Usefulness of adenosine for arrhythmias in infants and children.

Adenosine was administered to 25 infants and children (11 patients after presenting with a sustained arrhythmia, and 14 during a diagnostic electrophysiologic study) to determine its electrophysiologic effects. Adenosine was given as an intravenous bolus (starting dose 37.5 micrograms/kg, and increased by 37.5 micrograms/kg increments until an effect was seen). Adenosine caused tachycardia termination or transient increased atrioventricular (AV) block in all 25 patients. Seven patients had tachycardia requiring only the atria for perpetuation and developed increased AV nodal block (minimum eff

Normal variations and pathologic changes in structure of the cardiac conduction system and their functional significance.

The rarity with which the cardiac conduction system is carefully examined in cases of sudden unexpected cardiac death is deplorable. Whatever the reasons for this failure, it is clearly one of the major lost opportunities for improving our understanding of a major national heart problem. To illustrate some of the morphologic changes found in such cases, two categories are discussed: normal variations (which may themselves promote electrical instability) and changes due to disease. During normal postnatal morphogenesis of the atrioventricular (AV) node and His bundle, there are a variety of der

Kawasaki disease.

Kawasaki disease, a pathologic syndrome known to occur in children, was first described in 1967 as mucocutaneous lymph node syndrome by Kawasaki. The disease occurs chiefly in infants under 4 years of age, presenting with symptoms similar to scarlet fever or Stevens-Johnson syndrome. The changes are found at postmortem and consist of multiple aneurysms and thrombosis, which occur predominantly in the coronary arteries and are responsible for sudden death in most cases. Kawasaki disease is a systemic, acute inflammatory disease entity and in the early stages shows diffuse, necrotizing necrosis.

Electrophysiological effects of Org 7797 in the closed-chest anaesthetized dog.

1. The intravenous electrophysiological effects of a new antifibrillatory agent, Org 7797, were studied in closed chest anaesthetized dogs. Effects of fast sodium and slow calcium-mediated action potentials were also examined in guinea-pig isolated papillary muscle. 2. The major effects of a known antifibrillatory dose of Org 7797 (0.5 mg kg-1) were a protracted slowing of AV nodal conduction (for at least 20 min) and prolongation of the AV nodal functional refractory period. Conduction in the atria and His-Purkinje system (reflected by the St-A and HV intervals) were not significantly modifie

Predominant Right Ventricular Dysfunction in Cardiac Sarcoidosis.

BACKGROUND: Clinically significant cardiac sarcoidosis is rare, seen in 5% of patients with sarcoidosis. Predominant right ventricular (RV) dysfunction is seen even less often. CASE SUMMARY: A 59-year-old man presented to the emergency department from his outpatient cardiologist's office with high-grade atrioventricular block and signs of right-sided heart failure. Transthoracic echocardiogram showed normal left ventricular systolic function, with severe RV enlargement and dysfunction. Right heart catheterization revealed mild postcapillary pulmonary hypertension and disproportionate RV dysfu

Evaluating and managing bradycardia.

Bradycardia is a commonly observed arrhythmia and a frequent occasion for cardiac consultation. Defined as a heart rate of less than 50-60 bpm, bradycardia can be observed as a normal phenomenon in young athletic individuals, and in patients as part of normal aging or disease (Table 1). Pathology that produces bradycardia may occur within the sinus node, atrioventricular (AV) nodal tissue, and the specialized His-Purkinje conduction system. Given the overlap of heart rate ranges with non-pathologic changes, assessment of symptoms is a critical component in the evaluation and management of brad

Mucopolysaccharidosis VII: postmortem biochemical and pathological findings in a young adult with beta-glucuronidase deficiency.

The postmortem biochemical and pathological findings in the first patient reported with mucopolysaccharidosis VII are described. Clinical, radiographic, and biochemical features of this 19-yr-old black man were initially reported in 1973 when, at age 2 1/2 yr his enzymatic defect, deficiency of beta-glucuronidase, was identified. The autopsy findings are now described with biochemical data further characterizing the enzyme deficiency and resultant glycosaminoglycan accumulation. He had dysostosis multiplex and extensive cardiovascular lesions including arterial stenosis, and marked fibrous thi

The coronary arteries in Marfan's syndrome. A morphologic study.

The coronary arteries of five hearts of patients known to have had Marfan's syndrome were studied microscopically. In four of the five cases … f the sinus nodal artery and once in the musculoelastic layer of a coronary artery that reached the atrioventricular node from the ventricular septum by perforating the anulus fibrosus. However, in contrast to findings in a previous report, the intramural coronary arteries were otherwise unaffected. The clinical significance of these data is as yet unknown particularly …

Long PR Interval Leads to Atrial Remodeling, Fibrosis and Inducible Atrial Fibrillation in a Swine Model.

BACKGROUND: Pacing algorithms have been developed to minimize the risk of pacing-induced ventricular dysfunction at the expense of a prolonged PR interval. However, a long PR interval has been associated with incident atrial fibrillation (AF), and the impact of a prolonged PR interval on atrial remodeling remains unclear. OBJECTIVES: This study investigated the impact of a prolonged PR interval on atrial remodeling in swine. METHODS: In total, 24 Yucatan miniswine were divided into 3 groups: control (AAI 90 beats/min, n = 8), standard (160/180 ms, n = 8), and long (350 ms, n = 8) atrioventri

Superior approach from the pocket for atrioventricular junction ablation performed at the time of conduction system pacing implantation.

BACKGROUND: Conduction system pacing (CSP) and atrioventricular junction ablation (AVJA) improve outcomes in patients with symptomatic, refractory atrial fibrillation (AF). Superior approach (SA) from the pocket via axillary or subclavian vein has been proposed as an alternative to the conventional femoral venous access (FA) to perform AVJA. OBJECTIVE: To assess the feasibility and safety of SA for AVJA performed simultaneously with CSP, and to compare this approach with FA. METHODS: A prospective, observational study, enrolling consecutive patients with symptomatic, refractory AF undergoing

[Inadequate, inappropriate sinus-node tachycardia. An old cardiac arrhythmia in a new perspective].

Introduction: The inadequate, inappropriate sinus-node tachycardia is not a rare clinical syndrome, defined as a non-paroxysmal, increased sinus-rate at rest, and/or inadequate response to physical and/or emotional stress, and palpitations. Aim: The aim of this study was to describe our experiences with the investigations of our inappropriate sinus-node tachycardia patients. Method: In the last years, 104 patients (92 women, 12 men, mean age: 31 ± 10 years) were treated with this cardiac arrhythmia entity. All patients underwent 12 leads ECG, chest X-ray, echocardiography, Holter-monitoring an

Leadless pacemaker implantation and concurrent atrioventricular junction ablation in patients with atrial fibrillation.

BACKGROUND: Atrioventricular junctional (AVJ) ablation and pacemaker implantation are indicated when pharmacotherapy fails to achieve adequate rate control in atrial fibrillation (AF). The purpose of our study is to assess the feasibility and safety of concurrent Micra leadless transcatheter pacemaker implantation and AVJ ablation. METHODS: We retrospectively assessed patients who underwent Micra implantation and concurrent AVJ ablation at three institutions between August 2014 and March 2016. All patients and devices were followed at baseline and at 1, 3, 6, and 12 months postimplantion. RE

Atrioventricular conduction properties in patients with prolonged pauses undergoing ablation of longstanding persistent atrial fibrillation: do pauses during atrial fibrillation matter?

BACKGROUND: Atrioventricular (AV) conduction disturbances have often been considered as an etiology of prolonged pauses during atrial fibrillation (AF). We aimed to test whether there was a significant difference in the AV conduction properties between patients with and without clinically significant pauses who underwent ablation of longstanding persistent AF. METHODS: Ninety-nine patients undergoing ablation of longstanding persistent AF were divided into three groups according to the extent of pauses documented on the ambulatory electrocardiogram during AF; patients without pauses (n = 25),

Optimization techniques in cardiac resynchronization therapy.

Cardiac resynchronization therapy improves symptoms and cardiac function, as well as reduces mortality in patients with progressive congestive heart failure, reduced left ventricular ejection fraction and a left bundle branch block on the surface electrocardiogram. As many as 30% of patients fail to have an adequate response. The interplay between the atrioventricular delay and the contribution of a properly timed atrial contraction to ventricular filling along with a properly timed sequence of activation of the right and left ventricular is crucial to maximizing the benefits of cardiac resync

Surgical treatment of a giant cystic tumor of the atrioventricular nodal region.

Cystic tumor of the atrioventricular nodal region is a rare cardiac primary tumor that can cause heart blockage and sudden death. Antemortem diagnosis and successful excision of the atrioventricular nodal region are extremely rare. A 41-year-old woman who presented with dyspnea and palpitations is reported. Electrocardiography revealed third-degree atrioventricular block. Echocardiography showed a right atrial cystic mass attached to the interatrial septum. The patient underwent surgical excision of the mass. Histopathological findings were of a cystic tumor of the atrioventricular nodal regio

Rate-dependent AV nodal refractoriness: a new functional framework based on concurrent effects of basic and pretest cycle length.

The atrioventricular (AV) node filters atrial impulses. Underlying rate-dependent refractory properties are assessed with the effective (ERPN; longest nonconducted atrial cycle length) and functional (FRPN; shortest His bundle cycle) refractory period determined with premature protocols at different basic rates. Fast rates prolong ERPN and shorten FRPN, but these effects vary with subjects, age, and species. We propose that these opposite and variable effects reflect the net sum of concurrent cumulative and noncumulative effects associated with basic (BCL) and pretest cycle length (PTCL), resp

Transient heart block in a neonate associated with previously undiagnosed maternal anti-Ro/SSA and anti-La/SSB antibodies.

This report presents the case of an infant who was born with transient complete heart block. The cardiac rhythm converted to normal sinus rhythm within 12 hours of life. Following the diagnosis in this infant of congenital heart block, both the mother and the infant were tested for autoantibodies. Both were found to be strongly positive for anti-Ro/SSA antibodies. The infant was also weakly positive for the anti-La/SSB antibodies and her mother moderately positive for the same. Congenital heart block associated with these maternal antibodies is well documented in the literature; however, this
