Radial club-hand deformity. Long-term follow-up.

In 1970, the results of the treatment of fourteen patients with radial club hand were reported. Ten years later, we re-examined ten of these patients. There was little change in the measurements of ulnar bowing or ulnar length over the intervening decade, and the treated patients retained the cosmetic and functional improvement previously described. They also adapted satisfactorily to performing activities of daily living.

Reconstruction of shoulder abduction and external rotation with latissimus dorsi and teres major transfer in obstetric brachial plexus palsy.

OBJECTIVES: We evaluated the results of latissimus dorsi and teres major tendon transfer to the rotator cuff together with musculotendinous lengthening of the subscapularis and/or pectoralis major muscles in patients with internal rotation contracture and decreased external rotation and abduction secondary to obstetrical brachial plexus palsy. METHODS: Thirty patients (18 boys, 12 girls; mean age 9 years; range 4 to 15 years) with internal rotation contracture and loss of external rotation and abduction of the shoulder secondary to obstetrical brachial plexus palsy underwent transfer of the l…

Outcomes of free adipofascial flaps combined with tenolysis in scarred beds.

PURPOSE: To review our outcomes of transferring vascularized free adipofascial flaps used to change the local tissue conditions at the time of tenolysis in adhesion-prone beds. METHODS: Eleven free adipofascial flaps were transplanted in 10 patients after tenolysis on the forearm (3 cases), the dorsum of the hand (5 cases), or the dorsum of the proximal phalanx (3 cases). All recipient areas had badly scarred beds, 7 of which had previously failed tenolyses. In addition to tenolysis (10) or the insertion of bridging tendon grafts (1), arthrolysis of several involved joints and bone fixation f

Flexor carpi radialis tendinitis. Part II: Results of operative treatment.

We retrospectively reviewed the results of decompression of the flexor carpi radialis tunnel in ten patients (six women and four men) who ha … don in one patient. Additional procedures, such as excision of a ganglion, removal of an exostosis, tendon transfer, or application of a fat graft, were performed in seven patients. Nine of the ten patients had relief of the symptoms and were able to resume their preoperative employment and leisure activities. One patient continued to be symptomatic.(ABSTRACT TRU …

Upper limb reconstruction in quadriplegia: functional assessment and proposed treatment modifications.

The functional results following reconstruction of key grip (40 limbs in 30 patients) and active elbow extension (14 limbs in 9 patients) in … gers to the proximal phalanx. Other procedures include modification of the methods of attachment of tendon transfers to shorten convalescence following reconstruction of active elbow extension. (3) Postoperative care--postoperative problems develop insidiously. Frequent reassessment is mandatory.

Alternative treatment for varus instability of the hallux interphalangeal joint: A case report.

Forefoot injuries are commonly caused by sport-related activities and are often around the metatarsophalangeal joints. The hallux interphalangeal (IP) joint is anatomically stable; therefore, injuries to this joint are considered to be rare compared with those to the metatarsophalangeal joint. Instability of the hallux IP joint has rarely been reported in barefoot contact sports, and its treatment has not been sufficiently explored. This study investigated chronic varus instability of the hallux IP joint. We performed a surgical reconstruction owing to conservative treatment failure. A good sur

Posterior shoulder dislocation: systematic review and treatment algorithm.

PURPOSE: Posterior shoulder dislocations (PSDs) comprise a small subset of shoulder dislocations, and there are few evidence-based treatment protocols and no actual algorithm for the treatment of PSDs available in the literature. This article provides a systematic review of the literature, as well as an overview of clinical and radiologic diagnostic techniques, and presents an algorithm for treatment of PSDs, including minimally invasive treatment options. METHODS: For a systematic review of current literature, a systematic search was performed in the Medline and Cochrane databases. Journal a

Biomechanical evaluation of arthroscopic rotator cuff repairs over time.

PURPOSE: The aim of this study was to assess the contact pressure, force, and area over time for 4 common arthroscopic rotator cuff repair techniques. METHODS: The transosseous-equivalent, single-row, triangle double-row, and suture-chain transosseous repair techniques were used to repair a full-thickness tear of the supraspinatus in 16 cadaveric shoulders. Continuous data points were collected immediately after repair and for 160 minutes at set time intervals by use of a custom thin film pressure sensor. RESULTS: Each of the 4 rotator cuff repair techniques showed decreased contact force, p

New radiographic parameters assessing forefoot abduction in the adult acquired flatfoot deformity.

BACKGROUND: Stage II flatfoot secondary to posterior tibial tendon insufficiency may be subclassified into mild (IIa) and severe (IIb) deformity based on the degree of talonavicular abduction. Current assessment of this abduction is difficult. We hypothesized that two new anteroposterior radiographic parameters, the lateral talonavicular incongruency angle (IA) and incongruency distance (ID) would demonstrate good reliability, correlate with current abduction parameters, and differ in IIb deformity, IIa deformity, and controls. MATERIALS AND METHODS: Preoperative radiographs for consecutive p

Transposition of the bicipital tuberosity for treatment of fixed supination contracture in obstetric brachial plexus lesions.

In nine patients with obstetric brachial plexus lesions (Klumpke type), an impingement of the bicipital tuberosity on the ulna was the main cause for the forearm and hand to be fixed in supination. A surgical technique using reinsertion of the biceps tendon on the bicipital tuberosity is described in detail. It has substantially improved all patients. After a mean follow-up of 29.4 months the hand was in a more functional position than preoperatively in all patients. In seven cases pronation could be increased by contraction of the biceps muscle. By relaxing the biceps muscle and by contractio

[Correction of post-traumatic adduction insufficiency of the small finger by transposition of the tendon of the extensor indicis muscle].

The m. interosseus palmaris III adducts the small finger. Injuries of this muscle and lesions of the ulnar nerve lead to the small finger assuming an abducted posture. Tendon transposition of the extensor indicis to correct the abducted small finger in patients with injuries and ulnar nerve dysfunction is described.

Reoperation in treatment of clubfoot.

We evaluated 118 operations (including 57 reoperations) on clubfeet. We concluded that relapse is most often the consequence of insufficient primary surgery, and the less sufficient the intervention, the more severe the relapse or residual deformity. The severity of deformity is determined and shown, first of all, by the position of calcaneum. In milder cases, soft tissue operation can prove useful, whereas more severe cases require bony intervention as well. The second, rarer cause of relapse was the fact that the foot was originally stiff and nonreducible. These feet may need multiple reoper

Anterior transfer of the long toe flexors for the treatment of spastic equinovarus and equinus foot in cerebral palsy.

Anterior transfer of the long toe flexors combined with the Baker procedure, or heel cord lengthening was used to correct equinus foot deformity in 21 patients with cerebral palsy. In more than two-thirds of the cases, heel-to-ball gait became possible. There were no significant recurrences that required re-operation. There were no complications caused by loss of original function in the long toe flexors even in younger patients. Excellent and good results were seen in 57.1% of the patients according to the overall results.

Surgical treatment of the symptomatic accessory navicular.

The accessory navicular is occasionally the source of pain and local tenderness over the instep. If conservative measures fail, surgical treatment may be required and the results of 62 operations to one or both feet in 47 patients are reported. Twenty-six patients were treated by the Kidner operation, in which the main insertion of the tibialis posterior is re-routed; in the remaining 21 the ossicle was merely excised. Excision was as effective as the Kidner technique, provided that the medial surface of the main navicular bone was contoured to prevent any residual prominence. Both procedures

The "small patella" syndrome.

Twelve closely related members of one family were found to have small or absent patellae. Seven of these patients also had abnormalities of the pelvic girdle and upper femora. There was a resemblance to the nail--patella syndrome in the patellar abnormalities but not in the associated conditions, and it would therefore appear that the syndrome described is a new variety of congenital abnormality of the knee.

Treatment of chronic Achilles tendon rupture by shortening suture and free sural triceps aponeurosis graft.

INTRODUCTION: The Bosworth technique is old but still widely used. It involves problems of precisely determining the length of the Achilles tendon and of a volume effect in the turndown area. HYPOTHESIS: A new reconstruction technique is assessed, based on free sural triceps aponeurosis transfer without turndown, associated to tendon shortening suture. MATERIALS AND METHODS: Twenty-three patients were assessed by AOFAS score and clinical examination (plus MRI in 14 cases) at a mean 24.5 months' follow-up. Mean age was 52.1 years. Mean pre-operative AOFAS score was 63.6/100. RESULTS: Mean po
