Orthopaedic conditions in the newborn.

The occasional consultation on a neonate can be unfamiliar territory for many orthopaedic surgeons. Just as children are not little adults, … to identify and institute proper treatment for early developmental dysplasia of the hip, congenital knee dislocation, and torticollis. Clavicular pseudarthrosis and periosteal reactions may be discovered on radiographic examination. A basic understanding of the relevant conditions will help the orthopaedist with the initial diagnosis and management of orthopaedic …

[Post-traumatic knee joint arthroscopy in children and adolescents].

In a retrospective study we analyzed the arthroscopic findings of 166 knee-arthroscopies performed in children and young people up to the age of 16 years. A sports related trauma was present in 54%, a traffic injury in 15% and in 11% an other casualty. In 4% the precise injury was not sure and 16% did not refer any injury. Among the sports injuries (n = 90) ski injuries were the most frequent with 30% of all, followed by gymnastic injuries in 28%, soccer in 25% and other indoor sport injuries in 12%. 57% of all arthroscopies presented a hemarthrosis. The main finding of all arthroscopies was a

Change in joint space width: hyaline articular cartilage loss or alteration in meniscus?

OBJECTIVE: To explore the relative contribution of hyaline cartilage morphologic features and the meniscus to the radiographic joint space. METHODS: The Boston Osteoarthritis of the Knee Study is a natural history study of symptomatic knee osteoarthritis (OA). Baseline and 30-month followup assessments included knee magnetic resonance imaging (MRI) and fluoroscopically positioned weight-bearing knee radiographs. Cartilage and meniscal degeneration were scored on MRI in the medial and lateral tibiofemoral joints using a semiquantitative grading system. Meniscal position was measured to the nea

Total hip arthroplasty using a cementless dual-mobility cup provides increased stability and favorable gait parameters at five years follow-up.

BACKGROUND: Rates of dislocation following primary total hip arthroplasty (THA) vary from 0.5 to 10%. Dual-mobility cups in THA demonstrate increased stability. Clinical outcomes following THA with dual-mobility cups have been reported, but gait has not been assessed. Therefore we performed a retrospective case control study to answer: (1) is gait better in patients following THA with a dual-mobility cup than in frail, elderly patients of the same age? (2) Are clinical outcomes better in patients following THA with a dual-mobility cup than in frail, elderly patients? (3) What is the dislocatio

How Does Isolated Medial Patellofemoral Ligament Reconstruction Influence Patellar Height?

BACKGROUND: Reconstruction of the medial patellofemoral ligament (MPFL) is the gold standard treatment for recurrent patellar dislocation. Patella alta has been reported in about half of patients with recurrent patellofemoral instability. HYPOTHESIS: MPFL reconstruction (MPFLr) has a beneficial role in the correction of patellar height in patients with mild patella alta (Caton-Deschamps index [CDI] between 1.20 and 1.40). STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Skeletally mature patients, with no history of previous or concomitant knee surgical procedures, who underwent is

Hinged total knee arthroplasty for fracture cases: Retrospective study of 52 patients with a mean follow-up of 5 years.

INTRODUCTION: Hinged total knee arthroplasty (hTKA) is one of the treatment options available for complex fractures around an intact knee or an existing implant. The primary objective of this multicenter study was to evaluate the medium-term outcomes of hTKA for fractures. The secondary objective was to analyze the complications and risk factors based on fracture type, predisposition and time to surgery. We hypothesized that outcomes would be satisfactory, despite the context, and comparable to published data although impacted by significant mortality and morbidity. MATERIALS AND METHODS: Wit

Initial conservative treatment of osteochondral fracture of the patella following first-time patellar dislocation.

BACKGROUND: There has been no gold standard of the initial treatment strategy for acute patellar dislocation (APD) with osteochondral fracture (OCF). Hence the study aim is firstly, to review and compare clinical outcomes of patients who underwent conservative treatment for APD with or without OCF. Secondly, to characterize the location and size of fracture fragment. METHODS: Sixty-nine consecutive patients who were retrospectively evaluated after first-time APD over a 2- year period were divided into two groups (group 1 (n = 24): APD with OCF and group 2 (n = 45): APD only). [Orthopaedic examination of 1 500 infants in maternity (author's transl)].

1 500 infants have been examined by two orthopaedic surgeons within forty eight hours. The examination of the feet has showed that the usual position in utero was the dorsiflexion. A new classification of deformities of the feet is proposed. The examination of the knees has permitted to discover a rotatory instability between the femur and the tibia in 2% of the infants. Some of them were actual dislocations. During the examination of the hips, it has been researched an instability; the passive abduction, the tension of the adductors and an abduction contracture have been analysed. Some instab

[Arthroscopy of the knee joint].

The clinical usefulness of arthroscopies of the knee joint has been evaluated on the basis of 282 examinations. The procedures were performed under general anesthesia with sterility precautions as for arthrotomy. In nearly all cases, direct observation of articular surfaces, synovial membranes and the menisci of the ventral two thirds of the joint was possible. The dorsal recessus, the posterior third of medial meniscus, the posterior cruciate and Baker's cyst could not be inspected. The diagnostic accuracy of arthroscopy was compared with that of double contrast arthrography. The results are

Tibiofemoral rotation is related to differences in the lateral femoral condyle configuration in both normal subjects and women with hip dysplasia: a three-dimensional analysis.

BACKGROUND: We assessed the morphological differences in the knee joint related to knee rotation angle in patients with hip dysplasia. These results may explain the anatomy of the knee in patients with hip dysplasia and aid in planning knee surgery. METHODS: We enrolled 73 women (146 legs, 35.6 ± 9.0 years) with bilateral hip dysplasia and 45 healthy women (90 legs, 49.0 ± 18.9 years) without lumbago, knee symptoms, or osteoarthritic findings of the knee or spine on plain radiographs. We examined the parameters affecting knee rotation angle, such as the condylar twist angle and femoral condyl

The Efficacy of Continuous Adductor Canal Block Oblique Approach to Postoperative Analgesia After Total Knee Arthroplasty: A Randomized, Double-Blinded, Controlled Clinical Trial.

BACKGROUND: Continuous adductor canal block (CACB) is a highly effective analgesic technique in total knee arthroplasty. The brachy axis approach (BA) is currently the common method used for CACB. Although CACB for BA offers advantages such as easy localization and operation due to clear anatomy, it has limitations such as short catheter placement and a high dislocation rate. These limitations can result in insufficient postoperative analgesia and impact patient rehabilitation. The perineural catheters placed parallel for CACB increases the difficulty of operation and the risk of puncture inju
