Electrophysiology assessment and radiofrequency ablation of arrhythmias in adult patients with congenital heart defects: the Christchurch experience.

INTRODUCTION: Adults with congenital heart disease (CHD) frequently have cardiac arrhythmias, many of which are best treated with radiofrequency ablation (RFA). We present our experience in this group. METHODS: Retrospective chart based review of diagnosis, arrhythmia type, results of cardiac electrophysiological assessment, and procedural and long-term clinical success of radiofrequency ablation. RESULTS: Forty-five patients were identified with CHD and arrhythmias undergoing RFA; including surgically repaired atrial septal defects (21), Ebstein's anomaly (12), repaired transposition of the

What to expect after tricuspid valve replacement? Long-term results.

OBJECTIVE: Current knowledge in long-term results of tricuspid valve replacement is limited. Present study reviews our experience from a consecutive series. METHODS: Forty-two patients (16 male, 26 female; mean age: 33+/-15) underwent tricuspid valve replacement between March 1987 and December 2004. The etiology was rheumatic in 64%, Ebstein's anomaly in 31%, and endocarditis in 5%. Nineteen patients were in New York Heart Association (NYHA) Class III functional capacity (45%), and 13 in class IV (31%). Twenty patients (48%) underwent isolated tricuspid valve replacement. The remaining underw

Some aspects of fetal echocardiogram: 12 years of experience.

Fetal echocardiogram is a reliable tool for prenatal diagnosis of congenital heart disease. It is an important adjunction to obstetrical ultrasonography in caring for women during pregnancy. With improvement in resolution of 2-D echo images as well as the addition of parameter software, fetal echocardiography has found ever-expanding applications in both the structural and functional assessment of the fetal cardiovascular system. From the author's 12 years experience in fetal echocardiogram, 1,000 fetal hearts were studied and 12 cases had congenital heart defects (CHD): 4 ventricular septal d

Radiofrequency ablation for accessory pathways: success and variants.

INTRODUCTION: Radiofrequency ablation (RFA) is currently being advocated as the first line of therapy for symptomatic and drug refractory arrhythmias mediated by accessory pathways (APs). However, a large database on RFA for APs from India is still lacking. METHODS: Four hundred thirty-four patients (298 males, (69%)), with APs who underwent RF ablation between January, 1998 and May, 2001 were included. The success and variants were evaluated retrospectively from the data. RESULTS: The mean age was 36 +/- 11 years. There were 314 (73%) patients with WPW syndrome while the other 120 (27%) had

Abnormal neurohumoral responses to exercise in patients with heart disease: inhibition of an increase in endothelin-1 production during exercise.

We have reported that the plasma endothelin-1 (ET-1) level is significantly increased by exercise in healthy athletes and that it is elevated in the circulation of the non-working leg but not the working leg, suggesting that ET-1 plays an important role in redistribution of blood during exercise. This study was designed to compare alterations of neurohumoral substances by exercise in normal subjects and patients with heart disease. Study patients comprised three groups: eight patients with congestive heart failure (CHF) due to Ebstein's anomaly or single-ventricle heart after Fontan operation;

Morphology of tricuspid valve in pulmonary atresia with intact ventricular septum.

Pulmonary atresia with intact ventricular septum (PAIVS) is a rare congenital cardiac anomaly that has been classified into two types: one is a more frequent type having dysplasia of tricuspid valve (TV) with a small annulus, underdeveloped right ventricle (RV) with a hypoplastic cavity and a hypertrophic wall; the other type has severe dysplasia of TV and dilatation of RV, right atrium (RA), and right atrioventricular junction with thinning of the RV wall. We performed a morphologic study on 11 autopsied hearts with PAIVS, giving particular emphasis to the variation of morphology of the TV. W

Low energy direct current ablation in patients with the Wolff-Parkinson-White syndrome: clinical outcome according to accessory pathway location.

Forty-five patients with the Wolff-Parkinson-White syndrome underwent direct current (DC) ablation using a low energy power source (Cardiac Recorders). Anodal shocks of 10-40 joules were given to either a 6 French quadripolar catheter (Bard), a 7 French bipolar contoured catheter (Bard), or a 7 French deflectable catheter with a 4-mm distal electrode (Mansfield). The indifferent electrode consisted of a large patch that was positioned under the left scapula. There were 26 males and 19 females, with a mean age of 34 years (range 9-67). Accessory pathways were located in the left free wall in 30

Preexcited reciprocating tachycardia in patients with Wolff-Parkinson-White syndrome: incidence and mechanisms.

During the electrophysiologic study of 435 patients referred for evaluation of Wolff-Parkinson-White syndrome, 42 (10%) had preexcited reciprocating tachycardia (defined as a macro-reentrant tachycardia that used an accessory atrioventricular [AV] pathway for antegrade conduction). The ages of the patients ranged from 9 to 67 years (27 +/- 14). Thirty-three were male patients, nine female, and eight had Ebstein's anomaly. Preexcited reciprocating tachycardia cycle length was 220 to 430 msec (294 +/- 42). Significant hemodynamic compromise in the laboratory directly related to preexcitated reci

Right atrial-ventricular dissociation and entrainment while pacing from high right atrium and coronary sinus during circus movement tachycardias.

Thirteen patients with circus movement tachycardias (CMT) were studied. Twelve had left-sided, and one, right-sided, accessory pathways. Entrainment was possible during overdrive high right atrial stimulation in 13/13 patients, and during coronary sinus pacing in 10/12 patients. The minimal pacing rates required for this to occur were 10 to 31 beats/min faster than those of the tachycardias. Short episodes of right atrial-ventricular dissociation occurred while pacing from the high right atrium (6/13 patients), but not from the coronary sinus (0/13 patients). It is possible to explain this phe

Evaluation of cardiac diseases with computed tomography.

Fifty-six consecutive patients with various cardiac diseases were studied with a third-generation whole body computed tomographic system. Enhancement was performed by intravenous injection of angiographic contrast medium. Identification of the great vessels, atria and ventricles, and evaluation of abnormalities in these structures were possible by cardiac computed tomography. For example, enlarged left ventricle in the left ventricular volume load, enlarged right ventricle in the right ventricular volume load or hypertrophied interventricular septum in idiopathic hypertrophic subaortic stenosi

Anatomic repair of Ebstein's malformation: lessons learned with cone reconstruction.

BACKGROUND: Reproducible repair of Ebstein's malformation is challenging and numerous surgical techniques have been described. We reviewed our experience with the cone reconstruction. METHODS: Between June 2007 and December 2011, 89 patients (47 female; 53%) underwent cone reconstruction (median age 19 years; range, 19 days to 68 years). Indication for operation was progressive cardiomegaly in 43 (48%), cyanosis in 29 (33%), and heart failure in 13 (15%). Prior tricuspid valve repair was performed in 12 patients (13%). Severe tricuspid regurgitation (TR) was present in 75 patients (84%). RES

Sudden death in congenital heart disease.

Sudden cardiac death is a common mechanism of demise in association with congenital cardiac abnormalities. The varied mechanisms may include failure of the transitional circulation, arrhythmias, postoperative or perioperative complications in the neonate and coronary ischaemia, arrhythmias, sepsis, thrombosis, or pulmonary hypertensive crisis in the older child. Knowledge of the natural history of unoperated and operated forms of congenital heart disease and long term follow up with the detection and treatment of underlying hemodynamic abnormalities should improve outcomes. There are few patie

Clinics in diagnostic imaging (51). Multiloculated cerebral abscesses due to paradoxical cardiac emboli.

A 19-year-old woman with congenital cyanotic heart disease (Epstein anomaly and ventricular septal defect) had persistent headaches and intermittent fever for five weeks. Physical examination revealed central cyanosis, finger clubbing and fever. The leucocyte count was elevated. Cerebral MR imaging showed the characteristic features of brain abscesses in the right frontal lobe, including multiloculation with adjacent satellite lesions, ring enhancement, T1-hyperintense and T2-hypointense signal areas within the abscess rim, as well as hypointense internal concentric rings on T2-weighted images
