Establishing a central zone in scaphoid surgery: a computational approach.

PURPOSE: Scaphoid fractures are commonly fixed with headless cannulated screws positioned centrally in the scaphoid. Judgement of central placement of the screw may be difficult. We generated a central zone using computer analysis of 3D reconstructions of computed tomography (CT) images. As long as the screw axis is completely contained within this central zone, the screw would be considered as centrally placed. METHODS: Thirty cases of 3D CT reconstructions of normal scaphoids in a computerised operation planning and simulation system (Vxwork software) were obtained. The central zone was est

Scaphotrapeziotrapezoid arthrodesis and lunate excision for advanced Kienböck disease.

PURPOSE: To analyze the outcomes of lunate excision combined with scaphotrapeziotrapezoid arthrodesis for the treatment of advanced Kienböck disease. METHODS: Sixteen patients with Lichtman stage IIIB Kienböck disease were treated with scaphotrapeziotrapezoid arthrodesis and lunate excision and were followed for a mean of 67 months (range, 49 to 108 mo). The indications for lunate excision included pain and limited motion associated with a collapsed lunate. Clinical evaluation included range of motion, grip strength, and modified Mayo wrist score. Based on plain radiographs, the carpal height

A new volar vascularization technique using the superficial palmar branch of the radial artery for the collapsed scaphoid nonunion.

Achieving union to prevent scaphoid nonunion advanced collapse wrist in the scaphoid nonunion is a challenging clinical problem. Much of the difficulty relates to the tenuous blood supply to the scaphoid. One unsolved reconstructive problem is the collapsed scaphoid that requires an intercalated wedge graft with proximal pole avascular necrosis. We offer a simple technique that only requires preservation of the superficial palmar branch of the radial artery that is typically ligated during the volar approach to the scaphoid. This technique can also be used during any open volar approach to the

Comparative Computer-Aided Analysis of Three-Dimensional Carpal Alignment in Scapholunate Advanced Collapse and Healthy Wrists.

PURPOSE: Scapholunate dissociation frequently results in malalignment and scapholunate advanced collapse (SLAC). Previous analyses have relied on visual observations of carpal angles among the scaphoid, lunate, and capitate on lateral radiographs. However, the 3-dimensional carpal alignment during SLAC progression remains unclear. The purpose of this study was to analyze 3-dimensional carpal malalignment in SLAC wrists. METHODS: Using computer-aided cone-beam computed tomography analysis software based on segmentation and numerical modeling, we defined three-dimensional carpal axes and examin

Isolated fracture-dislocation of the scaphoid's proximal pole treated by scaphoid internal fixation and scapho-lunate ligament reconstruction.

We present a case of irreducible palmar dislocation of the proximal fragment of a scaphoid fracture without carpal dislocation. We observed this lesion 2 days after the injury and we immediately operated the patient with a combined volar and dorsal access: using the Henry access we reduced the fracture and we inserted a cannulated screw to synthesize the scaphoid, using the dorsal access we repaired the complete rupture of the scapho-lunate ligament using a mini anchor. Stabilization among scaphoid, lunate and capitate was performed using Kirschner wires. X-ray showed fracture healing after 90

Pure traumatic unstable radiocarpal dislocation over pre-existing ulnar impingement syndrome, extensively arched carpus and medially rotated scaphoid: An exceptional and complex case report.

INTRODUCTION AND IMPORTANCE: Radiocarpal dislocation is a rare and severe injury that demands urgent diagnosis and treatment. In this case report, we present the unique scenario of a 32-year-old male who suffered a traumatic pure unstable radiocarpal dislocation. This dislocation was associated with ulnar impingement syndrome, an extensively arched carpus, and a medially rotated scaphoid. The complexity of this injury underscores the importance of timely intervention and comprehensive management. CASE PRESENTATION: The patient had a pre-existing short ulnar head with radial-sided deformity, r

Cartilage damage in patients with scapholunate lesions: arthroscopic prevalence, location and associated clinical factors.

INTRODUCTION: Scapholunate ligament (SLL) injuries can lead to instability and osteoarthritis through stages of scapholunate dissociation and scapholunate advanced collapse. Although cartilage damage is traditionally seen as a late finding, recent evidence suggests that it may occur earlier. This study aimed to assess the prevalence and distribution of cartilage damage in patients with isolated SLL lesions confirmed by arthroscopy and to explore associated sociodemographic and clinical factors. METHODS: A retrospective cohort study was conducted in patients with arthroscopically confirmed SLL

Documented Cannabis Use Is a Risk Factor for Nonunion After Nonoperative Management of Scaphoid Fractures: A Retrospective Review of 159 998 Patients.

BACKGROUND: The purpose of the present study was to assess whether an association exists between cannabis use and nonunion among patients with acute scaphoid fractures initially treated nonoperatively. METHODS: Using the PearlDiver national data set (2010-2020), adult patients with a scaphoid fracture were identified. Patients initially treated with nonoperative management were selected, and 3 subgroups were defined: non-cannabis/non-tobacco (non-users), tobacco-only, cannabis-only. Matched cohorts for each subgroup controlled for age, sex, and comorbidities. Nonunion rates at 6 months, 12 mo

Early mobilization for suspected scaphoid injuries in children: a feasibility study.

BACKGROUND: The optimal management of children with a suspected scaphoid fracture and normal radiographs is unclear. Traditionally, children have been treated with empiric cast immobilization or with early CT or MRI to avoid missing an occult fracture. An early range of motion (ROM) protocol for these children may be an alternative strategy. METHODS: We conducted a prospective study to test the feasibility and potential benefits of an early ROM protocol for children with a clinically suspected scaphoid fracture but normal radiographs. Participants were evaluated at 6 weeks and, for those with

Four-Dimensional Computed Tomography-Derived Radiolunate Arthrokinematics With a Case Study in Four-Corner Arthrodesis.

BACKGROUND: The radiolunate joint is conventionally thought to be spared from radiocarpal osteoarthritis, but proximal lunate degenerative changes have been noted. An investigation of radiolunate arthrokinematics in ostensibly healthy wrists may improve understanding of posttraumatic and postoperative outcomes. We hypothesize that four-dimensional computed tomography (4DCT)-derived radiolunate arthrokinematics during motion will differ between wrists with type 1 and type 2 lunates. The clinical-translational relevance of this analysis is highlighted using a 4DCT case study of scaphoid excision

Assessment of scapholunate instability with dynamic computed tomography.

We performed a prospective study to evaluate the values of dynamic four-dimensional computed tomography in assessing suspected chronic scapholunate instability. Forty patients were evaluated with radiographs, arthrography, and four-dimensional computed tomography. On plain radiographs and computed tomography, we found 16 patients with definite scapholunate instability, five with questionable scapholunate instability, and 19 with absence of scapholunate instability. We used four-dimensional computed tomography to evaluate the size of the scapholunate gap during radioulnar deviation. The mean an

Scaphoid waist fractures fixation with staple. Retrospective study of a not widespread procedure.

INTRODUCTION: In carpal scaphoid fractures, the surgical treatment with screw is considered the gold standard; shape memory staple however presents substantial advantages. The authors report a study on unstable fractures of the scaphoid waist (type B1, B2, B5, according to Herbert classification) treated with shape memory staple on a large sample of patients, with the aim to confirm the usefulness of this method, the quality of reduction and fixation, the functional results, the time of union and the possible complications. MATERIALS AND METHODS: A retrospective analysis of 131 patients with

A Cadaveric Study on the Accuracy of an Individualized Guiding Template to Assist Scaphoid Fixation Using Computed Tomography and 3-Dimensional Printing.

PURPOSE: The aim of this study was to evaluate the feasibility and accuracy of scaphoid screw guidewire placement using a computer-assisted-designed, and 3-dimensional-printed, surgical guiding template in cadaver wrists. METHODS: Computed tomography (CT) scans of 12 fresh-frozen cadaver wrists were performed and the data imported into a surgical planning system. A 3-dimensional skin surface template block with a guiding hole was generated from the CT data to allow a screw guidewire to be placed in the central third of the scaphoid. This 3-dimensional model was printed and then put back onto

Prediction of outcome of non-operative treatment of acute scaphoid waist fracture.

Fifteen per cent of acute fractures of the scaphoid waist fail to unite if treated non-operatively in plaster, resulting in persistent loss of function. Suspected risk factors for non-union include proximal fracture fragment avascularity and assessments of fracture displacement and comminution. This series of studies investigated whether one can accurately identify which scaphoid waist fractures will unite with plaster treatment. They suggest that proximal fracture fragment vascularity is not a predictor of outcome. In contrast, assessments of fracture displacement on magnetic resonance imagin

A quantitative definition of scaphoid union: determining the inter-rater reliability of two techniques.

BACKGROUND: Despite extensive literature supporting the use of computerized tomography (CT) scans in evaluating scaphoid fractures, there has not been a consensus on the methodology for defining and quantifying union. The purpose of this study was to test the inter-observer reliability of two methods of quantifying scaphoid union. METHODS: The CT scans of 50 non-operatively treated scaphoid fractures were reviewed by four blinded observers. Each was asked to classify union into one of three categories, united, partially united, or tenuously united, based on their general impression. Each revi

Biomechanical analysis of second-generation headless compression screws.

INTRODUCTION: Headless Compression Screws (HCS) are commonly utilized for the fixation of small bone and articular fractures. Recently several new second generation HCS (SG-HCS) have been introduced with the purported benefits of improved biomechanical characteristics. We sought to determine and compare the biomechanical efficiencies of these screws. MATERIAL AND METHODS: Five HCS including four second generation (Mini-Acutrak 2 (Acumed), Twinfix (Stryker), Kompressor Mini (Integra), HCS 3.0 (Synthes)) and one first generation (Herbert-Whipple) were studied. Polyurethane foam blocks that repr
