Late sequelae of whiplash injury with dissection of cervical arteries.

BACKGROUND/AIMS: The objective of our study was to estimate the incidence of posttraumatic dissections of cervical arteries in patients with whiplash injury acquired in a car accident. METHODS AND PATIENTS: We performed a retrospective analysis of medical records of 500 patients with whiplash injury acquired in car accidents between 1996 and 2005 and searched for dissections of cervical arteries occurring within 12 months after injury. RESULTS: Eight cases of cervical arterial dissection occurred within 12 months following whiplash injury. In 7 cases (87.5%), the dissection was complicated b

Reversal of locked-in syndrome with anticoagulation, induced hypertension, and intravenous t-PA.

INTRODUCTION: Widespread use of intravenous tissue plasminogen activator (t-PA) for acute ischemic stroke is limited by multiple contraindications to its use. CASE REPORT: This article describes a patient with stuttering symptoms of pontine ischemia caused by vertebrobasilar dissection who suddenly deteriorated into a locked-in state 32 hours after symptom onset. The quadriparesis was successfully reversed within 3 hours of onset with the combination of pharmacologically induced hypertension, anticoagulation, and intravenous t-PA. DISCUSSION: Even in the face of numerous contraindications (i

[Utility of CT during aortography in depiction of the artery of Adamkiewicz and its origin: preliminary report].

The purpose of this study was to evaluate the feasibility of using CT during aortography with multidetector-row CT (MDCT) to depict the continuity of the Adamkiewicz artery and its origin. We performed CT during aortography in 4 patients with thoracoabdominal or descending aortic aneurysm, using multi-detector-row CT and portable DSA. The continuity of the Adamkiewicz artery and its origin were evaluated on multiplanar and curved planar reformation images and on cine-mode displays. In all patients, the Adamkiewicz artery was clearly visualized. The continuity of the Adamkiewicz artery and its

Brain stem compression by a giant vertebrobasilar aneurysm mimicking seronegative myasthenia.

A patient is described with a vertebrobasilar aneurysm who was erroneously thought to have myasthenia gravis on the basis of the clinical presentation and investigations, which were interpreted as supportive of a disorder of the neuromuscular junction. Despite the correct diagnosis being made at a late stage the patient made a full recovery after radiological intervention.

[Fatal outcome during physiotherapy (Vojta's method) in a 3-month old infant. Case report and comments on manual therapy in children].

Detailed clinical and neuropathological report on a fatal incident during the first manual therapy according to Vojta conducted in a 3 months old baby: during forced active rotation and head retraction the baby suffered from a bleeding into the adventitia of both her vertebral arteries at the level of C1 prompting ischemia of the caudal brainstem with subarachnoid haemorrhage around. It has to be suggested that similar cases already have occurred but have not been reported yet. There might be a time lag between the performance of physiotherapy and the beginning of neurologic symptoms. The risk

Ex-situ identification of vertebral artery injuries from stab wounds through contrast-enhanced fluoroscopy and micro-CT.

Vertebral artery injuries (VAIs) are rarely observed in forensic practice and can result from either penetrating injuries or blunt force trauma to the neck. The identification of such injuries represents a pivotal challenge for forensic pathologists, due to the protected anatomical course of these vessels, particularly in their extracranial tract. Over the years, several approaches have been proposed to address this issue, including vessel gross examination through different dissection techniques, the water injection method and, more recently, post-mortem computed tomography angiography (PMCTA

Microsurgical revascularization of a symptomatic proximal vertebral artery: pilot experiences from a single center.

BACKGROUND: Vertebral artery stenosis and occlusion (VASO) is a high-risk factor for posterior circulation stroke. Post-stent restenosis and drug tolerance have facilitated the exploration of microsurgical vascular reconstruction. This study aims to evaluate the safety and efficacy of microsurgical reconstruction of the proximal VA. METHODS: Twenty-nine patients (25 men, aged 63.2 years) who had symptoms of posterior circulation ischemia underwent microsurgical revascularization for proximal VASO were retrospectively included in this study. Procedural complications and clinical and angiograph

Lateral medullary syndrome: Case report and review of literature.

Lateral medullary syndrome (LMS) or Wallenberg's syndrome is an uncommon and often underdiagnosed cause of posterior circulation stroke. Thrombosis, embolization, or dissection of vertebral or posterior inferior cerebellar artery (PICA) often results into LMS. The most pathognomonic symptoms of LMS includes pain and temperature deficits on ipsilateral facial side and contralateral side of rest of the body, ipsilateral ataxia, vertigo, nystagmus, dysphagia, hoarseness, hiccups and Horner's syndrome. We report a case of LMS in a 49-year-old Indian female with no known classical risk factors for

Spinal subarachnoid hemorrhage mimicking an acute abdomen.

A 39-year-old woman with a history of systemic lupus erythematosum developed an acute abdomen. An intestinal ischemia was suspected and an exploratory laparotomy was performed. No abnormalities were found during surgery and five days later the patient was transferred to our institution. On admission, the patient presented a distended abdomen and paraparesis. The spinal tap showed hemorrhagic CSF and the MRI a subacute subarachnoid hemorrhage (SAH) of the dorsal-lumbar spine. Two days later, the patient suffered an episode of sudden headache. The CT scan revealed an acute SAH at the posterior f

Anatomical study of diaphragmatic crura and segmental vessels for lumbar spinal surgery.

STUDY DESIGN: Observational cadaveric anatomical study. PURPOSE: To investigate the anatomical relationship between the segmental vessels and the diaphragmatic crus, aiming to reduce the risk of segmental vessel injury. OVERVIEW OF LITERATURE: Total en bloc spondylectomy and lateral lumbar interbody fusion are advanced surgical procedures associated with segmental vessel injury. Previous research suggests these injuries may arise from anatomical variations where segmental vessels intersect intervertebral discs. At upper lumbar levels, the diaphragmatic crus lies adjacent to the vertebral bod

Incidence of branching patterns variations of the arch in aortic dissection in Chinese patients.

Several authors have described anatomic variations of the aortic arch in 13% to 20% of the patients who do not have aortic disease. However, few studies have evaluated these patterns in the thoracic aortic dissection (TAD). In the authors' knowledge, this is the first survey that specifically investigates the frequency of these variations in a broad, nonselected group of Chinese patients with aortic dissection. Furthermore, it compares this group with a group of patients without aortic disease. The objective of this study was to define the variation frequency of the aortic arch branches pattern

Vertebrobasilar junction fenestration with dumbbell-shaped aneurysms formation: computational fluid dynamics analysis.

BACKGROUND: We report 8 rare cases of paired ANs involving fenestrated vertebrobasilar junction and demonstrate the flow patterns of the paired ANs by qualitative CFD analysis in 5 cases. METHODS: Two-dimensional and 3-dimensional angiographic features of 8 cases were reviewed. Nine patient-specific geometries of CFD models in 5 cases were created for flow analysis. RESULTS: All 8 cases had 2 ANs, one large and the other small, projecting to the opposite sides at the proximal end of fenestrated vertebrobasilar junction. The different angiographic findings between right VA and left VA suggest

Cerebral blood flow after hybrid distal hemiarch repair.

OBJECTIVES: Aortic arch disease can be treated with hybrid repair (extra-anatomic bypass plus placement of aortic endoprostheses), but there is controversy about whether a bypass from one relatively small vessel will provide adequate blood flow to the entire brachiocephalic system. We, therefore, compared flow volumes before and after hybrid repair. METHODS: We reviewed the records of 16 patients who underwent a hybrid distal hemiarch repair between October 2010 and May 2012. The procedure consisted of debranching of the left subclavian and left common carotid arteries, creation of a bypass t

Recurrent vertebrobasilar strokes and transient-ischemic attacks with challenging workup: Case report.

Detecting the stroke etiology in young patients can be challenging. Among others, determining causality between ischemic stroke and patent foramen ovale (PFO) remains a complicated task for stroke neurologists, given the relatively high prevalence of PFOs. Thorough diagnostic workup to identify incidental vascular risk factors and rare embolic sources is crucial to avoid premature PFO closure suggesting successful secondary stroke prevention. In this paper, we report on a 38-year-old patient with recurrent vertebrobasilar territory, especially right posterior inferior cerebellar artery (PICA)

Increased Internal Carotid Artery Tortuosity is a Risk Factor for Spontaneous Cervicocerebral Artery Dissection.

OBJECTIVE: Spontaneous cervicocerebral artery dissection (sCCD) is an important cause of ischaemic stroke that often occurs in young and middle aged patients. The purpose of this study was to investigate the correlation between tortuosity of the carotid artery and sCCD. METHODS: Patients with confirmed sCCD who underwent computed tomography angiography (CTA) were reviewed retrospectively. Age and sex matched patients having CTA were used as controls. The tortuosity indices of the cervical arteries were measured from the CTA images. The carotid siphon and the extracranial internal carotid arte

Stenting and secondary coiling of intracranial internal carotid artery aneurysm: technical case report.

OBJECTIVE AND IMPORTANCE: Endovascular stents have been successfully used in the treatment of fusiform and dissecting aneurysms of the peripheral circulation and extracranial carotid and vertebral arteries. Technical limitations related to the inability to navigate the stent and the delivery system through tortuous vascular segments has limited their application with intracranial lesions. Availability of new flexible and pliable stent systems might overcome these difficulties. CLINICAL PRESENTATION: A 49-year-old woman presented with a dissecting pseudoaneurysm of the horizontal portion of th
