Neurosurgical Robot-Assisted Navigation Enhances Precision in Bypass Surgery for Moyamoya Disease: A Study of Vascular Protection and Haemodynamic Optimization.

BACKGROUND: Despite improvements in revascularisation, precise donor vessel localization and haemodynamic optimization remain critical challenges in moyamoya disease (MMD) bypass surgery, especially for patients with pre-existing extracranial-intracranial (EC-IC) collaterals. This study evaluates the feasibility of neurosurgical robot-assisted navigation in addressing these challenges. METHODS: Five consecutive MMD patients who received robot-assisted combined direct (STA-MCA) and indirect bypass from July to October 2023 were analysed retrospectively. Primary outcomes were intraoperative ves

Moyamoya Periventricular Choroidal Collateral Registry-A Multicenter Cohort Study of Moyamoya Disease with Choroidal Anastomosis: Study Protocol.

Recent cohort studies on hemorrhagic and asymptomatic moyamoya disease have revealed that choroidal anastomosis, a type of fragile periventricular collateral pathway (periventricular anastomosis) typical of the disease, is an independent predictor of hemorrhagic stroke. However, treatment strategies for less-symptomatic nonhemorrhagic patients with choroidal anastomosis remain unclear. The Moyamoya Periventricular Choroidal Collateral (P-ChoC) Registry is an ongoing multicentered observational study that will test the hypothesis that extracranial-intracranial bypass prevents de novo hemorrhagi

Minimally Invasive Surgical Repair of Vertebral Artery Ostium Stenosis in Patients with Ischemic Stroke: A Single-Center Case Series.

BACKGROUND: Ischemic stroke is the second leading cause of death in North Macedonia. Posterior circulation disease, caused by vertebral artery (VA) ostium (VAo) stenosis, is a common cause of ischemic stroke. We established a treatment approach using surgical revascularization of posterior circulation disease. In the present observational study, we assessed the outcome after surgical revascularization of the posterior circulation ischemia caused by VAo stenosis. METHODS: A retrospective analysis of 20 consecutive patients who had undergone surgery from January 2017 to December 2019. The VA wa

Successful Surgical Management of Traumatic Intracranial Hemorrhaging After Revascularization Surgery for Moyamoya Vasculopathy: A Case Report and Review of Literature.

BACKGROUND: Traumatic intracranial hemorrhaging associated with revascularization surgery for moyamoya vasculopathy is a potentially devastating problem that requires meticulous management, including surgery. However, only a few studies on this subject have been reported, and the clinical characteristics are poorly understood. We report a case of successful surgical management for a patient with traumatic intracranial hematoma managed with encephalo-duro-arterio-myo-synangiosis (EDAMS). The purpose of this article is to clarify the specific features of clinical scenarios, hemorrhagic sites, an

Internal Thoracic Artery to Vertebral Artery Bypass Surgery: A Cadaveric Feasibility Study.

OBJECTIVE: Posterior circulation strokes account for over one quarter of all ischemic strokes. The frequency of vertebral artery origin stenosis (VAOS) in patients with vertebrobasilar insufficiency (VBI) has been estimated to be as high 26%-32%, and VAOS is the direct cause of posterior circulation strokes in 9% of patients. This association could have a significant genetic component. This study examines the feasibility of the internal thoracic artery (ITA) as a donor vessel for revascularization in patients with VAOS. METHODS: Ten sides from 5 fresh-frozen white cadaveric necks derived from

External carotid stenting for symptomatic stenosis in a patient with patent EDAS for Moyamoya disease.

BACKGROUND: Moyamoya disease is characterized by progressive narrowing of the internal carotid artery (ICA). Symptomatic patients typically undergo cerebrovascular intervention via extracranial-intracranial (EC-IC) bypass, most often with the use of the superficial temporal artery. This case of Moyamoya disease is of particular interest as the patient presented with a unilateral atherosclerotic external carotid artery (ECA) stenosis after EC-IC bypass that eliminated the benefit of his original surgery, resulting in a symptomatic presentation. CLINICAL PRESENTATION: A 53-year-old man presenti

Subarachnoid hemorrhage caused by ruptured posterior inferior cerebellar artery aneurysm arising from the extracranial portion of an anomalous vertebral artery.

A 42-year-old woman presented with subarachnoid hemorrhage caused by a ruptured posterior inferior cerebellar artery aneurysm arising from the extracranial portion of an anomalous vertebral artery. We speculated that the aneurysm formed by dissection of the media because of its shape and location. Therefore, the aneurysm was isolated by trapping and excised, with occipital artery to posterior inferior cerebellar artery bypass. Histological examination showed a typical true aneurysm without evidence of dissection. This case suggests that an aneurysm of an anomalous artery is caused by hemodynam

Revascularisation surgery and long-term follow-up in juvenile Moyamoya syndrome: a retrospective analysis.

Due to its low incidence in Western countries, physician awareness of juvenile Moyamoya disease should be improved. The benefits of revascularisation surgery have only been proven in the juvenile version of the disease. Therefore, early revascularisation may prevent irreversible ischaemic deficits and rapidly progressive mental retardation in young patients. From 1984 to 2009, a total of 19 children (mean age 8 years, range 1-18 years, female predominance 2:1, 17/19 European white patients, 2/19 Asian origin of at least one parent) were treated for juvenile Moyamoya disease by surgical revascul

[Treatment of trigeminal neuralgia with microvascular decompression].

Pure microvascular decompression (MVD) can cure (that is, no pain, no medication) primary trigeminal neuralgia (TN) caused by vascular compression in 75% of patients (90% when compression is pronounced), according to a Kaplan-Meier survival study at 15 years. MRI with high resolution evidences neurovascular conflicts with good reliability. The results were found to be significantly better when the prosthesis implanted to maintain the compressive vessel away was not touching the nerve. This argues in favor of a real decompressive mechanism of the MVD procedure, rather than a conduction block. B

Omental transposition decreases ischemic brain damage examined in a new ischemia model.

PURPOSE: The aim of this study was to determine whether omental transposition at the time of focal cerebral ischemia can decrease ischemic brain damage produced in dogs, in a new ischemia model, which had been described by us. METHODS: In group 1 (n = 5), the left internal carotid artery and arterial circle of the brain (posterior communicating artery in humans) were occluded permanently. In group 2 (n = 5), additionally to this ischemia model, omental transposition was performed simultaneously. In the postoperative early period (first 24 h), single photon emission computed tomography (SPECT)

Blood brain barrier breaking. Is still an absolute contraindication to early surgical reperfusion of the brain?

In the early treatment of the patients with cerebrovascular insufficiency due to internal carotid artery stenosis, the presence of a cerebral infarct and especially the blood brain barrier breaking (BBB) are considered by many as a contraindication to early reperfusion by carotid endarterectomy (CEA). Generally, it has been recommended to differ the operation at least for 4-6 weeks because of the high risk to convert an ischemic infarct into an hemorrhagic one. On the other hand, because unfavorable natural history has been reported as for the progressing unstable neurological deficit as for t

[Prevention of cerebrovascular disorders].

The decision for the optimal preventive and therapeutic interventions in cerebrovascular disease depends on the underlying disease process. Therefore it is important to identify the different pathomechanisms by modern techniques. The significantly increased cardiovascular risk of patients with atherosclerotic extracranial arterial disease--even when neurologically asymptomatic--makes identification and elimination of all vascular risk factors of crucial importance for primary and secondary prevention. The low risk of stroke without prior transient ischemic attacks makes prophylactic carotid su

[Cognitive functions after surgical interventions on extracranial carotid arteries (literature review)].

In recent years, surgical interventions on the carotid arteries - such as carotid endarterectomy and stenting - have become key methods for preventing ischemic stroke. In addition to their proven clinical effectiveness, increasing attention is being paid to their influence on patients' cognitive functions. This review summarizes current data from both domestic and international studies on the dynamics of cognitive function following carotid revascularization. It discusses the primary mechanisms behind cognitive changes, including reperfusion injury, microembolism, and cerebral hyperperfusion s

Moyamoya disease and syndrome: a review.

Moyamoya disease is a chronic occlusive cerebrovascular disease that is non-inflammatory and non-atherosclerotic. It is characterized by endothelial hyperplasia and fibrosis of the intracranial portion of the carotid artery and its proximal branches, leading to progressive stenosis and occlusion, often clinically manifesting as ischemic or hemorrhagic stroke with high rates of morbidity and mortality. On cerebral angiography, the formation of collateral vessels has the appearance of a puff of smoke (moyamoya in Japanese), which became more conspicuous with the refinement of modern imaging tech

Moyamoya Disease in a Six Month Caucasian Female.

Moyamoya disease (MMD) is a progressive cerebral arteriopathy characterized by stenosis and/or occlusion of the internal carotid arteries and the arteries around the Circle of Willis, with the development of "moyamoya" vessels, which are an attempt at revascularization at the base of the brain. In this paper we describe a 6 month, 3-week-old girl who presented with seizures and strokes due to moyamoya disease. The diagnosis of early onset MMD was made due to the magnetic resonance angiography results showing severe stenosis of the terminal/supraclinoid carotid arteries bilaterally with moyamoy
