but the latter was most common between one and 10 years. Males were predominantly affected with all except tinea axillaris, candidiasis and piedra. The incidence of p…

Epidermodysplasia verruciformis associated with HPV 10.

Epidermodysplasia verruciformis (EV) is a rare, inherited dermatologic condition demonstrating an increased susceptibility to specific HPV genotypes, resulting in both benign and malignant skin lesions. Epidermodysplasia verruciformis lesions are frequently described as pityriasis versicolor-like scaly macules, flat wart-like papules, or verrucous and seborrhic keratosis-like papules and plaques. Acquired EV has been described in patients with HIV and in those who are on immunosuppressive therapy. We discuss a patient with congenital EV who presents with skin lesions associated with HPV 10, a …

Infliximab in dermatological treatment: beyond psoriasis.

Infliximab is a chimeric monoclonal antibody of the IgG1 class. Experience from its use has been accumulated in rheumatology and gastroenterology by the treatment of rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis and inflammatory bowel disease, respectively. Because of its TNF-alpha binding capacity it has been approved for the treatment of moderate-to-severe plaque psoriasis. Moreover, it has also been evaluated in other inflammatory dermatoses as well as systemic diseases involving the skin such as severe atopic dermatitis, pityriasis rubra pilaris, pyoderma gangrenosum an…

Granulomatous rosacea-like demodicidosis.

Demodicidosis may present as pityriasis folliculitis, papulopustular lesions, rosacea-like eruptions, and granulomatous rosacea-like eruptions. We report a case of demodex granuloma presenting with recurrent granulomatous rosacea-like papules on the face in a middle-aged woman. The diagnosis of demodicidosis was made by finding extrafollicular mites in the perifollicular inflammatory infiltrate. The papules resolved after 3 weeks of systemic and topical metronidazole, and low-dose oral prednisolone therapy. In summary, demodex granuloma may be mistaken for granulomatous rosacea-like papules. C…

Tioconazole in the treatment of fungal infections of the skin. An international clinical research program.

In 32 studies involving 1,304 patients tioconazole 1% dermal cream has been shown to be effective and safe in the treatment of a wide variety of superficial fungal infections of the skin and erythrasma. Tioconazole cream is more effective than miconazole nitrate 2% cream in the treatment of pityriasis versicolor and in infections with Trichophyton rubrum and Trichophyton mentagrophytes which cause 70% of dermatophyte infections in man. Data from comparisons with econazole and clotrimazole are too few to allow conclusions to be drawn on relative efficacy. All the creams were easy to apply and t…
