Urticarial contact dermatitis in food handlers.

OBJECTIVE: To show that prick testing (using fresh samples of the food suspected from the patient's history) and not only patch testing is the appropriate investigation in selected cases of hand dermatitis in patients who spend considerable time handling foods (for example, catering workers, cooks). SETTING: The Contact and Occupational Dermatitis Clinic at the Skin and Cancer Foundation, a tertiary referral centre in Sydney. PATIENTS: Fourteen patients with hand dermatitis present for an average of 6.17 years referred by dermatologists and occupational health physicians. INTERVENTIONS: Pat

The prognosis of contact dermatitis.

This article reviews the prognosis of contact dermatitis, particularly of occupational contact dermatitis. Most studies document a poor prognosis for occupational and nonoccupational contact dermatitis. The prognoses of occupational and nonoccupational contact dermatitis, irritant contact dermatitis, and allergic contact dermatitis are similar. Only a minority of studies on the prognosis of occupational contact dermatitis have found that a job change by the affected worker leads to clearing of the dermatitis. Dermatologic and nondermatologic factors associated with a poor prognosis are discuss

Nickel sensitivity and the course of atopic dermatitis in adulthood.

The course of dermatitis was followed in nickel-sensitive and nickel-negative atopic and non-atopic patients. Manifest dermatitis was seen in 70% of the nickel-allergic and in 64% of the nickel-negative female atopic dermatitis (AD) patients. Those atopic subjects who had minor symptoms in their teens suffered more from dermatitis if they had developed nickel allergy (p less than 0.025). Hands and the head region were the most common sites for current dermatitis in both groups.

[Buruli ulcer--necrotizing infection of the hand of a plastic surgeon].

The Buruli ulcer is caused by mycobacterium ulcerans. The clinical features of this atypical mycobacteriosis are similar to tuberculosis. Differentiation is only possible by microbiological investigation. In cases of chronic soft tissue infections mycobacteriosis must be taken into consideration. Since microbiological differentiation needs 8-12 weeks and chemotherapy is not effective, early and wide surgical excision is the treatment of choice.

Hospital employees and skin problems.

A total of 1481 hospital employees answered a questionnaire on atopy, hand eczema, and dry chapped hands. Out of 864 (58.7%) responders, 17% claimed to suffer from hand eczema. There was not significantly more hand eczema among women (17.6%) than men (15.7%). Atopic disposition was present in 17.5% of responders, of whom a significantly higher number (36.4%) claimed to have hand eczema. Dry chapped hands seemed to be a problem in 33.1%, mainly among nurses, assistant nurses and laboratory assistants. Technicians and X-ray assistants (38%) and kitchen workers (35.7%) claimed to suffer significa

Individual and environmental risk factors for hand eczema in hospital workers.

Individual and environmental risk factors in hand eczema have been investigated in a prospective cohort study of 2452 newly employed hospital workers with a follow-up time of 20 months. Current hand eczema was analyzed in 142 wet hospital workers from this cohort with respect to the etiologic importance of irritants, allergens and contact urticants. The density of the microflora and the effect on the microflora of topical treatment with a potent corticosteroid were studied in 20 patients with hand eczema. 'Wet' hospital work was found to increase the odds of developing hand eczema only twice c

Occupational Contact Dermatitis in the Post-COVID Era: From Barrier Dysfunction and Microbiome Dysbiosis to Prevention and Precision Management.

Background/Objectives: Occupational contact dermatitis (OCD) is the most common work-related skin disease, accounting for roughly 90-95% of occupational dermatoses and falling predominantly on the hands. It is rarely dangerous yet imposes a substantial burden through impaired quality of life, lost productivity, and premature exit from affected trades. The COVID-19 pandemic intensified this burden among healthcare workers, in whom the pooled one-year prevalence of self-reported hand eczema reaches around 27%; meta-analytic data link the increased risk principally to frequent handwashing and wet

Conventional versus daylight photodynamic therapy for recalcitrant hand warts: Efficacy, safety and recurrence.

BACKGROUND: Acral warts appear as skin-colored, verrucous, hyperkeratotic papules most located on the dorsal hands, distal fingers, subungual skin, and feet. Conventional photodynamic therapy (C-PDT) has shown a good efficacy and safety profile in patients with viral warts, in both adults and children, with a good cure rate and excellent cosmetic results in front of few and light side effects. The efficacy of daylight photodynamic therapy (DL-PDT) has not yet been evaluated. OBJECTIVES: to investigate and assess the short- and long-term efficacy of PDT using 10 % aminolevulinic acid (ALA) in

Hand and upper extremity function in workers with hand dermatitis.

BACKGROUND: Traditional methods of assessing impairment associated with skin conditions focus on clinical severity, medication usage, activities of daily living, and quality of life. Assessment of function and ability to work may provide important additional information when determining readiness to work. OBJECTIVES: The objective was to gain an understanding of the functional impairment of the hand and upper extremity associated with hand dermatitis and the impact of hand dermatitis on quality of life, work instability, and productivity. METHODS: Sixty-two patients with suspected contact de

[Products for hand hygiene and antisepsis: use by health professionals and relationship with hand eczema].

Hand hygiene is the most important measure for the prevention of nosocomial infection. We describe the different products available for hygiene and antisepsis of the hands and the use of these products in daily practice. Hand hygiene products such as soaps and detergents are a cause of irritant dermatitis in health professionals. This irritation is one of the principal factors affecting their use in clinical practice. Alcohol-based products are better tolerated and less irritant than soap and water; irritation should not therefore be a limiting factor in the use of these products and they are

Skin exposure in geriatric care - a comparison between observation and self-assessment of exposure.

The aim of this study was to compare observation and self-assessment of skin exposure to irritants during a working day in nursing in geriatric care. The study group consisted of 40 volunteers, 13 nurses, and 27 assistant nurses. Before the start of a working day, the participants completed a questionnaire regarding skin exposure to water, gloves, hand disinfection, and moisturizers. Observers subsequently used a hand-held computer to register the time and frequency of each exposure. The total mean skin exposure time was 96 min per working day, including glove use. The mean exposure time to wa

Hyperkeratotic hand eczema compared to other subgroups of hand eczema - a retrospective study with a follow-up questionnaire.

BACKGROUND: Only a few clinical trials on hand eczema have included further classification into subtypes, and there is a need for studies evaluating the present classifications. OBJECTIVES: To examine differences in demographic factors, lifestyle factors and severity between subgroups of hand eczema patients, with a focus on hyperkeratotic hand eczema. METHODS: This was a retrospective study including hand eczema patients referred to the outpatient dermatological clinic, Bispebjerg Hospital, between January 2013 and July 2014. The study comprises information on subdiagnoses, treatment and fo

Onychomycosis in Cameroon: a clinical and epidemiological study among dermatological patients.

BACKGROUND: There are currently no studies on epidemiology and clinical aspects of nail fungal infections in the general population of Cameroon. PATIENTS AND METHODS: Two series of patients observed at a hospital dermatological service of Yaoundé and in a volunteer service of Douala were evaluated. All the patients, regardless of the reason for the consultation, were examined by an expert dermatologist to discover signs of onychomycosis. Patients with suspected nail lesions underwent mycological examination according to the standard techniques. RESULTS: Among 590 subjects (317 males and 273
