Management of reproductive risks in people with epilepsy.

Epilepsy is a common neurological condition worldwide, presenting unique management challenges for those affected during reproductive age. The effectiveness of contraceptives can be modified by antiseizure medication treatments and pregnancy can alter the pharmacokinetics of antiseizure medications. Furthermore, although treatment with some antiseizure medications convey lifelong risks to offspring, inadequately controlled epilepsy can lead to injury or, in rare cases, death of the mother. In this complex set of circumstances, safe and effective antiseizure medication treatment, evidence-based

Physical activity and male reproductive function.

Fecundity is declining in humans, which is partly due to male infertility. Poor sperm parameters, the main contributors to male infertility, are associated with sedentary, unhealthy lifestyle and poor dietary habits. Long periods of sedentary work lead to visceral adiposity and persistently elevated scrotal temperatures, which adversely affect spermatogenesis. Apart from increasing scrotal temperatures, excessive visceral adiposity exacerbates adipocyte dysfunction with increased pro-inflammatory adipokine release, like leptin. These, together with the increased scrotal temperature, are respon

AI in Reproductive Medicine: Hope or Hype?

Artificial intelligence (AI) is increasingly used in reproductive endocrinology and infertility (REI), influencing nearly all aspects of assisted reproduction. Reported applications include ovarian stimulation, gamete and embryo assessment, endometrial evaluation, and prediction of IVF outcomes. These tools may reduce subjectivity, standardize grading, and provide individualized prognostic information. Predictive models for fertilization, implantation, miscarriage risk, and live birth have been developed, along with decision-support tools for embryo transfer and treatment continuation. However…

Reproductive health in the liver transplant recipient.

Rates of liver transplantation (LT) are rising in young adults, including among reproductive-aged females. While pregnancy outcomes are overall favorable, these pregnancies are at higher risk and warrant careful planning and immunosuppression management. Transplant recipients value receiving reproductive health counseling from their transplant team; thus, provider knowledge surrounding pre-conception through postpartum management is key to optimizing obstetric and graft outcomes. A critical role of transplant providers lies in reproductive counseling to ascertain whether patients are sexually

Functions of RhoA in the female reproductive system.

The Ras homolog gene family member A (RhoA) is a small GTPase. RhoA plays major roles in cytoskeletal regulation, transcriptional control, and cell cycle maintenance. RhoA is widely expressed in the female reproductive system (FRS). In vitro studies have implicated RhoA in several FRS functions and studies defining the in vivo functions of RhoA in the FRS are emerging. In the ovary, RhoA is essential for corpus luteum development and progesterone synthesis and is implicated in ovarian cancer. Some studies on the oviduct/fallopian tube suggest potential functions of RhoA in post-ovulation cumul

The roles of YY1 in reproductive system: an overview.

Yin Yang 1 (YY1) is a highly conserved multifunctional transcription factor with multiple biological functions including cell proliferation, differentiation and transcriptional regulation. In recent years, the discovery of the functional activity of YY1 in the reproductive tissues of animals, including humans has attracted more and more attention. YY1 plays important roles in the development of follicular, embryo and placenta, as well as spermatogenesis. In addition, the role of YY1 in reproductive diseases including infertility, polycystic ovarian syndrome, recurrent spontaneous abortion (RSA

Human oocyte quality and reproductive health.

Declining female fertility is a health issue that has received broad global attention. Oocyte quality is the key limiting factor of female fertility, and key processes affecting oocyte quality involve the secretion of and response to hormones, ovarian function, oogenesis, oocyte maturation, and meiosis. However, compared with other species, the research and understanding of human oocyte quality and human reproductive health is limited. This review highlights our current understanding of the physiological factors and pathological factors related to human oocyte quality and discusses potential t

Nicotine as a female reproductive toxicant-A review.

The preceding decades have seen an extensive emergence of the harmful effects of tobacco smoke on systemic health. Among the various compounds of tobacco, nicotine is one of the principal, potentially hazardous, and toxic components which is an oxidant agent that can affect both men's and women's fertility. Nicotine exerts its effect by modulating the expression of transmembrane ligand-gated ion channels called nicotinic acetylcholine receptors. The activities of female reproduction might be disrupted by exposure to nicotine at various sites, such as the ovary or reproductive tract. It's been …

Implications of the Law on Reproductive and Sexual Health.

The United States has a longstanding history of using laws to define the scope of government involvement in controlling personal matters related to sex and sexuality. Although the government serves a valuable role in protecting and promoting public health, sexual and reproductive health is unduly impacted by social stigma in ways that other fields of medicine are not. Consequently, this care is often singled out by legislation that limits rather than protects this care. Health care professionals are uniquely positioned to advocate for legal protection of the patient-provider relationship and f

The reproductive microbiome in dogs: Friend or foe?

The microbiome of the reproductive tract is an area of research in full development. Specifically, the microbiome may be involved in reproductive health, disease, and pregnancy outcomes, as has been shown in humans and animals, including dogs. The aim of the present review was to summarize current knowledge on the microbiome of the canine reproductive tract, to expose the controversial role that some bacterial agents may play in canine subfertility, and to highlight future research perspectives. This review discussed whether the use of antimicrobials in dogs is appropriate to increase reproduc

The marijuana, cannabinoids, and female reproductive system.

The marijuana is considered as widely used recreational illicit drug that has become popular among women of reproductive age. It is believed that the marijuana use may have negative impacts on the female fertility. However, the exact mechanisms of its reproductive toxicity remain unclear. The studies suggest that the exogenous cannabinoids may interfere with endocannabinoid system and disrupt hypothalamic-pituitary-ovary axis. Consequently, it impacts the female fertility by disruption of normal secretion of ovarian sex hormones and menstrual cycles. However, other studies have shown that medi

Infertility problems in the context of reproductive ageing.

The article evaluates the prevalence of infertility problems in the Czech population, identifies methods used by individuals or couples attempting to achieve pregnancy and evaluates in more detail the use of assisted reproduction technology (ART) in contemporary Czech society. The results show that 27% of women and men in their forties declare they have experienced a time when they were trying to get pregnant but did not conceive within at least 12 months. In the general population of reproductive age, one in five declares experience with methods helping to get pregnant. Methods that do not re…

Reproductive Health in RA, Lupus, and APS.

Systemic lupus erythematosus, antiphospholipid syndrome, and rheumatoid arthritis are chronic autoimmune diseases affecting women of childbearing age. These diseases may impair fertility and fecundity, as well as complicate pregnancy and the puerperium in these patients including disease flare and obstetric complications on both the maternal and fetal side. For each patient, an appropriate preconceptional counseling with risk stratification is required, including assessment of disease activity, organ involvement, serological profile, and comorbidities. In cases of pregnancy, the aims of treatme

Mevalonate pathway and male reproductive aging.

Male fertility declines with age. The mevalonate pathway, through which cholesterol and nonsteroidal isoprenoids are synthesized, plays key role in metabolic processes and is an essential pathway for cholesterol production and protein prenylation. Male reproductive aging is accompanied by dramatic changes in the metabolic microenvironment of the testis. Since the mevalonate pathway has an important role in spermatogenesis, we attempted to explore the association between male reproductive aging and the mevalonate pathway to explain the mechanism of male reproductive aging. Alterations in the me

Female reproductive tract-organ axes.

The female reproductive tract (FRT) and remote/versatile organs in the body share bidirectional communication. In this review, we discuss the framework of the "FRT-organ axes." Each axis, namely, the vagina-gut axis, uterus-gut axis, ovary-gut axis, vagina-bladder axis, vagina-oral axis, uterus-oral axis, vagina-brain axis, uterus-brain axis, and vagina-joint axis, is comprehensively discussed separately. Each axis could be involved in the pathogenesis of not only gynecological diseases but also diseases occurring apart from the FRT. Although the microbiota is clearly a key player in the FRT-o

Reproductive inequality in humans and other mammals.

To address claims of human exceptionalism, we determine where humans fit within the greater mammalian distribution of reproductive inequality. We show that humans exhibit lower reproductive skew (i.e., inequality in the number of surviving offspring) among males and smaller sex differences in reproductive skew than most other mammals, while nevertheless falling within the mammalian range. Additionally, female reproductive skew is higher in polygynous human populations than in polygynous nonhumans mammals on average. This patterning of skew can be attributed in part to the prevalence of monogam
