[Risk factors study of ischemic stroke in young adults in Southwest China].

OBJECTIVE: To analyze the etiology and risk factors of ischemic stroke in young adults in Southwest of China. METHODS: This study retrospectively reviewed the 109 records of all 18-45 year-old patients who were admitted to the West China Hospital of Sichuan University from 2008 to 2010. Etiology was classified by Trial of Org 10172 in Acute Stroke Treatment (TOAST) criteria. We surveyed the risk factors of all the patients. RESULTS: Of 109 patients (male female 1.7 : 1), the mean age of onset was (37.9 +/- 6.3) years. An increased occurrence of ischemic stroke was found in the older age grou

Clinical characteristics of vertebrobasilar artery dissection.

Sixteen patients with the diagnosis of vertebral or basilar artery dissection who were admitted at the Seoul National University Hospital from 1972 to 1996 are described. During the same period, we encountered 76 patients with posterior circulation aneurysms, so the vertebrobasilar artery dissection was 21% of posterior circulation aneurysms. The mean age was 44 years, and male predominated. Nine patients presented with subarachnoid hemorrhage (SAH) and seven with ischemic symptoms. The characteristic angiographic finding of patients with SAH was aneurysmal dilatation (pseudoaneurysm) in eight

Retrosigmoid Craniotomy for the Removal of Left Sided Tentorial and Posterior Fossa Meningioma Combined with Microvascular Decompression for Hemifacial Spasm.

This 68-year-old woman presented with repeated episodes of bilateral hemifacial spasm with headache for 5 years and with recent progression of left sided symptoms. Preoperative imaging showed a left sided tentorial meningioma with brain stem and cerebellar compression. Left facial nerve was compressed by the vertebral artery (VA) and the right facial nerve by the anterior inferior cerebellar artery (AICA). This patient underwent left side retrosigmoid craniotomy and mastoidectomy. The cisterna magna was drained to relax the brain. The tumor was very firm, attached to the tentorium and had medi

[Strokes associated with cervical artery dissection, and systemic mastocytosis: an unfortuitous association? A report of two cases].

INTRODUCTION: Neuropsychiatric symptoms in systemic mastocytosis are usually cognitive and affective changes. EXEGESIS: We describe here two systemic mastocytosis patients without eosinophilia presenting strokes associated with cervical artery dissection. CONCLUSION: These observations are the first reported and they suggest that systemic mastocytosis could be add to the predisposing factors of spontaneous cervical artery dissections.

Dissection of epiaortic vessels: clinical appearance and potentiality of imaging techniques.

PURPOSE: To analyse the major radiological and clinical features of dissections of the epiaortic vessels and evaluate the potential of imaging techniques on the basis of the findings reported in the literature over the last few years. MATERIALS AND METHODS: We evaluated 9 dissections (7 carotid lesions and 2 vertebral lesions) in 9 patients (5 women and 4 men; age range: 30-56 years) who came to our attention between September 1999 and June 2002. These were all stenoses and obstructions located in the cervical region and, in two patients, extended intracranially. The morphologic features were

[Progress of problems related to the reconstruction of aortic arch in acute type A aortic dissection].

Aortic arch reconstruction is one of the most challenging operations in aortic surgery. Anatomical abnormalities of the aortic arch, including bovine aortic arch, aberrant right subclavian artery, and single vertebral artery have direct impact on the choice of surgical procedures and methods of brain protection. The risk of aortic arch reconstruction and the difficulty of operation, it is possible to avoid the injury of patients by strictly grasping the indication of operation. Intraoperative arterial cannulation and brain protection strategies are directly related to the success of the operati

Centennial dissertation. Honoring Arthur W. Goodspeed, MD and James B. Bullitt, MD. CT and MR imaging and nontraumatic neurologic emergencies.

This review has highlighted some of the disease processes that produce diagnostic difficulty in the emergency neuroradiology setting. Because radiologists are often the first individuals to consider these entities, they must be familiar with the clinical features that suggest the diagnosis. Furthermore, acquaintance with the various imaging findings of these diseases will allow early diagnosis and will help limit the severe complications that follow these neurologic emergency conditions if left untreated.

Beyond the Bifurcation: There Is More to Cerebrovascular Ultrasound Than Internal Carotid Artery Stenosis!

Carotid ultrasonography (US) is most commonly performed to identify potential areas of flow-limiting stenosis in the internal carotid artery. Identification of stenosis and unstable plaque is important because these entities are leading predictors of stroke, the fourth most common cause of death in the United States. However, US can detect other important but less common vascular pathologies if proper techniques and nuances of these entities are understood. In this article, we discuss the US appearance of abnormalities involving the carotid, vertebral, subclavian, and innominate arteries as wel

[Life-threatening differential diagnoses of vertigo and sudden hearing loss].

Many patients call the ENT emergency department because of vertigo and sudden hearing loss. The majority of cases are due to peripheral or neurootological reasons. A serious and ongoing problem is that life-threatening ischemic, hemorrhagic and inflammatory diseases of the central nervous system may cause identical symptoms, making it difficult to differentiate between them. On the basis of our own patients with cerebellar ischemia, basilar thrombosis, dissection of the vertebral artery, cerebellar abscess, brain tumor and cholesteatoma and on the basis of expert opinions, typical sets of symp

Clinical implications of concomitant variations of the testicular, suprarenal and renal veins: a case report.

Multiple venous anomalies have been observed during dissection of the posterior abdominal wall in a 65-year-old, white male cadaver. The left testicular and suprarenal veins united inferior to the superior mesenteric artery, coursed anterior to the abdominal aorta and drained into the inferior vena cava (IVC). Further the left renal vein coursed retroaortically and divided into three branches. The superior branch coursed on the vertebral column and drained into the azygos vein while middle and inferior branches drained into the IVC. The right renal vein was double and both drained into the IVC

Stent placement for vertebral artery occlusive disease: preliminary clinical experience.

Symptoms of vertebrobasilar insufficiency may precede neurological sequelae in up to 50% of patients. Although select patients may benefit from microsurgical revascularization, combined perioperative morbidity and mortality rates can be as high as 20%. The authors present their preliminary clinical experience using stent placement for symptomatic vertebral artery (VA) occlusive disease. Six patients with clinical symptoms of vertebrobasilar insufficiency in whom VA stents were placed from 1995 to 1998 were identified. Diagnostic four-vessel cerebral angiography identified causative stenotic, a

Lateral medullary syndrome in a boy with hereditary dysfibrinogenemia.

A 9-year-old boy presented with sudden onset of nausea, vomiting and unsteady gait after a bread-eating game, which possibly caused neck hyperextension. Neurological examination revealed hemisensory loss of pain and temperature sensation in the right trunk and limbs along with left Horner's syndrome, suggesting lateral medullary syndrome (LMS). Magnetic resonance (MR) imaging of the brain revealed infarction at the left lateral medulla. MR angiography showed no sign of arterial dissection and no occlusion or stenosis of the intracranial, basilar or vertebral arteries or their branches. No evid

Minimally invasive endoscopic approach to the cervicothoracic junction for vertebral metastases: report of two cases.

The anterior cervicothoracic junction is difficult to expose and many techniques have previously been described. Most of them require an extensile exposure, which can lead to significant morbidity. The aim of this study is to present a less invasive approach, allowing the same exposure on the spine as a larger one. The approach begins with the same incision as the Smith-Robinson technique: a blunt dissection of the posterior face of the manubrium is performed with the finger. An endoscope is inserted through 10-mm trocars, one above the manubrium and the second through the second rib space. Th

Posterior Exposure of C1-C2 for Instrumentation and Fusion.

BACKGROUND: Posterior C1-C2 instrumentation and fusion is a well-established technique for the treatment of atlantoaxial instability. Safe posterior exposure is critically important because of the proximity of the vertebral artery, venous plexus, and C2 dorsal root ganglion. However, practical exposure nuances and safety pearls are often underemphasized. DESCRIPTION: The present video article demonstrates a stepwise approach for posterior exposure for C1-C2 instrumentation and fusion. Preoperative assessment includes dynamic flexion-extension radiographs to evaluate instability and reducibili

The juxtacondylar approach to the jugular foramen.

OBJECTIVE: We sought to describe the juxtacondylar approach to jugular foramen tumors. METHODS: Through an anterolateral approach, the third segment of the vertebral artery (between C2 and the dura mater) is controlled. The C1 transverse process of the atlas, which is located just inferiorly to the jugular foramen, is then removed. The dissection of the internal jugular vein is performed as high as possible, with control of the IXth, Xth, XIth, and XIIth cranial nerves. If required by a tumor extending into the neck, the internal and external carotid arteries can be exposed and controlled. Th

Peripheral Blood Gene Expression Profiling Reveals Molecular Pathways Associated with Cervical Artery Dissection.

Cervical artery dissection (CeAD) is the primary cause of ischemic stroke in young adults. Monogenic heritable connective tissue diseases account for fewer than 5% of cases of CeAD. The remaining sporadic cases have known risk factors. The clinical, radiological, and histological characteristics of systemic vasculopathy and undifferentiated connective tissue dysplasia are present in up to 70% of individuals with sporadic CeAD. Genome-wide association studies identified CeAD-associated genetic variants in the non-coding genomic regions that may impact the gene transcription and RNA processing.

Approach to cervicogenic dizziness: a comprehensive review of its aetiopathology and management.

PURPOSE: Though there is abundant literature on cervicogenic dizziness with at least half a dozen of review articles, the condition remains to be enigmatic for clinicians dealing with the dizzy patients. However, most of these studies have studied the cervicogenic dizziness in general without separating the constitute conditions. Since the aetiopathological mechanism of dizziness varies between these cervicogenic causes, one cannot rely on the universal conclusions of these studies unless the constitute conditions of cervicogenic dizziness are separated and contrasted against each other. Prognosis of spontaneous cervical artery dissection and transcranial Doppler findings associated with clinical outcomes.

OBJECTIVES: We categorised spontaneous cervical artery dissection (sCAD) by radiological features and investigated factors associated with favourable outcomes. METHODS: We retrospectively analysed 128 patients with sCAD with a median follow-up duration of 25 months. Twenty-nine constituted the aneurysm group, 52 the stenotic group, and 47 the occlusive group. Various relevant factors, including National Institute of Health Stroke Scale (NIHSS) scores, type of antithrombotic therapy, stroke progression in the first week, and transcranial Doppler (TCD) flow-waveforms (in the occlusive subgroup)

The V(2) segment of the vertebral artery: anatomical considerations and surgical implications.

OBJECT: Iatrogenic injury of the V(2) segment of the vertebral artery (VA) is a rare but serious complication and can be catastrophic. The purpose of this study was to characterize the relationship of the V(2) segment of the VA to the surrounding anatomical structures and to highlight the potential site and mechanisms of injury that can occur during common neurosurgical procedures involving the subaxial cervical spine. METHODS: Ten adult cadaveric specimens (20 sides) were included in this study. Quantitative anatomical measurements between selected landmarks and the VA were obtained. In addi
