Genetic association of antinuclear antibodies with HLA in JIA patients: a Swedish cohort study.

BACKGROUND: Juvenile Idiopathic Arthritis (JIA) is a complex autoimmune disease and the most common chronic rheumatological disease affecting children under the age of 16. The etiology of JIA remains poorly understood, but evidence suggests a significant genetic predisposition. METHODS: We analyzed a Swedish cohort of 329 JIA patients and 728 healthy adult controls using the Illumina OmniExpress array for genotyping. HLA alleles were imputed from GWAS data using the SNP2HLA algorithm. RESULTS: Case-control analysis yielded 12 SNPs with genome-wide significant association to JIA, all located

Type 2 diabetes genetic loci informed by multi-trait associations point to disease mechanisms and subtypes: A soft clustering analysis.

BACKGROUND: Type 2 diabetes (T2D) is a heterogeneous disease for which (1) disease-causing pathways are incompletely understood and (2) subclassification may improve patient management. Unlike other biomarkers, germline genetic markers do not change with disease progression or treatment. In this paper, we test whether a germline genetic approach informed by physiology can be used to deconstruct T2D heterogeneity. First, we aimed to categorize genetic loci into groups representing likely disease mechanistic pathways. Second, we asked whether the novel clusters of genetic loci we identified have

Machine learning for enhanced prognostication: predicting 30-day outcomes following posterior fossa decompression surgery for Chiari malformation type I in a pediatric cohort.

OBJECTIVE: Chiari malformation type I (CM-I) is a congenital disorder occurring in 0.1% of the population. In symptomatic cases, surgery with posterior fossa decompression (PFD) is the treatment of choice. Surgery is, however, associated with peri- and postoperative complications that may require readmission or renewed surgical intervention. Given the associated financial costs and the impact on patients' well-being, there is a need for predictive tools that can assess the likelihood of such adverse events. The aim of this study was therefore to leverage machine learning algorithms to develop

Methods of diffusion MRI tractography for localization of the anterior optic pathway: A systematic review of validated methods.

The anterior optic pathway (AOP) is a system of three structures (optic nerves, optic chiasma, and optic tracts) that convey visual stimuli from the retina to the lateral geniculate nuclei. A successful reconstruction of the AOP using tractography could be helpful in several clinical scenarios, from presurgical planning and neuronavigation of sellar and parasellar surgery to monitoring the stage of fiber degeneration both in acute (e.g., traumatic optic neuropathy) or chronic conditions that affect AOP structures (e.g., amblyopia, glaucoma, demyelinating disorders or genetic optic nerve atroph

Genetically Predicted Gastroesophageal Reflux Disease and Common Thyroid Disorders: A Two-sample Mendelian Randomization Study.

INTRODUCTION: The causality between thyroid disorders and Gastroesophageal Reflux Disease (GERD) remains to be deciphered. This two-sample Mendelian Randomization (MR) study was performed to elucidate the causal association between GERD and thyroid diseases and functions. METHODS: Summary statistics for GERD were retrieved from a published GWAS dataset deposited in the Integrative Epidemiology Unit OpenGWAS database. Thyroid hormone level data were obtained from the ThyroidOmics Consortium, and genetic variants associated with thyroid disorders were sourced from the FinnGen Project. MR statis

Genetic Architecture of Idiopathic Inflammatory Myopathies From Meta-Analyses.

OBJECTIVE: Idiopathic inflammatory myopathies (IIMs, myositis) are rare systemic autoimmune disorders that lead to muscle inflammation, weakness, and extramuscular manifestations, with a strong genetic component influencing disease development and progression. Previous genome-wide association studies identified loci associated with IIMs. In this study, we imputed data from two prior genome-wide myositis studies and analyzed the largest myositis data set to date to identify novel risk loci and susceptibility genes associated with IIMs and its clinical subtypes. METHODS: We performed associatio

Cryptic Diversity and Ecological Overlap in Sporothrix schenckii: Insights from Multilocus Phylogenetics of Clinical and Environmental Isolates.

Sporothrix schenckii is a pathogenic fungus with both clinical and environmental origins that was traditionally described as a single species but is increasingly recognized as being genetically diverse. In this study, we analyzed multiple isolates recovered from human sporotrichosis cases and environmental sources across Latin America (Mexico, Guatemala, Colombia). We conducted a polyphasic analysis of 16 isolates, integrating morphological data with multilocus sequence analysis (MLSA) targeting the internal transcribed spacer (ITS), calmodulin (CAL), β-tubulin (BT2), and translation elongatio

Pathogenic variants in CFAP46, CFAP54, CFAP74 and CFAP221 cause primary ciliary dyskinesia with a defective C1d projection of the central apparatus.

BACKGROUND: Primary ciliary dyskinesia is a rare genetic disorder caused by insufficient mucociliary clearance leading to chronic airway infections. The diagnostic guideline of the European Respiratory Society primarily recommends an evaluation of the clinical history (e.g. by the PICADAR prediction tool), nasal nitric oxide production rate measurements, high-speed videomicroscopy analysis of ciliary beating and an assessment of ciliary axonemes via transmission electron microscopy. Genetic testing can be implemented as a last step. AIMS: In this study, we aimed to characterise primary ciliar

A model for prediction of spontaneous preterm birth in asymptomatic women.

BACKGROUND: Preterm birth is a complex health problem with social, environmental, behavioral, and genetic determinants of an individual's risk and remains a major challenge in obstetrics. Recent research has caused improvements in predicting preterm birth; however, there is still controversy about the prediction of preterm birth in asymptomatic women. The purpose of this study was to determine if Bayesian filtering can be used in a clinical setting to predict spontaneous preterm birth in asymptomatic women. METHODS: A model of predicting spontaneous preterm birth using PopBayes based on a Bay

Supratentorial cerebral cavernous malformations: clinical, surgical, and genetic involvement.

OBJECT: Although there is general agreement on the methods of treatment for symptomatic supratentorial cerebral cavernous malformations (CMs) located in noneloquent areas, some controversy exists regarding the management of cerebral CMs that are asymptomatic and/or located in eloquent or deep areas. Moreover, recent advances in genetic findings could influence both standard clinical management and the follow-up strategy in affected individuals. Thus, the objective of this study was to develop, based on the authors' experience and a literature review, a management algorithm to deal with suprate

A novel synonymous ABCA3 variant identified in a Chinese family with lethal neonatal respiratory failure.

BACKGROUND: Lethal respiratory failure is primarily caused by a deficiency of pulmonary surfactant, and is the main cause of neonatal death among preterm infants. Pulmonary surfactant metabolism dysfunction caused by variants in the ABCA3 gene is a rare disease with very poor prognosis. Currently, the mechanisms associated with some ABCA3 variants have been determined, including protein mistrafficking and impaired phospholipid transport. However, some novel variants and their underlying pathogenesis has not been fully elucidated yet. In this study we aimed to identify the genetic features in a

Design and Structure of Protein Cages Based on Helical Fusion and Machine Learning.

Self-assembling protein cages are versatile nanoscale architectures with broad applications in drug delivery, vaccine development, and structural biology. Historically, two main strategies have been used to construct such cages: genetic fusion of oligomeric domains connected by helical linkers, and computational interface design using either physics-based or machine learning-based methods. Here, we extend the original fusion approach using modern AI algorithms and more sophisticated treatments of helix bending to create protein cages with novel architectures composed exclusively of trimeric bu

Antineutrophil Cytoplasmic Antibodies: Integrating History, Pathogenesis, and Modern Diagnostics.

Antineutrophil cytoplasmic antibodies (ANCA) define a distinct group of small-vessel vasculitides collectively termed ANCA-associated vasculitis (AAV). Since their initial recognition as immunofluorescence patterns, ANCA have evolved into well-characterized pathogenic autoantibodies directed primarily against proteinase 3 (PR3) and myeloperoxidase (MPO). This review integrates the historical evolution of ANCA discovery with current understanding of disease pathogenesis and contemporary diagnostic practice. We summarize key biological mechanisms underlying AAV, including genetic susceptibility,

Proteomic Profiling of Nodal T-Follicular Helper Cell Lymphoma Reveals Distinct Biological Signatures and Improves Prognostic Stratification Beyond the IPI.

Nodal T-follicular helper cell lymphoma (nTFHL), most commonly of angioimmunoblastic type, exhibits heterogeneous clinical outcomes that are not fully explained by current clinical and biological variables. While genetic alterations have been extensively investigated, its proteomic landscape remains largely unexplored. Here, in a discovery cohort, the classical and alternative proteome of 63 nTFHL patients at diagnosis was profiled by mass spectrometry and analyzed using unsupervised k-means clustering. Two nTFHL signatures with distinct pathway enrichment and significantly different overall s

Reappraisal of Goal-Directed Medical Therapy in Diabetic Kidney Disease: Beyond the Quadruple Therapy Paradigm.

Diabetic kidney disease (DKD) affects 40% of individuals with diabetes and is the leading cause of chronic kidney disease worldwide. While the KDIGO guidelines have established quadruple therapy comprising renin-angiotensin system inhibitors, SGLT2 inhibitors, GLP-1 receptor agonists, and mineralocorticoid receptor antagonists as the cornerstone of DKD management, substantial phenotypic and molecular heterogeneity among patients limits the efficacy of standardized protocols. Emerging evidence highlights distinct DKD subtypes defined by inflammatory, metabolic, fibrotic, and vascular pathobiolo

Chronic endometritis and reproductive failure: The paradigm shift from microbial eradication to ecological restoration.

Despite advancements in assisted reproductive technologies, such as preimplantation genetic testing, clinical success rates have plateaued, thereby increasing the clinical focus on the endometrial factor. Chronic endometritis (CE), a persistent and often asymptomatic mucosal inflammation, is an underdiagnosed condition associated with reproductive failure, particularly in cohorts with repeated implantation failure (RIF) and recurrent pregnancy loss (RPL). This review examines the evolving understanding of CE pathophysiology, transitioning from a traditional infection model toward a framework i
