Differential blood count as triage tool in evaluation of pelvic masses.

OBJECTIVE: Triaging patients with presumptive ovarian cancer to the appropriate specialist may improve survival. Therefore, there is increasing interest in complementary diagnostic markers to the standard serum CA125. In patients with pelvic masses, we examined the ability of epidemiologic variables and preoperative differential blood counts to improve detection of ovarian cancer over CA125 alone. METHODS: From pathology reports, patients were classified as having: epithelial ovarian cancer (n=743), including fallopian tube and primary peritoneal cancer, non-epithelial ovarian cancers (n=46),

A Conceptual Framework for the Assessment of Cumulative Exposure to Air Pollution at a Fine Spatial Scale.

Many epidemiological studies examining long-term health effects of exposure to air pollutants have characterized exposure by the outdoor air concentrations at sites that may be distant to subjects' residences at different points in time. The temporal and spatial mobility of subjects and the spatial scale of exposure assessment could thus lead to misclassification in the cumulative exposure estimation. This paper attempts to fill the gap regarding cumulative exposure assessment to air pollution at a fine spatial scale in epidemiological studies investigating long-term health effects. We propose

Measuring recent in-country TB transmission using a classification model with whole genome sequencing data.

BACKGROUND: Molecular typing of Mycobacterium tuberculosis isolates provides insight into TB transmission by identifying clustering isolates. However, clustering alone does not necessarily indicate ongoing in-country transmission, as infections may have occurred abroad or long ago. METHODS: We developed a classification model categorising clustering TB patients into three groups: i) likely, ii) possibly, and iii) unlikely to have been recently infected in the Netherlands. The model consists of two stages: i) individual labelling, followed by ii) cluster labelling for the remaining cases, and

Epidemiologic and Economic Impacts of Halting National HIV Prevention Efforts.

News outlets are reporting that CDC's HIV prevention program including its Ending the HIV Epidemic (EHE) efforts are slated to be defunded. One way to estimate the potential impacts of halting HIV prevention in the nation is to use mathematical modeling to provide informed estimates of the epidemiologic and economic impacts of this purported policy shift. We describe here a basic transmission rate model to estimate the HIV-related epidemiologic and economic horizon through 2030 as if funding were untouched and as if it were to be ended. This model is compared to other lenses through which to v

Revisiting immunity vs. exposure in schistosomiasis: A mathematical modeling study of delayed concomitant immunity.

The relative contributions of exposure vs. acquired immunity to the epidemiology of human schistosomiasis has been long debated. While there is considerable evidence that humans acquire partial immunity to infection, age- and sex-related contact patterns with water bodies contaminated with infectious cercarial schistosome larvae also contribute to typical epidemiological profiles of infection. Here, we develop a novel schistosome transmission model that incorporates both partially protective "delayed concomitant" acquired immunity-stimulated by dying worms-and host age- and sex-dependent patte

Comparison of random forest and parametric imputation models for imputing missing data using MICE: a CALIBER study.

Multivariate imputation by chained equations (MICE) is commonly used for imputing missing data in epidemiologic research. The "true" imputation model may contain nonlinearities which are not included in default imputation models. Random forest imputation is a machine learning technique which can accommodate nonlinearities and interactions and does not require a particular regression model to be specified. We compared parametric MICE with a random forest-based MICE algorithm in 2 simulation studies. The first study used 1,000 random samples of 2,000 persons drawn from the 10,128 stable angina p

Cost-effectiveness of a community-based intervention for reducing the transmission of Schistosoma haematobium and HIV in Africa.

Epidemiological studies from sub-Saharan Africa show that genital infection with Schistosoma haematobium [corrected] may increase the risk for HIV infection in young women. Therefore, preventing schistosomiasis has the potential to reduce HIV transmission in sub-Saharan Africa. We developed a transmission model of female genital schistosomiasis and HIV infections that we fit to epidemiological data of HIV and female genital schistosomiasis prevalence and coinfection in rural Zimbabwe. We used the model to evaluate the cost-effectiveness of a multifaceted community-based intervention for preven

Genetic-epidemiologic study of omphalocele and gastroschisis: evidence for heterogeneity.

On the basis of clinical manifestations, epidemiologic characteristics, and the presence of additional malformations, omphalocele (OM) and gastroschisis (GA) are considered casually and pathogenetically distinct abdominal wall defects. More than 50% of infants with OM have additional defects, but only about 15% of those with GA do. To evaluate whether there is heterogeneity between isolated and multiply affected cases of OM and GA, we analyzed epidemiologic characteristics and familial risks of major defects for 82 OM and 81 GA cases drawn from a population-based study in the Maryland-Washingt

Use of mathematical modelling to estimate the impact of interventions and risk factors on cardiovascular diseases in Africa: A scoping review.

BACKGROUND: Achieving health-related Sustainable Development Goals (SDGs) in Africa is increasingly threatened by the rising burden of cardiovascular diseases (CVD), exacerbated by limited healthcare infrastructure and constrained resources. Mathematical modelling offers a powerful tool for informing CVD prevention and control strategies; however, its application in Africa remains poorly understood. This scoping review aims to map existing modelling studies, identify methodological and contextual gaps in the literature, and inform future model development for effective decision-making. METHOD

Incidence of acute hemorrhagic conjunctivitis in Chongqing: a forecasting study based on mathematical models.

BACKGROUND: Acute hemorrhagic conjunctivitis (AHC) is a highly infectious eye disease. It poses a significant threat to public health given its propensity for rapid transmission in densely populated areas. Recent epidemiological data have demonstrated a distinct seasonal outbreak pattern in Chongqing. However, conventional single prediction models exhibit limitations in accurately capturing the complex spatiotemporal transmission characteristics of AHC. This study endeavors to compare the performance of different mathematical models in forecasting AHC incidence in Chongqing. Through the invest

[The 'diabetes crisis': a historical reflection on the nature of the problem].

Prompted by new epidemiological data on 'prediabetes', the current debate on the 'diabetes crisis' focuses on the scale of the problem. Less attention has been paid to the nature of the problem and the precise meaning of the aforementioned epidemiological data. In this regard, the history of prediabetes provides two valuable insights. First, a 'taxonomic expansion' has occurred, with the categories 'prediabetes' and 'diabetes' penetrating ever deeper into the domain of the asymptomatic. In addition, the 'convergence' of disease and risk has created confusion between two different ways of model

The epileptic heart: Cardiac comorbidities and complications of epilepsy. Atrial and ventricular structure and function by echocardiography in individuals with epilepsy - From clinical implications to individualized assessment.

Epilepsy is an increasing global neurological health issue. Recently, epidemiological and mechanistic studies have raised concern about cardiac involvement in individuals with epilepsy. This has resulted in the "epileptic heart" concept. Epidemiological data linking epilepsy to cardiovascular disease indicate an increased risk for ventricular and atrial arrhythmias, myocardial infarction, heart failure, and sudden death among individuals with epilepsy. Pathways of this interaction comprise increased prevalence of traditional cardiac risk factors, genetic abnormalities, altered brain circuitry

Targeting malaria parasites inside mosquitoes: ecoevolutionary consequences.

Proof-of-concept studies demonstrate that antimalarial drugs designed for human treatment can also be applied to mosquitoes to interrupt malaria transmission. Deploying a new control tool is ideally undertaken within a stewardship programme that maximises a drug's lifespan by minimising the risk of resistance evolution and slowing its spread once emerged. We ask: what are the epidemiological and evolutionary consequences of targeting parasites within mosquitoes? Our synthesis argues that targeting parasites inside mosquitoes (i) can be modelled by readily expanding existing epidemiological fra

Spatial and temporal dynamic analysis of rabies: A review of current methodologies.

Rabies continues to be one of the deadliest, high risk diseases worldwide, posing a severe threat to public health. The lack of human-to-human transmission means that the spread of rabies is not significantly affected by the distribution of humans or migra- tion. Thus, the spatiotemporal dynamic of cases in both wild and domestic animals is an important issue that can result in human cases. This paper gives an overview of the methodologies for the spatial and temporal dynamic analysis of this disease. It introduces the most representative research progress of spatial aggregation, dynamic trans

Epidemics Have Lost the Plot.

This article draws on Charles Rosenberg's classic essay "What Is an Epidemic?" (1989) to reflect on the complex narrative structures and temporalities of epidemics as they are experienced and storied. We begin with an analysis of Rosenberg's use of Albert Camus's The Plague and a discussion of how epidemics have been modeled in literature and in epidemiology concomitantly. Then, we argue that Charles Rosenberg's characterization of epidemics as events bounded in time that display narrative and epidemiological purity fails to account for the reinvention of life within health crises. Cholera: recent updates.

PURPOSE OF REVIEW: In this review, we will examine updates in cholera epidemiology, advances in our understanding of pathogenesis and protective immunity, and changes to prevention strategies. RECENT FINDINGS: New modeling techniques and molecular epidemiology have led to advancements in our understanding of how Vibrio cholerae has persisted and re-emerged in new areas during the seventh pandemic. Use of next-generation sequencing has shed new light on immune responses to disease and vaccination, and the role of the gut microbiome in cholera. Increased efficacy and availability of vaccines ha

Modeling control strategies of respiratory pathogens.

Effectively controlling infectious diseases requires quantitative comparisons of quarantine, infection control precautions, case identification and isolation, and immunization interventions. We used contact network epidemiology to predict the effect of various control policies for a mildly contagious disease, such as severe acute respiratory syndrome, and a moderately contagious disease, such as smallpox. The success of an intervention depends on the transmissibility of the disease and the contact pattern between persons within a community. The model predicts that use of face masks and general

Spatial analysis of the distribution of intestinal nematode infections in Uganda.

The spatial epidemiology of intestinal nematodes in Uganda was investigated using generalized additive models and geostatistical methods. The prevalence of Ascaris lumbricoides and Trichuris trichiura was unevenly distributed in the country with prevalence greatest in southwest Uganda whereas hookworm was more homogeneously distributed. A. lumbricoides and T. Trichiura prevalence were nonlinearly related to satellite sensor-based estimates of land surface temperature; hookworm was nonlinearly associated with rainfall. Semivariogram analysis indicated that T. trichiura prevalence exhibited no s

Epidemiology of binge eating disorder.

OBJECTIVE: First described over 50 years ago, binge eating disorder (BED) only recently has become the focus of epidemiologic studies. This article provides a comprehensive review of these studies. METHOD: Relevant studies were examined and summarized in the form of a narrative review. RESULTS: Similar to the early studies of bulimia nervosa (BN), the first generation of epidemiologic studies of BED is limited in scope or methodology. They focus on prevalence rates and provide only basic demographic characteristics and often use less than optimal sampling or assessment methods. DISCUSSION:
