Management of severely bleeding ethmoidal arteries.

OBJECTIVES: Ethmoidal arteries (EAs) can be responsible for severe bleeding. Clinical features of EA bleeding are rather extensive because it can occur within the nasal cavity or in orbital spaces. Furthermore, surgical management of EA bleeding is challenging. STUDY DESIGN: Retrospective evaluation. METHOD OF STUDY: Five clinical patients with severe bleeding from EAs and/or epistaxis refractory to sphenopalatine artery closure were included in this work. Careful anatomic dissection of the orbitoethmoidal region was performed in 3 fresh injected heads. RESULTS: Ethmoidal artery management

"Twist technique" for pterygomaxillary dysjunction in minimally invasive Le Fort I osteotomy.

PURPOSE: To present a new technique for effective, rapid, and safe pterygomaxillary dysjunction in the context of a minimally invasive Le Fort I protocol and to provide the authors' preliminary experience. MATERIALS AND METHODS: In total, 1,297 consecutive patients underwent Le Fort I osteotomy as an isolated procedure or in combination with mandibular surgery. In all cases, the "twist technique" was used to downfracture the maxilla. This method achieves pterygomaxillary dysjunction using a frontal approach and a straight osteotome that is driven along the standard Le Fort I horizontal osteot

Base-of-skull/infratemporal fossa shell fragment retrieval.

PURPOSE: Shell fragments and bullets are frequently observed to have settled in the skull base and infratemporal fossa (ITF) with the impact resulting in a new surgically complicated trauma. This compact region is one of the most difficult and dangerous locations for lodged objects. When called upon to retrieve these objects, the surgeon faces a complicated challenge. The complexity of the task depends on many factors, such as size, deformation, and surface hooks, as well as identification of the precise anatomic location. MATERIALS AND METHODS: New surgical approaches and successful methods

LeFort III advancement with and without osteogenesis distraction.

The LeFort III osteotomy is the surgical treatment performed in patients with mid-facial retrusions in craniofacial dysostoses such as Crouzon, Apert, Pfeiffer syndromes, etc. The first authors to report the accomplishment of this osteotomy were Gillies and Harrison in 1951, this technique was then resumed and improved by Tessier in 1967, who made five different variants mainly regarding the typology of the osteotomy concerning the lateral wall of the orbit. Recently, distraction techniques have been applied to the upper mid-face which foresees a gradual advancement of the mid-face through the

The evaluation of surgical factors related to recovery period of upper lip hypoaesthesia after Le Fort I osteotomy.

PURPOSE: It is unclear whether surgical factors can affect the upper lip sensitivity. The aim of this study was to assess the factors that can affect the recovery period of hypoaesthesia of the upper lip after Le Fort I osteotomy, using trigeminal somatosensory evoked potential (TSEP) objectively. PATIENTS AND METHODS: Twenty-nine patients with mandibular prognathism underwent Le Fort I osteotomy with and without artificial pterygoid plate fracture. Trigeminal nerve hypoaesthesia at the region of the upper lip was assessed bilaterally by the TSEP method. The distance between the infraorbital

Skull-base trauma: neurosurgical perspective.

Trauma to the cranial base can complicate craniofacial injuries and lead to significant neurological morbidity, related to brain and/or cranial nerve injury. The optimal management involves a multidisciplinary effort. This article provides the neurosurgeon's perspective in management of such trauma using a 5-year retrospective analysis of patients sustaining skull-base trauma. The salient features of anterior and middle skull-base (temporal bone) trauma are summarized, and the importance of frontal basilar trauma as well as brain injury is evident. With these injuries, all cranial nerves (exce

Single-step 3D printing aided cranio-orbital reconstruction with patient specific polyetheretherketone implants after resection of benign spheno-orbital tumors.

PURPOSE: Computer-aided design (CAD) and computer-aided manufacturing (CAM) techniques have paved the way for single-step resections and cranio-orbital reconstructions with patient specific implants in spheno-orbital tumors. Here, we present our interdisciplinary maxillofacial and neurosurgical workflow and a case series of patients treated with this integrated approach. METHODS: Patients, who underwent single-step resection of benign spheno-orbital tumors and cranio-orbital reconstruction with polyetheretherketone (PEEK) patient specific implants (PSI) from 2019 to 2024 in our institution we

A follow-up study on outcomes of endoscopic transsphenoidal approach for acromegaly.

OBJECTIVE: A thorough follow-up study in which the same clinic presents the change in the surgical outcomes of acromegaly over the years, is still lacking in the endoscopic era. In this study, we intended to evaluate the clinical characteristics, radiological features, surgical and late remission rates of newly diagnosed acromegaly patients treated in our clinic between 2014 and 2019 in order to delineate the surgical remission status according to radiological, microscopic, and hormonal features. As a follow-up to our initial report, we also aimed to display the change of surgical remission ra

Female gender and exogenous progesterone exposition as risk factors for spheno-orbital meningiomas.

OBJECTIVE: The great heterogeneity of meningiomas is challenging and we need to distinguish relevant subgroups. Spheno-orbital osteomeningiomas (SOOM) constitute a clinically specific entity, with slow-growing benign osteo-meningiomatous tumors, which recur after surgery in one fourth of cases. Neurosurgical daily practice, supported by the literature, shows that the vast majority of patients with SOOM are women, and we explored whether their epidemiological and hormonal profiles suggest a progesterone influence. METHODS: We retrospectively documented all radiologically and histologically con

Pediatric pituitary adenomas.

BACKGROUND: Pediatric pituitary adenomas are a rare medical entity that makes up a small portion of intracranial tumors in children and adolescents. Although benign, the majority of these lesions are secreting functional tumors with the potential for physiological sequela that can profoundly affect a child's development. FOCUS OF REVIEW: In this review, we discuss the medical and surgical management of these tumors with a focus on clinical presentation, diagnostic identification, surgical approach, and associated adjuvant therapies. We will also discuss our current treatment paradigm using en

Olfactory outcomes after endoscopic pituitary surgery with nasoseptal "rescue" flaps: electrocautery versus cold knife.

BACKGROUND: Olfaction has been shown to have a large impact on patients' lives. Endoscopic pituitary surgery is associated with potentially significant damage to olfactory tissues. The objective of this study was to determine the effect of cauterization on olfactory function when performing endoscopic pituitary surgery with a nasoseptal "rescue" flap. METHODS: A retrospective review was performed of prospectively collected data. Olfaction was the primary outcome and was measured using the subjective visual analog scale (VAS; 0-100) and Cross-Cultural Smell Identification Test (CC-SIT) before

The awake endoscope-guided sealant technique with fibrin glue in the treatment of postoperative cerebrospinal fluid leak after extended transsphenoidal surgery: technical note.

BACKGROUND: The introduction of extended endoscopic endonasal approaches for the management of midline skull base lesions has brought again the focus on the problem of postoperative cerebrospinal fluid (CSF) leak management. Notwithstanding the improvements in reconstruction techniques that have reduced the rate of postoperative CSF leakage, no technique has proven to be thoroughly effective. METHODS: Nine patients complaining of postoperative CSF leaking after extended endoscopic endonasal surgery for different suprasellar lesions were managed without reoperation by means of repeated endosco

The Dorello canal: historical development, controversies in microsurgical anatomy, and clinical implications.

Interest in studying the anatomy of the abducent nerve arose from early clinical experience with abducent palsy seen in middle ear infection. Primo Dorello, an Italian anatomist working in Rome in the early 1900s, studied the anatomy of the petroclival region to formulate his own explanation of this pathological entity. His work led to his being credited with the discovery of the canal that bears his name, although this structure had been described 50 years previously by Wenzel Leopold Gruber. Renewed interest in the anatomy of this region arose due to advances in surgical approaches to tumors

Preoperative evaluation of the maxillary sinus roof as a guide for posterior ethmoid and sphenoid sinus surgery.

BACKGROUND: The maxillary sinus roof has long been regarded as a reliable reference point for both endoscopic and skull base surgery. OBJECTIVE: This study was designed to evaluate the maxillary sinus roof as a preoperative guide for posterior ethmoid and sphenoid sinus surgery. METHOD: This was a cross-sectional observational study of 50 randomly selected computed tomographic (CT) sinus scans from patients previously seen at a tertiary rhinology clinic. Each side of the sinuses was treated independently, giving a total of 100 sides for analysis. Using the medial maxillary sinus roof as a fi
