The predictive efficacy of the Müller maneuver in uvulopalatopharyngoplasty.

Flexible pharyngoscopy with the Müller maneuver has been proposed as a method for selection of uvulopalatopharyngoplasty candidates. In this prospective study, its predictive efficacy has been evaluated in 24 consecutive obstructive sleep apnea patients undergoing uvulopalatopharyngoplasty. FPMM enabled us to accurately predict the uvulopalatopharyngoplasty outcome in 12 of the 24 patients (50%). Although flexible pharyngoscopy with the Müller maneuver appears to be helpful in the selection of candidates for UPP, its distinct predictive efficacy is low.

Uvulopalatopharyngoplasty reduces the incidence of cardiovascular complications caused by obstructive sleep apnea: results from the national insurance service survey 2007-2014.

OBJECTIVE: Untreated obstructive sleep apnea (OSA) is a risk factor for cardiovascular disease including myocardial infarction (MI), congestive heart failure (CHF), and atrial fibrillation (AF). Continuous positive airway pressure (CPAP) is an effective treatment for OSA; however, compliance with CPAP can be challenging for some patients. The objective of this study was to investigate whether uvulopalatopharyngoplasty (UPPP) reduced the risk of cardiovascular complications for patients with OSA. METHODS: Data from Korea National Health Insurance Corporation, a national health care database in

SKUP3 : 6 and 24 months follow-up of changes in respiration and sleepiness after modified UPPP.

OBJECTIVE: Our previous randomized controlled trial of patients with obstructive sleep apnea syndrome (OSAS) showed that modified uvulopalatopharyngoplasty (UPPP), including tonsillectomy, significantly improved nocturnal respiration, daytime sleepiness, and quality of life in the intervention group compared to controls who had delayed surgery after 6 months. This is the continuous report with the 6- and 24-month postoperative results. STUDY DESIGN: Single-center prospective cohort study. METHODS: Sixty-five patients with apnea-hypopnea index (AHI) ≥ 15, body mass index (BMI) < 36, Epworth S

Depth-dependent changes of obstruction patterns under increasing sedation during drug-induced sedation endoscopy: results of a German monocentric clinical trial.

PURPOSE: Drug-induced sedation endoscopy (DISE) and simulated snoring (SimS) can locate the site of obstruction in patients with sleep-disordered breathing (SDB). There is clinical evidence for a change in collapsibility of the upper airway depending on the depth of sedation. So far, a dose-response relationship between sedation and collapsibility has not been demonstrated. METHODS: DISE and SimS were performed in 60 consecutive patients with SDB under monitoring of depth of sedation by BiSpectral Index® (BIS). Initially, SimS was conducted followed by DISE using bolus application of propofol

Patients' and partners' perceptions of uvulopalatopharyngoplasty for snoring.

OBJECTIVE: Uvulopalatopharyngoplasty (UPPP) has become a routine operation for snoring. Many centres have published their results. Few, if any, have questioned their patient's subjective feelings regarding the operation and more importantly, the patient's partner. METHOD: The study involved postal questionnaires and a retrospective case note review. Between the years 1988 and 1995, 125 patients underwent a UPPP for snoring at Derriford Hospital, Plymouth, England, a large district general hospital. Postal questionnaires were sent to all patients and their partners, asking them about their exp

Surgical treatment of obstructive sleep apnea in neurologically compromised patients.

Children with cerebral palsy are at risk of developing obstructive sleep apnea, which is initially managed by medical therapy but often requires tracheostomy for stabilization of the airway. We report preoperative and postoperative polysomnographic findings in a prospective series of 18 patients with cerebral palsy and obstructive sleep apnea who were refractory to medical management and underwent aggressive surgical treatment of upper airway obstruction. Fifteen of the 18 children (83 percent) in whom tracheostomy was recommended were spared the procedure. Eighteen children with cerebral pals

Uvulopalatopharyngoplasty for the obstructive sleep apnoea syndrome: value of polysomnography, Mueller manoeuvre and cephalometry in predicting surgical outcome.

Sixty consecutive patients with the obstructive sleep apnoea syndrome (53 men and seven women) were analysed by questionnaire, polysomnography, röntgenographic cephalometry and the Mueller manoeuvre before and 6 months after uvulopalatopharyngoplasty (UPPP), to assess the surgical outcome and the prognostic value of preoperative evaluation. Seventy-three per cent of patients reported improvement of snoring and 55% reported improvement of excessive daytime sleepiness. Thirty-five per cent showed a decrease of at least 50% in the desaturation index, and 13% had a postoperative desaturation index

Laser-uvulopalatoplasty (LUPP) under local anesthesia: effective, safe and comfortable.

Conventional uvulopalatopharyngoplasty has in the last years to an increasing extent been succeeded by a variety of laser procedures for snorers obstructed by lax palates only. These surgical techniques have the advantages of being less traumatic and therefore more suitable for local anesthesia and outpatient surgery. However, to the authors' knowledge, there are no studies on degree of patient discomfort during this type of surgery as well as the value of anticholinergic component in premedication in preventing bradycardia and hypersalivation during the operation. We studied 53 consecutive pa

Obstructive sleep apnea syndrome: a surgical protocol for dynamic upper airway reconstruction.

A surgical protocol for dynamic upper airway reconstruction in the treatment of obstructive sleep apnea syndrome is presented. Two hundred thirty-nine consecutively treated patients were evaluated. All patients underwent a presurgical evaluation that included a physical examination, fiberoptic pharyngoscopy, cephalometric analysis, and polygraphic monitoring. The goal of the presurgical evaluation was to document sleep apnea and isolate the area of obstruction. The treatment was then directed to the obstructive site. The surgical protocol included two phases. Phase 1 was a conservative approac

Redefining the timing of surgery for obstructive sleep apnea in anatomically favorable patients.

OBJECTIVES/HYPOTHESIS: Healthcare remunerating agencies in North America require patients with obstructive sleep apnea (OSA) to undergo a continuous positive airway pressure (CPAP) trial before funding surgical therapy. The adherence rate of CPAP is problematic. This study's objective was to determine the proportion of surgically favorable patients who failed CPAP who subsequently benefitted from surgical therapy, and to explore consideration of surgical therapy as first-line treatment in this specific OSA subpopulation. STUDY DESIGN: This was a prospective cohort study. METHODS: Patients wi

Timely recognition of pemphigus vulgaris by dental professionals.

Pemphigus vulgaris is a potentially fatal mucocutaneous, vesiculobullous disorder of autoimmune etiology. Regrettably, affected patients frequently experience considerable discomfort and diagnostic delay for months in spite of multiple clinician contacts. The high likelihood of disease manifestations in the oral cavity and serious nature of potential oral and systemic complications mandates dental professionals to recognize early oral signs and symptoms of pemphigus vulgaris and contribute to timely diagnosis and medical intervention to prevent disease progression. This case report presents a

Imaging findings of chronic subluxation of the os odontoideum and cervical myelopathy in a child with Beare-Stevenson cutis gyrata syndrome after surgery to the head and neck.

INTRODUCTION: Although uncommon, fractures of the os odontoideum are known to occur in children under 7 years old, following acute trauma. CLINICAL PICTURE: We report a case of chronic subluxation of the os odontoideum resulting in cervical myelopathy in a child with Beare-Stevenson cutis gyrata syndrome after surgery to the head and neck. TREATMENT AND OUTCOME: The patient was initially put in a Halo vest, following which occipital cervical fusion was performed. CONCLUSION: Subluxations and fractures at the odontoid synchondrosis are rare but should be anticipated in young children with ri

A cephalometric evaluation of the pharyngeal airway space in patients with mandibular retrognathia and prognathia, and normal subjects.

The antero-posterior diameter of the pharyngeal airway space (PAS) at the level of the soft palate and base of the tongue was assessed in age-matched females with a normal mandible (n=31), mandibular retrognathism (n=30) or mandibular prognathism (n=38). All subjects were examined by lateral cephalometry. Measured variables were corrected with the use of appropriate regression equations to eliminate the effects of head posture on the PAS. The corrected data showed more clear-cut differences in the PAS among the three groups than did the measured data. Pharyngeal airway diameter was largest in
