Update: adolescent gynecology.

As adolescents develop their sexual identities they are at risk for acquiring sexually transmitted diseases, unwanted pregnancies, and cervical dysplasia. As physical growth and development progress during adolescence, the risk of developing dysmenoorhea or adverse effects from exposure to diethylstilbestrol increase. The pediatrician with a background in growth and development should be capable of recognizing these conditions and participating in their management.

Artificial Intelligence in Child and Adolescent Psychiatry: A Narrative Review of Recent Clinical Applications and Ethical Considerations.

PURPOSE OF REVIEW: This narrative review examines recent literature of artificial intelligence (AI) in child and adolescent psychiatry. With increasing mental health disorders in young people along with persistent workforce shortages, AI has emerged as a potential tool to improve efficiency and support clinical decision making. However, using AI raises important ethical concerns which are summarized in our review. RECENT FINDINGS: Most AI applications in child and adolescent psychiatry remain early in development and are not ready for routine clinical use. AI scribes are likely impractical in

Acute ketamine withdrawal disrupts memory and monoaminergic neurotransmission in adolescent female rats.

Adolescence is a period of profound behavioral changes associated with high brain vulnerability to negative stimuli including psychotropic drugs misuse which may contribute to the development of psychiatric disorders. Ketamine, an NMDA receptor antagonist, is an anesthetic agent used in a recreational manner in night clubs and raves, especially by intranasal route. The psychedelic effects of ketamine include euphoria, reward and mood swings, leading to an altered state of consciousness. Ketamine use has increased among adolescents and young adults, becoming a relevant global public health conc

Self-report Instruments for Anxiety, Depression, and Stress in Adolescents: A Scoping Review of Psychometric Properties and Cultural Applicability.

PURPOSE OF THE REVIEW: This scoping review mapped self-report instruments assessing anxiety, depression, and stress among adolescents, focusing on their psychometric properties and cultural applicability in the Indian context. It was anticipated that most instruments would be developed and validated in Western settings, with limited validation in Indian adolescent samples; that multidimensional tools covering all three constructs would be fewer than single-construct measures, and that cultural and linguistic adaptation for Indian adolescents would be insufficient. METHODS: Following Arksey an

Mental health and substance use problems among adolescents in Lesotho: Prevalence, access to care, and association with lifestyle factors.

Adolescence is a developmental window in which mental health symptomatology and other health behaviors can set lifelong trajectories. Yet, adolescent development research in rural areas of low-resource communities is severely lacking. Conceptualized in the Adolescent Well-Being Framework, where diet and activity form a core domain of well-being, we examined two modifiable risk behaviors (sugar-sweetened beverage consumption and physical activity) and their associations with mental health and substance use problems among 1351 adolescents aged 10-17 years (51% girls) in a rural setting in Lesoth…

Vaccination for prevention of hearing loss: a scoping review.

BACKGROUND: Infectious diseases in childhood and adolescence are significant and often preventable causes of hearing loss, especially in low- and middle-income countries. We conducted a scoping review to examine the extent, range and nature of available evidence on the role of vaccination for prevention of hearing loss worldwide. METHODS: We reviewed the published scientific literature to identify studies providing quantitative information on the relationship between vaccination and hearing loss. We searched four databases: MEDLINE, EMBASE, Cochrane Library and Global Index Medicus. No date,

POLE-mutated atypical endometrial hyperplasia in an adolescent: a 5-year follow-up with LNG-IUS-a case report and literature review.

This case report describes a rare instance of atypical endometrial hyperplasia (AEH) in a 16-year-old adolescent female presenting with secondary amenorrhea lasting 3 years. Imaging studies demonstrated a honeycomb-like intrauterine mass abutting the endometrial-myometrial junction, measuring 60 mm in length and 33 mm in maximal thickness. Diagnosis of POLE-mutated AEH (WHO 2020 criteria) was confirmed via hysteroscopic biopsy and molecular profiling. Initial treatment with oral megestrol acetate (160 mg/day) for 6 months failed to resolve complex hyperplasia, prompting a transition to a levon

The Role of School Engagement in the Link Between Adverse Childhood Experiences and Adolescent Mental Health.

PURPOSE: Adverse childhood experiences (ACEs) increase the risk for poor adolescent mental health, yet mediational pathways that connect ACEs with mental health problems remain unaddressed. Our study investigates whether school involvement and disengagement mediate the longitudinal associations between ACEs and adolescent mental health problems. METHODS: Using data from the longitudinal Adolescent Brain Cognitive Development Study, this study included 7,558 adolescents aged 9-10 years at baseline. ACEs were a cumulative risk score of 21 adverse experiences (e.g., emotional abuse, community vi

Lifelong exposure to n-3 PUFA deficiency leads to anxiety-like profile in male and female adolescent rats: Impact on spleen-brain axis.

Low consumption of n-3 polyunsaturated fatty acids (PUFA) during the developmental period has been increasingly associated with an increased risk of depressive-like symptoms in both male and female sexes. Therefore, here we performed behavioral and biochemical quantifications in adolescent rats to evaluate possible sex-driven differences in the development of anxiety-like disorders related to life-long n-3 PUFA low intake. Male and female adolescent rats fed for their entire life with n-3 PUFA poor diet showed an anxiety-like profile compared to n6/n-3 PUFA balanced diet. However, such deficie

Adolescent exposure to sucrose increases cocaine-mediated behaviours in adulthood via Smad3.

Adolescence, a critical period of developmental period, is marked by neurobiological changes influenced by environmental factors. Here, we show how exposure to sucrose, which is ubiquitously available in modern diets, results in changes in behavioural response to cocaine as an adult. Rats were given daily access to either 10% sucrose or water during the adolescent period (PND28-42). Following this period, rats are left undisturbed until they reach adulthood. In adulthood, rats were tested for (i) acquisition of a low dose of cocaine, (ii) progressive ratio (PR) test, and (iii) resistance to pu

Prevalence and factors associated with gynoid lipodystrophy in Brazilian adolescent girls: a cross-sectional study.

BACKGROUND: Gynoid lipodystrophy (GLD), or female cellulitis, is a chronic multifactorial disorder characterized by the irregular appearance of the skin surface on the thighs and gluteal regions, which affects 80-90% of women after puberty. No curative treatments are available, and there is a paucity of epidemiological studies related to its prevalence, quality of life impact, and risk factors. We aimed to identify the prevalence and factors associated with the development of GLD in Brazilian adolescent girls. METHODS: A cross-sectional study examined adolescent girls from São Paulo, Brazil.

Early childhood lead exposure and adolescent heart rate variability: A longitudinal cohort study.

Lead is a known neurotoxicant with many detrimental health effects, including neurocognitive deficits and cardiovascular and metabolic disorders. However, few studies have tested the association between lead exposure and the physiological stress response, which in and of itself may act as a precursor to and/or underlying mechanism of detrimental health outcomes. The purpose of this study was to examine the influence of early childhood and early adolescent low-level lead exposure on early adolescent heart rate variability, a widely-used measure of physiological stress. Participants were 408 chi

An Adolescent Sensitive Period for Social Dominance Hierarchy Plasticity Is Regulated by Cortical Plasticity Modulators in Mice.

Social dominance hierarchies are a common adaptation to group living and exist across a broad range of the animal kingdom. Social dominance is known to rely on the prefrontal cortex (PFC), a brain region that shows a protracted developmental trajectory in mice. However, it is unknown to what extent the social dominance hierarchy is plastic across postnatal development and how it is regulated. Here we identified a sensitive period for experience-dependent social dominance plasticity in adolescent male mice, which is regulated by mechanisms that affect cortical plasticity. We show that social do

Repetitive Mild Traumatic Brain Injury Alters Glymphatic Clearance Rates in Limbic Structures of Adolescent Female Rats.

The glymphatic system is the macroscopic waste clearance system for the central nervous system. Glymphatic dysfunction has been linked to several neurological conditions, including traumatic brain injury (TBI). Adolescents are at particularly high risk for experiencing a TBI, particularly mild TBI (mTBI) and repetitive mTBI (RmTBI); however, glymphatic clearance, and how it relates to behavioral outcomes, has not been investigated in this context. Therefore, this study examined glymphatic function in the adolescent brain following RmTBI. Female adolescent Sprague Dawley rats were subjected to

A review of the literature on good practice considerations for initial health system response to child and adolescent sexual abuse.

BACKGROUND: Healthcare workers play an important role in responding to the needs of the millions of children and adolescents who experience sexual abuse around the globe. A supportive child and adolescent-centered initial response is crucial for the physical and emotional wellbeing of survivors. OBJECTIVE: In 2017, the World Health Organization (WHO) published clinical guidelines for responding to child and adolescent sexual abuse. The review described in this paper informed the development of good practice statements on how best to deliver health care to survivors. PARTICIPANTS AND SETTING:

Adolescent males with atypical anorexia nervosa and premorbid obesity: three case reports.

PURPOSE: Premorbid obesity is an identified risk factor for eating disorder (ED) development among adolescent males. However, pervasive gender- and weight-related biases about ED inhibit timely diagnosis and treatment among this demographic. This study examined the psychological and medical characteristics of three adolescent males with premorbid obesity who were not diagnosed with atypical anorexia nervosa (AAN) until medical sequelae of malnutrition warranted emergent hospitalization. Factors associated with diagnostic delays among these cases were identified to facilitate increased awarenes

Disruption of adolescents' circadian clock: The vicious circle of media use, exposure to light at night, sleep loss and risk behaviors.

Although sleep is a key element in adolescent development, teens are spending increasing amounts of time online with health risks related to excessive use of electronic media (computers, smartphones, tablets, consoles…) negatively associated with daytime functioning and sleep outcomes. Adolescent sleep becomes irregular, shortened and delayed in relation with later sleep onset and early waking time due to early school starting times on weekdays which results in rhythm desynchronization and sleep loss. In addition, exposure of adolescents to the numerous electronic devices prior to bedtime has …
