Structured care to improve outcomes in primary single stage laryngotracheal reconstruction.

PURPOSE: To examine single stage laryngotracheal reconstruction (SSLTR) care to reduce complication and failure rate. METHODS: Forty-five patients that underwent primary SSLTR were examined retrospectively. All had pre-operative direct laryngoscopy and bronchoscopy, esophagoscopy with biopsy and MRSA screening. Pre-operative subglottic stenosis (SGS) grade and associated comorbidities were recorded. Intraoperative graft location and type was documented. Hospital course and results were evaluated and compared to cited literature. RESULTS: The median age at reconstruction was 2 years (0-15 yea

Vocal Fold Paralysis after Esophagectomy for Carcinoma.

OBJECTIVES: (1) To recognize factors that contribute to vocal fold paralysis (VFP) after esophagectomy. (2) To describe the morbidity associ … ecreased VFP (P = .0223). Patients with VFP had longer lengths of stay (P = .0078), higher rates of tracheotomy (P = .0439), and required more outpatient swallow evaluations (P = .0017). Mean time to diagnosis of VFP was 45.6 days (median, 7.5 days). CONCLUSIONS: Cervical approaches are associated with increased VFP in patients undergoing esophagectomy for m …

A useful algorithm for managing the difficult pediatric airway.

Management of the pediatric airway can be extremely challenging. A useful algorithm is presented that can be used when faced with even the most difficult of airways. A new adjuvant airway tool is also presented, as well as a list of equipment for the operating room. We will attempt to emphasize key points while presenting a patient that we have recently treated.

Percutaneous transtracheal emergency ventilation during respiratory arrest: comparison of the oxygen flow modulator with a hand-triggered emergency jet injector in an animal model.

The oxygen flow modulator is a device for percutaneous transtracheal emergency ventilation. Simulating a respiratory arrest situation, we studied the effects of this device in comparison with a hand-triggered emergency jet injector during pulmonary resuscitation. Nine pigs were anesthetized and mechanically ventilated. After surgical exposure, an emergency transtracheal airway catheter was inserted into the trachea. Ventilation was stopped until SpO2 was below 70%. Each animal was subsequently randomly ventilated via the transtracheal airway catheter with either the hand-triggered emergency je

Laser posterior ventriculocordectomy with partial arytenoidectomy for the treatment of bilateral vocal fold immobility.

Bilateral vocal fold immobility in adduction usually creates severe dyspnoea. Many surgical procedures have been established to improve the airway insufficiency in affected patients. Over the last six years 22 patients with bilateral vocal fold immobility in our department have undergone CO(2) laser posterior ventriculocordectomy with partial arytenoidectomy (PVCPA). None had dyspnoea or a disturbance in the immediate post-operative period or during follow up, which ranged from five months to six years. Only one patient required a second procedure. Laser PVCPA appears to be an effective and re

[Home ventilation after intensive care].

In the course of chronic respiratory insufficiency, acute episodes often require the use of mechanical ventilation. Failure of weaning, or worsening of alveolar hypoventilation, results in long term ventilatory assistance with the need of overall care of the patient. The pneumologist has a key role in the choice of indications, devices and mode of home mechanical ventilation. Thanks to the non-invasive ventilation with a facial or nasal mask, tracheostomy is less often needed. Respiratory failure due to lung restriction is the best indication of mechanical ventilation. The results in COPD are

Laryngeal scleroma.

Respiratory scleroma is a chronic, progressive granulomatous disease of the respiratory tract. The causal organism is the Klebsiella rhinoscleromatis. The disease has three stages: the initial catarrhal stage, granulomatous stage, and sclerotic stage. The object of this report was to determine the clinical behaviour of the laryngeal scleroma. The study included 17 patients with this manifestation characterized by airway obstruction and dysphonia. The report revealed that respiratory scleroma affected the larynx in 40%, and the principal findings were glottic/subglottic stenosis.

Changes in breathing when switching from nares to tracheostomy breathing in awake ponies.

We assessed the consequences of respiratory unloading associated with tracheostomy breathing (TBr). Three normal and three carotid body-denervated (CBD) ponies were prepared with chronic tracheostomies that at rest reduced physiological dead space (VD) from 483 +/- 60 to 255 +/- 30 ml and lung resistance from 1.5 +/- 0.14 to 0.5 +/- 0.07 cmH2O . l-1 . s. At rest and during steady-state mild-to-heavy exercise arterial PCO2 (PaCO2) was approximately 1 Torr higher during nares breathing (NBr) than during TBr. Pulmonary ventilation and tidal volume (VT) were greater and alveolar ventilation was le

Late cataclysmic hemorrhage from the innominate artery after tracheostomy.

We report on 12 patients sustaining late cataclysmic hemorrhage as a complication of tracheostomy or endotracheal intubation, resulting from erosion of the innominate artery. Four long-term survivors are reported. A number of surgical techniques was used and up-to-date management is briefly described. Our technique mainly consists of transecting and suturing the innominate artery without prior debridement. Innominate artery blood flow was not reestablished in all patients, thereby avoiding local wound infection and recurrence of hemorrhage. Initial temporary control of bleeding was achieved ei

Tracheostomy and its complications. A retrospective study of 794 tracheostomized patients.

The records of 794 tracheostomized patients were studied in order to discover complication associated with tracheostomy. Bacterial colonization was found in 92% of patients from whom tracheal secretions were cultured, crusted airways in 5%, tracheobronchitis is 16%, pneumonia in 22% and tracheobronchial bleeding in 22% of patients in the form of bloody secretions. Tracheo-arterial erosion with massive bleeding was confirmed in five patients and treatment was successful in one case only. Tracheo-oesophageal fistula was diagnosed in three patients at autopsy. After extubation, symptomatic trache

[Acute epiglottitis in adults. Three cases and review of the literature (author's transl)].

Acute epiglottitis is much less frequent in adults than in children. Three personal cases are reported and clinical findings associated with epiglottitis in adults are reviewed; sudden onset of acute respiratory failure is outlined. Mechanical desobstruction of airway may be required promptly, tracheostomy being often preferred to tracheal intubation. Overall prognosis has been evaluated from 130 cases reported in the literature in the last twenty years: the mortality rate, reaching 24,6 per cent in 73 cases published between 1958 and 1973, has been reduced to 5 per cent in 60 most recent case

5. New approaches to the management of flail chest.

Although continuous positive-pressure ventilation (internal pneumatic fixation) was a great advance in the treatment of flail chest and is now the standard treatment of this condition, early and late complications related to tracheostomy and long-term ventilation are associated with this method. These complications can be avoided by use of three recently adopted techniques--expectant therapy, intermittent mandatory ventilation with positive end-expiratory pressure, and early surgical stabilization of fractures. All patients should continue to be treated in intensive care units so that impendin

Hypersomnia with periodic apneas in acquired micrognathia. A bird-like face syndrome.

We describe a syndrome characterized by acquired micrognathia, hypersomnia, and periodic apneas during sleep. Six patients affected with the syndrome underwent nocturnal and diurnal polygraphic recordings that demonstrated that during sleep there is an uninterrupted succession of apneas, primarily the obstructive type, analogous to those observed in Pickwickian syndrome. Simultaneous recording of pulmonary and systemic arterial pressure during sleep and repeated blood gas analyses have shown that as soon as the apneas appear there is a decisive increase in pulmonary and systemic pressure and s

Stenosis following tracheostomy. A quantitative study of long term results.

Seventy out of the 320 patients treated with tracheostomy and respiratory care in an intensive care unit, were included in a follow-up study. A variety of surgical (38) and medical (32) conditions had prompted IPPV for 1-59 days; 33 had had primary and 37 secondary tracheostomies. The final study included an interview, physical examination, radiographic examination and spirometry. Using radiographic measures, the area of stenosis was calculated as well as the pressure drop across the stenosis at various flow rates. The methods of calculation were tested in one patient and compared with the act

Acquired and congenital subglottic stenosis in the infant.

During the calendar year of 1974, the Intermountain Newborn Intensive Care Center at the University of Utah Medical Center had 603 admissions. A representative group of 293 charts were reviewed which indicated that 44% of these children were intubated from hours to weeks. The overall mortality rate for the 293 children was 29%. Eighteen of the 603 children were diagnosed as having subglottic stenosis. Fifteen of these children appeared to have acquired subglottic stenosis secondary to endotracheal intubation. Three children had congenital subglottic stenosis. Tracheostomy was necessary in the
