UNLABELLED: High-energy injuries can result in complete or partial loss of the talus. Ipsilateral fractures to the lower limb increase the complexity of surgical management, and treatment is guided by previous case reports of similar injuries. A case of complex lower-extremity trauma with extruded and missing talar body and ipsilateral type IIIB open tibia fracture is presented. Surgical limb reconstruction and salvage was performed successfully with a single orthopaedic implant in a manner not described previously in the literature. The purpose of this case report is to present the novel use

OBJECT Results of radial nerve grafting are largely unknown for lesions of the radial nerve that occur proximal to the humerus, including those within the posterior cord. METHODS The authors describe 13 patients with proximal radial nerve injuries who were surgically treated and then followed for at least 24 months. The patients' average age was 26 years and the average time between accident and surgery was 6 months. Sural nerve graft length averaged 12 cm. Recovery was scored according to the British Medical Research Council (BMRC) scale, which ranges from M0 to M5 (normal muscle strength). R

A 45-year-old motorcyclist was hit by a car and presented to the emergency department with minor neck pain and radiating pain into both arms. Routine radiography did not show any pathology. Additional CT and MRI images revealed an anterolisthesis of Cvii-Thi with a bilateral facet dislocation and myelum compression.

A patient with traumatic thoracic injury and hypovolemic shock is presented to stress important differences in preoperative and postoperative aortic diameters. The patient had a blood pressure of 80/40 mm Hg. A diagnostic computed tomography angiography revealed a rupture of the thoracic aorta, and a thoracic endograft was sized based on these data. However, the postoperative computed tomography angiography (Riva-Rocci, 164/70 mm Hg) showed an increase in aortic diameters of about 30% at multiple levels. In this patient, with rupture of the thoracic aorta and hypovolemia, the aortic diameter w

Fasciotomies performed for compartment syndrome and ischemic vascular disease often requires closure in 2 to 4 weeks by skin graft. This leaves the patient with an unsightly scar and a limb with reduced strength. The use of vacuum-assisted closure (VAC) and hyperbaric oxygen therapy (HBOT) quickly reduce the edema and permit earlier closure with adjacent skin. A study of three trauma patients with compartment syndrome, fasciotomies, and the use of the VAC and HBOT to close the fasciotomy wounds with adjacent skin is presented. The pathophysiology of compartment syndrome and ischemia-reperfusio

OBJECTIVE: Road traffic injuries (RTIs) have been identified as a major public health problem worldwide. Motorcyclists are one of the most vulnerable groups in terms of injuries and fatalities in road traffic crashes. Not using a helmet and head and neck injuries are the main causes of death and disability among them. This study explores barriers and facilitators of helmet use among motorcyclists.
METHODS: A qualitative approach to conventional content analysis was employed in this research. The purposive sampling method, with maximum diversity, was used to select participants among motorcycl

OBJECT: Action motorsports, including motocross, have been gaining popularity among children and adolescents, raising concerns for increased risk of concussions in participating youth. The authors undertook this study to test the following hypotheses: 1) that there is a high rate of concussion symptoms associated with a number of preventable or adjustable risk factors, and 2) that a high percentage of these symptoms are not be reported to adults and medical personnel.
METHODS: The authors identified all motocross riders under the age of 18 at a regional racetrack during the riding season betw

BACKGROUND: Motorcycle crashes (MCCs) constitute a disproportionately high number of road accidents that result in mortality and injury, compared with other motor vehicle collisions. Distribution and characteristics of upper extremity injuries sustained by motorcyclists and their implications are not well established. We sought to determine the epidemiology of upper extremity injuries in motorcyclists and the independent effects of the injuries on mortality and need for rehabilitative services.
METHODS: All motorcyclist admissions at our Level I trauma center from 2006 through 2010 were retro

PURPOSE: Sporting activities that involve repetitive stress to muscle compartments can elicit chronic exertional compartment syndrome. Its occurrence in the lower leg muscle compartments is most common, but other locations are less well known and the pathophysiology is not completely understood. In motocross racers, chronic exertional compartment syndrome can occur in the muscles of the lower arm. Currently, the only accepted treatment of correctly diagnosed chronic exertional compartment syndrome is surgical release of the fascia, which successfully relieves pain and allows patients to return

Although millions of electric bikes (E-bikes) operate in China and many associated deaths and injuries have been reported, E-bikers' on-road practices are poorly characterized and few direct observational studies have been performed. This study aims to describe riding behaviors among E-bikers and to investigate factors influencing these practices to inform injury prevention. In March 2012, a cross-sectional observational study was conducted at 14 randomly selected intersections in Suzhou during a 7-day period. A pro-forma observation checklist was used to collect data on road riding practice.

BACKGROUND: The number of dirt bike injuries in children in the United States is increasing and poses a public health problem.
OBJECTIVE: The purpose of our study was to identify the imaging patterns of dirt bike injuries in children and associations with morbidity and mortality.
METHODS: The study included 85 children (83 boys, 2 girls) <18 year of age (mean age 12.3 years, standard deviation 3 years) with dirt bike injury treated at a tertiary care pediatric hospital. Imaging studies and hospital medical records were reviewed. Outcomes were classified into the following categories: short-t

The authors report a unique case of a broken motorbike handle that presented as a bilateral orbital foreign body. A 30-year-old male sustained an injury to the right side of his face when he skidded from his motorbike while riding. He had bilateral sudden loss of vision and presented to emergency services with bilateral proptosis, motility restriction, and a right lower eyelid laceration. CT scan revealed a 7-cm-long motorbike handle lodged in the retrobulbar space of both orbits, close to the cribriform plate. Left lateral orbitotomy was performed to remove the foreign body; the right eye reg

A two-part study was undertaken to investigate warning signal detection by motorcyclists. First the acoustic environment was established for both the helmeted and bareheaded rider between speeds of 0 and 100 mph. For the helmeted rider it was found that below 20 mph vehicle noise was dominant and between 20 and 40 mph there was a variable contribution from both vehicle and wind noise. At 40 mph wind noise became the dominant sound source and increased linearly with the log10 of speed from 90 dB(A) to reach 112 dB(A) at 100 mph. Sound levels were consistently 18 dB(A) greater for the bareheaded

Objective: Few studies have investigated U.S. emergency department (ED) moped-related injuries. Our objective was to determine the characteristics of moped crashes among adult riders and to identify associations between key variables. Methods: Data were obtained from the National Electronic Injury Surveillance System (NEISS) for 2002-2014. Descriptive and comparative analyses of the identified cases were performed. Results: Based on a probability sample of 2,453 cases, an estimated 108,229 U.S. adult moped-related ED visits occurred during the study period. Injuries increased 2.7-fold over time.
