Therapeutic Neuroendocrine Agonist and Antagonist Analogs of Hypothalamic Neuropeptides as Modulators of the Hypothalamic-Pituitary-Gonadal Axis.

Reproductive hormones play a role at all stages of life and affect most tissues of the body. Gonadotropin-releasing hormone (GnRH) synthesized in the hypothalamus stimulates the secretion of gonadotropins which in turn stimulate gonadal sex hormone production and gamete formation. This hypothalamic-pituitary-gonadal (HPG) axis has, therefore, been the target for the development of numerous drugs which regulate it at various points. These include sex steroid agonists and antagonists, inhibitors of sex steroid biosynthesis, and GnRH agonists and antagonists, which have found extensive applicatio

Differential associations between childhood trauma subtypes and adolescent HPA-axis functioning.

UNLABELLED: Studies examining the association between childhood trauma exposure and neuroendocrine functioning have returned inconsistent findings. To date, few studies have accounted for the role exposure to different types of childhood trauma may have on different neuroendocrine adaptations, and no study has examined this association using multiple indices of hypothalamic-pituitary-adrenal axis (HPA-axis) functioning. The purpose of this study was to characterize the unique associations between exposure to physical abuse, emotional abuse, and non-intentional trauma, and multiple indices of H

Physiological basis for the etiology, diagnosis, and treatment of adrenal disorders: Cushing's syndrome, adrenal insufficiency, and congenital adrenal hyperplasia.

The hypothalamic-pituitary-adrenal (HPA) axis is a classic neuroendocrine system. One of the best ways to understand the HPA axis is to appreciate its dynamics in the variety of diseases and syndromes that affect it. Excess glucocorticoid activity can be due to endogenous cortisol overproduction (spontaneous Cushing's syndrome) or exogenous glucocorticoid therapy (iatrogenic Cushing's syndrome). Endogenous Cushing's syndrome can be subdivided into ACTH-dependent and ACTH-independent, the latter of which is usually due to autonomous adrenal overproduction. The former can be due to a pituitary c

Genetic aspects of hypothalamic and pituitary gland development.

Hypothalamo-pituitary development during embryogenesis is a highly complex process involving the interaction of a network of spatiotemporally regulated signaling molecules and transcription factors. Mutations in any of the genes encoding these components can lead to congenital hypopituitarism, which is often associated with a wide spectrum of defects affecting craniofacial/midline development. In turn, these defects can be incompatible with life, or lead to disorders encompassing holoprosencephaly (HPE) and cleft palate, and septo-optic dysplasia (SOD). In recent years, there has been increasi

The organization of the stress system and its dysregulation in depressive illness.

Stressors are imminent or perceived challenges to homeostasis. The stress response is an innate, stereotypic, adaptive response to stressors that has evolved in the service of restoring the nonstressed homeostatic set point. It is encoded in specific neuroanatomical sites that activate a specific repertoire of cognitive, behavioral and physiologic phenomena. Adaptive responses, though essential for survival, can become dysregulated and result in disease. A clear example is autoimmune disease. I postulate that depression, like autoimmunity, represents a dysregulated adaptive response: a stress

Harmful effects of functional hypercortisolism: a working hypothesis.

Functional hypercortisolism (FH) is caused by conditions able to chronically activate hypothalamic-pituitary-adrenal axis and usually occurs in cases of major depression, anorexia nervosa, bulimia nervosa, alcoholism, diabetes mellitus, simple obesity, polycystic ovary syndrome, obstructive sleep apnea syndrome, panic disorder, generalized anxiety disorder, shift work, and end-stage renal disease. Most of these states belong to pseudo-Cushing disease, a condition which is difficult to distinguish from Cushing's syndrome and characterized not only by biochemical findings but also by objective o

Menstrual disturbances in puberty.

The most significant corporal change observed in adolescent girls is the onset of menstruation, which occurs between 12 and 13 years of age. In several cases, described with the term 'precocious puberty', pubertal development can begin at a significantly younger age. The term 'delayed puberty' refers to absence of pubertal development in a girl over the age of 14. Amenorrhoea can occur due to a broad spectrum of causes, such as anatomic deficiencies of the reproductive tract and hormonal disorders. Hypogonadotrophic hypogonadism, which implies a permanent malfunction in gonadotrophin secretion

Hypopituitarism: clinical features, diagnosis, and management.

Hypopituitarism is characterized by loss of function of the anterior pituitary gland. It is a rare condition that can present at any age and is caused by pathology of the hypothalamic-pituitary axis or one of many gene mutations. The symptoms and signs of hypopituitarism may evolve over several years and be nonspecific or related to the effects of the underlying disease process or to hormone deficiencies. Investigation of patients requires a combination of basal hormone levels and dynamic function tests; management requires regular monitoring. The goal of physicians managing patients who have

Child maltreatment and the developing HPA axis.

The developing HPA axis is under strong social regulation in infancy and early childhood and is vulnerable to perturbation in the absence of sensitive, responsive caregiving. Child maltreatment has complex, long-term influences both on basal cortisol levels and on HPA responsivity to pharmacological and psychological stressors, depending on current psychiatric status, current life adversity, age, and most likely, genetic factors. Among the more consistent findings, maltreated children with internalizing problems have elevated basal cortisol most often detected in early AM concentrations, where

[Is alopecia areata a psychosomatic disease?].

INTRODUCTION: Destruction of hair follicles by lymphocytes induces alopecia aerata. Hence, immunological mechanisms are involved in this disease, as for numerous dermatoses. Nonetheless, alopecia aerata appears to be a psychosomatic disease. Is there any contradiction? CURRENT KNOWLEDGE AND KEY POINTS: Alopecia aerata often occurs after stress, particularly during mourning. Psychiatric disorders are more frequent in patients with alopecia than in healthy subjects. But these disorders might be secondary to the visible hair disease. Psychopathological mechanisms need to be clarified but alexith

Autism and multiple pituitary deficiency.

We describe a 9-year-old boy who presented with abnormal development in language and social interaction. He also showed evidence of stereotyped behaviour, thus fulfilling all the criteria for an ICD-10 diagnosis of autism. This was associated with multiple pituitary deficiency. No case of autism associated with hypopituitarism has hitherto been reported. The authors discuss the evidence for linking the two conditions as opposed to accepting them as coincidental. In some studies of autism, anatomical and imaging studies have provided evidence of pathology in the limbic lobe. This lobe plays an

Functional neurolobectomy induced by controlled compression of the pituitary stalk.

Degeneration of magnocellular nerve terminals in the neurohypophysis was induced by compressing the pituitary stalk of anesthetized rats for 30 s using a triangle-shaped wire. Immediately after stalk compression (SC), rats exhibited markedly increased water intake characteristic of diabetes insipidus, followed by a triphasic pattern of fluid intake. In SC rats, arginine vasopressin (AVP) and oxytocin (OT) contents of the neurointermediate lobe (NIL) of the pituitary gland were significantly reduced to approximately 2.5% and approximately 10% of sham-operated controls, respectively. In contrast

Neuroendocrine abnormalities in patients with traumatic brain injury.

This article provides an overview of hypothalamic and pituitary alterations in brain trauma, including the incidence of hypothalamic-pituitary damage, injury mechanisms, features of the hypothalamic-pituitary defects, and major hypothalamic-pituitary disturbances in brain trauma. While hypothalamic-pituitary lesions have been commonly described at postmortem examination, only a limited number of clinical cases of traumatic hypothalamic-pituitary dysfunction have been reported, probably because head injury of sufficient severity to cause hypothalamic and pituitary damage usually leads to early

Effect of codeine and oxazepam on afternoon cortisol secretion in men.

The objective of this study was to investigate the effects of oral codeine and oxazepam on afternoon cortisol secretion. Nine subjects received either oxazepam (30 mg) or codeine (30 mg) or placebo at 1700h on separate days in a counterbalanced design; the subjects were not aware of the sequence. Blood samples were collected with an indwelling intravenous catheter at 30-min intervals from 1500h to 1630h. Codeine, but not oxazepam, suppressed cortisol secretion. The trend of the declining cortisol values following codeine was significantly linear. These results are consistent with other evidenc

Comparison of the duration of action of nalmefene and naloxone on the hypothalamic-pituitary axis of the rhesus monkey.

The duration of action of nalmefene, a relatively new opiate antagonist, on the hypothalamic-pituitary axis of the rhesus monkey was compared to that exhibited by naloxone. Ovariectomized monkeys (n = 5) were pretreated with 10 mg nalmefene, 10 mg naloxone or an equivalent volume of saline 12, 24, or 48 h prior to the administration of 10 mg morphine. Blood samples were collected by venipuncture 0, 1, 2, 3 and 4 h after injection of morphine and were assayed for luteinizing hormone (LH) and prolactin (PRL). Duration of action of these two opiate antagonists was estimated from their ability to

Thyroliberin (TRH) induced growth hormone (GH) release: test of maturation of hypothalamo-pituitary axis in postnatal rat.

Small doses of TRH elicit GH release from rat pituitary provided the pituitary lacks CNS influences (e.g. in vitro or in ectopic pituitaries). We have found previously that hypothalamic regulation of TSH secretion matures between the 5th and 12th postnatal day. Therefore the possibility of TRH induced GH release was investigated in neonatal rats. GH concentration was measured by RIA at 10 min after s.c. TRH administration (4.1 pmol g-1 BW) into female Wistar rats at the age 3, 9, 12, 15, 21, 30 and 100 days. Control animals were injected the same volume (1 microliter g-1 BW) of saline. Serum G
