[First experience in endonasal surgery using a new 3D-Chipendoscope].

BACKGROUND: Endoscopic surgery for treatment of nasal, paranasal and anterior skull base pathologies is an established treatment modality. Available rigid endoscopes with angled views provide a 2-dimensional view with restriction of depth perception. In this study we report about our first experience with a new 3D-Chipendoscope in surgery at the nose, paranasal sinuses und anterior skull base. MATERIAL AND METHODS: 30 patients were enrolled in this study with chronic rhinosinusitis, inverted papilloma, frontal sinus mucocele, frontal sinus osteoma, chronic dacryocystitis and pituitary adenoma

Surgical management of trigeminal neuralgia patients with recurrent or persistent pain despite three or more prior operations.

OBJECTIVE: To compare facial pain outcomes from different surgical techniques for patients with idiopathic trigeminal neuralgia (TN) who continue to have persistent or recurrent TN despite multiple operations. METHODS: Review of a prospective surgical database identified 62 TN patients with ongoing facial pain despite having undergone three or more prior operations (mean = 3.4) from July 1999 to March 2008. The mean patient age was 66.5 years and the mean pain duration was 11.8 years. Twenty-six patients (42%) underwent 33 additional procedures during the follow-up period. In total, 95 operat

Anterior clivectomy: surgical technique and clinical applications.

OBJECT: Midline clival lesions, whether involving the clivus or simply situated anterior to the brainstem, present a technical challenge for adequate exposure and safe resection. The authors describe, as a minimally invasive technique, an anterior clivectomy performed via an expanded transsphenoidal approach coupled with the use of a neuronavigation on mobile head and endoscopic-assisted technique. Wide and direct exposure, with the ability to resect extra- and intradural tumors, was achieved without mortality and with a low rate of complications. METHODS: Cadaveric dissections were performed

Multiple neurenteric cysts in the posterior fossa and cervical spinal canal--case report.

A 46-year-old woman presented with multiple neurenteric cysts in the posterior fossa and spinal canal. Neuroimaging demonstrated neurenteric cysts in the interspace between the left cerebellar hemisphere and vermis, the lateral side of the right cerebellar hemisphere, and the ventral side of the spinal cord at the C-2 and C-4 levels. Total resection of the paravermian cyst and partial removal of the spinal cyst at the C-4 level were performed. Histological examination showed the cyst wall consisted of single or multiple layers of columnar epithelial cells with secretory granules, with mucin se

Karyotype studies in 18 ependymomas with literature review of 107 cases.

Cytogenetic studies from 17 pediatric ependymomas and 1 ependymoblastoma are presented. Eight tumors had abnormal karyotypes. Another 107 published cases of cytogenetic analyses from pediatric and adult ependymomas or ependymoblastomas were reviewed. Of the total 125 tumors, 83 (66%) had abnormal karyotypes, of which 24 had a sole autosomal abnormality. Approximately one third had monosomy 22 (-22) or breakpoint 22q11-13, with a higher incidence in adult (56%) versus pediatric (28%) tumors. Structural abnormalities of chromosomes 1, 6, and 17, and numerical abnormalities of 7, 9, 12, and 20, i

Chiari I malformation redefined: clinical and radiographic findings for 364 symptomatic patients.

OBJECTIVE: Chiari malformations are regarded as a pathological continuum of hindbrain maldevelopments characterized by downward herniation of the cerebellar tonsils. The Chiari I malformation (CMI) is defined as tonsillar herniation of at least 3 to 5 mm below the foramen magnum. Increased detection of CMI has emphasized the need for more information regarding the clinical features of the disorder. METHODS: We examined a prospective cohort of 364 symptomatic patients. All patients underwent magnetic resonance imaging of the head and spine, and some were evaluated using CINE-magnetic resonance

Trigeminal neuralgia treated by microvascular decompression: a long-term follow-up study.

Since 1976, 133 patients treated by posterior fossa microvascular decompression (MVD) for trigeminal neuralgia (TN) have been followed-up prospectively to determine the incidence of recurrent TN. The follow-up period was between 6 months and 15 years, with 60 patients having been follow-up for more than 5 years. Of these patients, 71% have remained pain-free, while 29% have suffered a major or minor recurrence. Of patients who developed recurrent TN, 90% did so within 2 years of operation. No patients who were pain-free up to 5 years after operation subsequently developed recurrent TN. Recurre

[Results of surgery of acoustic neuroma via translabyrinth and suprapetrous approaches. Development of a classification by House and Brackmann based on facial function].

Two studies were conducted consecutively on two series of post-operative acoustic neurinoma patients. The first one included 104 patients over a period spanning from January 1982 through April 1986; the second one bore on 75 cases enrolled between October 1985 and April 1988. Post-operative complications, sequelae, and findings were analyzed. As far as facial function was concerned, this was assessed on the basis of a classification worked out by J.W. House and D.E. Brackmann. For the first series (93 patients tested, 86 followed up and 7 who completed and returned a form arranged from Brackma

Cisternal abnormalities produced by clinical tumours in the posterior cranial fossa. II. Fourth ventricle tumours.

A detailed analysis of the distortion of the subarachnoid cisterns produced by intraventricular tumours of the fourth ventricle was carried out on a clinical material of 30 patients examined with pneumography. The cisternal distortions in cases with a tumour originating from the roof of the ventricle have been compared with those in tumours originating from the floor, and the typical appearances for each group of tumours are recorded. The results should be applicable when analysing cisterns examined not only with pneumography but also with other radiologic methods, such as computer tomography

Coronal and sagittal reconstruction in computerised tomography.

As useful and dramatic as axial computed tomograms are, severe limitations have been encountered. Most of these involve the accurate location of masses and, as in angiography, projections in several planes are essential. Recent advances in both hard and softward allow three millimeter slice thicknesses at low radiation dosage. Coronal and sagittal reconstructions are practical and obtained with ease. The computerized tomogram is invaluable in complementing standard neuroradiological procedures and, in some cases, allows definitive diagnosis. This degree of accuracy is not possible without mult

High-Grade Biphenotypic Sinonasal Sarcoma: A Case Report.

Introduction  Biphenotypic sinonasal sarcoma (BSNS) is a recently found entity that first described by Lewis et al. It was then added to the 4th edition of the World Health Organization (WHO) of head and neck tumors in 2012. BSNS has been described as a rare low-grade sarcoma arising in the upper sinonasal tract. It is believed that in the past, BSNS was, likely, previously diagnosed as other low-grade or benign malignancies. Fibrosarcoma, leiomyosarcoma, and peripheral nerve sheath tumors, all fall within the differential diagnosis of BSNS. However, BSNS is unlike other mesenchymal sinonasal

Indocyanine Green-Assisted Endoscopic Transorbital Excision of Lateral Orbital Apex Cavernous Hemangioma.

Orbital apex lesions posed operative difficulties to neurosurgeons and ophthalmologists due to limited surgical corridor and close vicinity to cranial nerves and arteries. Lateral orbital apex lesions were traditionally operated via the transcranial route by neurosurgeons. Recently, only a handful of reports have described the use of endoscope alone for excision of lateral orbital apex lesion. Our group, with both endoscopic skull base neurosurgeons and oculoplastic surgeons, has adopted the endoscopic transorbital approach for orbital apex lesions. We also used an indocyanine green (ICG) endo

The endoscope-assisted ventral approach compared with open microscope-assisted surgery for clival chordomas.

BACKGROUND: The current management paradigm for clival chordomas includes cytoreductive surgery with adjuvant radiotherapy. Surgical approaches have traditionally utilized the microscope to remove these lesions through approaches that require extensive bone drilling, brain retraction, and mobilization of normal anatomy to create a suitably large corridor. The endoscopic ventral approaches provide a direct route to the tumor using natural orifices. Little data exist comparing these 2 surgical strategies. We conducted a systematic review of case series and case reports in hope of furthering our

Clinical course of abducens nerve palsy associated with skull base tumours.

BACKGROUND: The clinical course of abducens nerve palsy associated with skull base tumour is rarely reported. In this study, we examined the post-operative course of abducens nerve palsies associated with various skull base tumours. METHOD: Between January 2003 and December 2006, 240 patients with various skull base tumours underwent surgery at Kyushu University Hospital. Among them, nine patients presented with abducens nerve palsies (ten nerves) following surgery. The conditions included two pituitary adenomas, two trigeminal schwannomas and five meningiomas. We evaluated the function of th
